Speech Assessment Summary: David is a 4 year, 10 month old male that has been receiving speech and language therapy at New Beginnings Pediatric Therapy Services and was re-evaluated to determine the need for continued services. The re-evaluation took place in clinic room with parent present. Moderate to maximum prompts required to assist with cooperation and participation during assessment. Patient observed to be upset at times during re-evaluation. However, at times was able to be redirected. During expressive communication portion of assessment, David demonstrated refusal to participate at times despite parent and clinician assistance with redirection. Behaviors observed during assessment could have contributed to a decrease in scores. During assessment, David was evaluated using the Preschool Language Scales Fifth Edition. On the PLS-5, David obtained the following scores: Auditory Comprehension 81 (raw score: 43, percentile rank: 10, age equivalent: 3-9), Expressive Communication 69 (raw score: 34, percentile rank: 2, age equivalent: 2-9), and Total Language 74 (raw score: 150, percentile rank: 4, age equivalent: 3-3). Based on David’s scores on the PLS-5, David presents with a MILD RECEPTIVE and SEVERE EXPRESSIVE LANGUAGE DISORDER. Since previous re-evaluation, David has made measurable progress as evidenced by an increase in raw scores and having met all targeted therapy goals. Based on overall assessment, speech therapy continues to be warranted to target functional communication which will promote safety and independence due to David’s high safety risk. Parents will remain responsible for implementing the Home Management Intervention Program (HMIP).

When labor time came, body didnt progress and when contractions hit, babys heartrate wavered. Cesarean birth. Passed newborn screening. 3. No major illnesses. No medical diagnoses. Just seasonal allergies when the wind and dust gets bad. 4. Mostly a developmental milestones were met at appropriate ages, only thing that has had a hard time with is the potty training. 5. Sleeps great. Starting to notice more mouth breathing. Gets 10 hours of sleep every night. No notice on enlarged tonsils. 6. No daycare or preschool. 7. No issues with vision or hearing. 8. My son can understand everything we tell him but he just had a hard time explaining what he wants. We can tell he is right there is telling us but is having trouble finding the words for it. He is a very shy kid when you first meet him but when you sit with him after a few minutes he will start to open up. In a year, I would like to have full conversations with my son about the things he likes and wants to do.
