Ms Peña reported that Axel has struggled with his feeding since he was 18 months old. Axel reportedly does not eat many foods and will only eat the foods he likes. Ms Peña also stated concerns regarding Axel’s tongue mobility when eating foods. Axel has history of therapy services in which he is diagnosed with sensory delay, speech delay, and oral apraxia. A bedside swallow evaluation and skilled mealtime feeding observation were completed to determine the need for therapy. LANGUAGE OF INTERVENTION: English.
Feeding Medical History: Per parent report, medical history is not remarkable. TOTs released at 4 years old Sleep study completed at 4 years old; pending another sleep study. Sleep study findings came back in the mild range and no surgery was required.

Axel’s mother served as the informant for this evaluation. Ms Peña reported that Axel has struggled with his feeding since he was 18 months old. Axel reportedly does not eat many foods and will only eat the foods he likes. Ms Peña also stated concerns regarding Axel’s tongue mobility when eating foods. Axel has history of therapy services in which he is diagnosed with sensory delay, speech delay, and oral apraxia. A bedside swallow evaluation and skilled mealtime feeding observation were completed to determine the need for therapy. LANGUAGE OF INTERVENTION: English. ORAL MECH EXAM: Although Axel has structural differences in his oral cavity, the overall structure is currently adequate for feeding and swallowing. However, the function of Axel’s oral cavity is impaired, which affects his ability to properly chew and swallow solids and liquids. Structurally, Axel has a high, narrow palate and large teeth relative to his oral cavity size. Functionally, Axel demonstrates decreased lingual elevation and lateralization, reduced coordination of the tongue, and weakened labial protrusion and closure. Additionally, there is a reduction in both lingual and labial strength. Axel also presents with a hypersensitive gag reflex and rooting reflex on the right side of his face. These functional impairments require therapeutic intervention to ensure safe and efficient feeding and swallowing. It is important to note that: Axel had his tethered oral tissues released at 4 years old, and this may be attributed to his decreased oral function. FEEDING/SWALLOWING EXAM: A bedside swallow evaluation and skilled meal feeding observation revealed that Axel has a CHRONIC PEDIATRIC FEEDING DISORDER and ORAL DYSPHAGIA. These disorders are characterized by impaired oral intake that is not age-appropriate and is associated with multiple factors, including medical diagnoses (oral apraxia, constipation, allergies), nutritional status (limited food variety), feeding skills (sensory difficulties, oral motor difficulties), and/or psychosocial factors (problematic mealtime behaviors, selective/restrictive eating). FOOD INVENTORY: Axel’s mother reported that he consumes 7 foods and 5 liquids, including the following: Proteins: None. Starches: Crackers, donuts/cake, cookies, dry cereals, French fries. Fruits: Banana, watermelon. Vegetables: Corn on the cob. Liquids: Apple juice, milk, vanilla supplement, water. Axel prefers crunchy, crisp, or smooth foods with sweet, salty, and bland flavors. He has an overall poor appetite. Axel’s mother would like him to consume more sopitas, pancakes, pastas, and rice. NORM REFERENCED ASSESSMENT: Ms. Peña completed the Pediatric Eating Assessment Tool (PediEAT), which is used to subjectively assess feeding behaviors in children ages 6 months to 7 years. Although Axel is older than the intended age range, his scores provide useful descriptive comparison. Axel received a “high” rating for physiological symptoms, problematic mealtime behavior, and selective/restrictive feeding behaviors, which align with his performance in the evaluation. According to Axel’s mother, he takes longer than 30-45 minutes to finish meals and struggles to sit down during mealtime with the family. MEAL TIME OBSERVATION: Axel sat at the feeding therapy table. When presented with his preferred foods (animal crackers, Ritz crackers, and gummies), Axel initially refused all trials but eventually consumed only the gummies. When non-preferred foods (turkey, cheese, and strawberries) were offered, Axel looked at them but did not interact. With support from the clinician, Axel touched the non-preferred foods with his fingers, hands, and oral cavity. He took a small bite of strawberry but appeared scared afterward and refused further interaction with the non-preferred foods. SWALLOW/ORAL MOTOR EVALUATION: Axel ate his gummies using a munching chew pattern with poor tongue lateralization. He demonstrated an open-mouth chew and forward tongue protrusion while chewing. There was also decreased anterior-posterior transfer of the bolus. Currently, no concerns were observed regarding the pharyngeal or esophageal phases of swallowing. Axel has undergone two swallow studies, both of which showed findings within functional limits. Ms. Peña reported on the PediEAT that Axel sometimes coughs during food intake, throws up during mealtime, and demonstrates wet, gurgly vocal quality. These issues were not observed during the evaluation, possibly because he only consumed gummies at that time. Ms. Peña has not reported unexplained fevers or a history of aspiration pneumonia. Axel’s swallow function should be closely monitored by his treating clinician to assess if further intervention is needed. RECOMMENDATIONS: At this time, feeding therapy at the rate of 1x per week for 30 minutes is recommended to treat Axel’s CHRONIC PEDIATRIC FEEDING DISORDER and ORAL DYSPHAGIA. Axel’s health and development are at risk due to his limited diet. At this age, Axel should have 30+ foods in his food inventory with at least 10 foods per food category (i.e., fruits, vegetables, proteins, etc.). Therapy should target increasing Axel’s tolerance to non? preferred foods via steps to eating hierarchy and desensitization methods, as well as improved family mealtime behavior/routine and parent education on pediatric feeding disorders. Axel’s therapy should also target oral motor exercises to support his oral function for adequate feeding/swallowing. Without feeding therapy, safety risks include but are not limited to: lack of appropriate nutrition due to a restricted diet characterized by foods of inappropriate nutritional value and food jagging on limited foods in his inventory. Axel’s mother will be responsible for his Home Management Intervention Program (HMIP).

Then on 3/5/25 Bertha called us and stated in order for them to give us an authorization for feeding is that the parent would write a letter stating that she agreed to go down to 1x week for speech at Action Care since they have a 52 visits granted for speech therapy and wanted to come 1x week for feeding at new beginnings. Parent was notified and parent agreed, parent brought the letter signed and we faxed in to EPH on 3/06/2025 for feeding authorization.

His swalllowing has been good, but my biggest concern is his eating. He doesn't eat anything. The only things he does eat is ice cream, chocolate, Crackers, French fries (not often) cookies, cereal(not often). I have noticed how he doesn't use his tongue as much as we all should. His last reevaluation for speech at action care the therapist mentioned him having Oral Apraxia. Idk if that will be helpful for you.

Suck-swallow-blow activities: Use tools like straws, whistles, bubbles, and chewing tubes/straws to strengthen oral motor skills. Oral mapping: Use a finger or banana inside the mouth and around the lips to improve body awareness and desensitize hypersensitive areas. You can also use the banana to work on the tongue lateralization. Thank you, and I will see him next week!
