Subjective: Patient was friendly and cooperative,

Assessment of Goals/Response to Treatment: Alaina engaged in activities requiring two-handed use ( opening/closing containers, twisting lids, pulling apart snap containers). Demonstrated moderate difficulty coordinating both hands simultaneously, requiring verbal cues and occasional hand-over-hand assistance. Successfully opened 2/6 containers independently. She participated in prewriting tasks including vertical/horizontal lines and circular scribbles using crayons. Utilized a digital pronate grasp, transitioning briefly to a four-finger grasp with cues. Able to imitate vertical lines inconsistently and horizontal lines with moderate assistance; circles required hand-over-hand support. She then engaged in turn-taking play with therapist using toys. Demonstrated emerging skills in joint attention and reciprocal interaction, requiring prompts to maintain engagement and respond to social cues ( waiting, responding to name, sharing materials). Alaina participated in clean-up routine at end of session. Required moderate verbal prompting to initiate and complete task. Demonstrated ability to pick up and place toys in designated bins with guidance, showing increased participation compared to previous sessions. I wanted to confirm our evaluation date, which is April 9th, 2015, at 10:30 a.m. Our address is 3551 Rich Been, Suite B, 79938. Please complete this questionnaire no later than 6 p.m. It will help me determine which assessment I will be using. 1.) Name and DOB 2.) Any complications with pregnancy or birth? 3.) Any major illnesses (i.e., ear infections) or accidents since the last check-up? Any medical diagnoses (i.e., Autism, ADHD, Down syndrome, etc.) or allergies (i.e., medications, foods, seasonal, etc.)? Does the patient take any medication? 4.) When were these developmental milestones met (please be specific): crawling, sitting up, standing up, walking, feeding self, dressing self, potty training, first word, word combinations? 5.) Sleeping or breathing issues? Does the patient get 8-10 hrs of sleep? Wake up several times a night? Snoring? Sleeps with mouth open or closed? Enlarged tonsils? 6.) No medical diagnosis or known allergies and no medication 4. I dont remember when she started crawling and sitting up by herself, but it was within a normal range. She could pull herself up to stand around 10-11 months, standing by herself around 14 months and walking at 17 months. I don't remember when she started feeding herself, but it was pretty early on. She isn't potty trained yet, but we haven't started teaching her. She said dada around 8-9 months old. She can get dressed with assistance. She cannot said any word combinations, as she can only say a few actual words. She babbles a lot, but not many actual words. 5. No sleeping or breathing issues. She's always been a great sleeper! 6. She does not attend preschool or daycare 7. No hearing or vision issues. She has not had any recent hearing tests. 8. She needs help with saying actual words and talking to communicate. She only says a few actual words and then babbles to herself

Here is a brief summary of Alaina’s goals based on her performance in the language assessment, clinical observations, and the information you provided. We will work on sustaining attention to tasks for at least one minute and completing a functional or relational play-based task from beginning to end. We will imitate gestures/words modeled by the clinician, imitate nonspeech/environmental sounds, and follow routine directions.
04-10-2025 09:37 AM	Jessica Ortiz	-Here is a list of the rules and expectations we have for parents and patients of New Beginnings, which you can also view in your app: Parents are not allowed to leave the premises. This is for the safety of the patient. Parents are more than welcome to wait in the lobby, enter the therapy room, or wait in their car. The clinic has a policy requiring 90% attendance from patients. When patients attend their sessions regularly, we are able to see progress toward their goals. A patient will be discharged if they miss 3 consecutive sessions without notifying their therapist. In case of an emergency, we allow a courtesy “hold” of up to 4 weeks. If this time is exceeded and the parent does not contact the therapist or the office, the patient will be discharged. The clinic has a zero-tolerance policy for any kind of abuse or insults directed at clinic staff. If you need any type of document from the office, please let us know at least 7 days in advance. Food is not allowed in the lobby or therapy rooms unless the patient is receiving feeding therapy. IT IS VERY IMPORTANT THAT YOU INFORM THE OFFICE OF ANY CHANGES TO YOUR INSURANCE. It is important that you communicate with your therapist if for any reason you are unable to attend therapy (emergency, the patient is sick, etc.). Parents are asked to notify their therapist in advance, preferably the day before, as this allows us to offer the session to another patient. In the app, you also have the option to notify your therapist if you are running late, etc. If you have any questions, please let me know.:)
