Speech Background History: Lukas is a 1-year, 7-month-old male who was referred for a speech and language evaluation by his pediatrician due to concerns regarding speech delay. His parents served as the primary informants during the evaluation and reported that Lukas primarily produces sounds rather than fully formed words, with inconsistent use of communicative intent. Currently, Lukas is reported to have an expressive vocabulary of approximately five words. Clinically, his communication was observed to consist mainly of vocalizations and nonverbal behaviors such as pulling an adult’s hand to indicate wants or needs. His speech and language abilities were assessed using the Preschool Language Scales, Fifth Edition (PLS-5), English version. Based on Lukas’ language exposure and use, English was determined to be the appropriate language for intervention.
Speech Medical History: Per parent report, medical history is not remarkable. Lukas was born at 41 weeks by emergency C-section due to being stuck in mother's pelvis during vaginal delivery. Parents reported Lukas was in the NICU for approximately 5 days total due to being in shock at the time of birth, requiring oxygen, and being lactose sensitive at the time of discharge. Parents confirmed that Lukas is no longer lactose sensitive.

He participated in structured therapeutic activities targeting foundational play and socialization skills. The patient demonstrated emerging progress toward completing play tasks from beginning to end with the clinician, achieving success in 3 out of 10 opportunities (30%), which is foundational for increasing joint attention and creating opportunities for language modeling. Expressive language was addressed by targeting gesture imitation, with the patient successfully imitating clinician models in approximately 2.9 out of 10 opportunities (29%). Further work on expressive communication involved the use of a total communication approach to express wants and needs; the patient initiated functional requests using gestures and/or word approximations without adverse behaviors in 3 out of 10 opportunities (30%). Any major illnesses (i.e., ear infections) or accidents since last check up? Any medical diagnoses (i.e., Autism, ADHD, Down syndrome, etc.) or allergies (i.e., medications, foods, seasonal, etc.)? 4.) When were these developmental milestones met: Crawl, sat-up, stood up, walked, fed self, dressed self, potty trained? 5.) Sleeping or breathing issues? Does patient get 8-10 hrs of sleep? Wake up several times a night? Snoring? Sleeps with mouth open or closed? Enlarged tonsils? 6.) Education history (i.e., attends daycare, preschool, etc.)? 7.) Vision or hearing issues? 8.) In a few words write your parental concerns with patient’s speech and/or language, and how you would like it to improve within a year.

During labor my wife was not able to push him out and his head got stuck in the birthing canal so we ended up having to have a C-section. 3.) No major illnesses. No medical diagnoses. Was sensitive to lactose as a baby but seems to tolerate it fine now. 4.) Crawled at about six months. Sat up around same time. Started standing up about 10 months. Walked at about a year. Learning to feed himself now but does okay. Does not dress himself, not potty trained yet. 5.) No unusual sleeping issues. No breathing issues. Sleep varies. No Snoring. Sleeps with mouth closed. No tonsil issues. 6.) No education history. 7.) No Vision or hearing issues 8.) essentially our concern is although he does make a lot of different sounds and at times they do sound like words we are concerned that he is not speaking more at this age.

04-14-2025 03:50 PM	Jordan Orozco	Hello, as discussed, I am sending a brief summary from today’s evaluation. Lukas is diagnosed with a mixed receptive expressive language disorder. His goals will focus on completion of tasks from beginning to end, following directions with common objects, imitating gestures, imitating speech sounds or monosyllabic words, and use of total communication to express wants/needs. I do recommend an OT consult to determine if Lukas can benefit from additional support with his fine motor skills and attention/completion of tasks. Typically you would need to call his pediatrician to request for an OT referral to be sent to new beginnings just like the speech referral was. It was a pleasure meeting you all today and Lukas’ assigned therapist will reach out when she receives clearance to begin therapy. :)
