Assessment of Goals/Response to Treatment: The patient presented as alert, oriented, and was a cooperative participant in all therapeutic activities. Significant progress was noted in the development of foundational play and social skills, as the patient demonstrated the ability to sustain attention to clinician-led, play-based tasks for 30-45 seconds in 9 out of 10 opportunities. This consistent performance exceeded the established target and facilitated increased opportunities for language learning. Continued gains were also observed in the area of expressive language precursors, where the patient successfully imitated clinician-modeled gestures in 8 out of 10 opportunities, demonstrating emerging intent for functional communication. Subjective: Patient was friendly and cooperative,

Assessment of Goals/Response to Treatment: Enzo increased bilateral coordination when successfully lacing beads IND, cutting play-doh with MOD tactile A, and utilizing bilateral hands to stabilize/manipulate blocks to create 3D structures with MIN A. He also demonstrated bilateral coordination during a gross motor task when as dribbling a ball with bilateral hands with MIN A for compensations. Enzo attended to and completed a 3 step familiar task with play-doh with MOD VCs, to improve task sequencing and attention during daily routines. He also required MOD A to imitate a 3 block structure to increase task sequencing, VMI, and problem solving skills. Enzo manipulated play-doh with bilateral hands with only MIN aversion initially. After verbal encouragement and modeling, Enzo engaged with the play-doh with no aversion to increase sensory integration required for feeding and self-care tasks.

He has fed himself at month 7. He is not potty trained and does not dress himself. 5. He sleeps the right amount of hours. Mouth open. He had a tongue and lip tie procedure done 6. He does not attend day care. 7. No vision or hearing issues 8. Enzo mumbles a lot and say little to no words. I would like for him to speak a little more because he's missing his speech milestones. I see children his age speaking a lot compared to him. It's concerning and hard to see

As discussed, here is a brief summary of Enzo’s performance today: Enzo has been diagnosed with a mild to moderate mixed receptive-expressive language disorder. He demonstrates strengths in his awareness of others/environment, use of eye contact, purposeful vocalizations to express himself, and attempts at relational/functional play. Enzo does require support in areas such as maintaining sustained attention to tasks, completing play tasks, following commands with common objects, imitating gestures, imitating speech sounds, and using functional communication to express his wants and needs. His therapy goals will be focused on addressing these areas of need. I recommend that Enzo receive a formal occupational therapy evaluation to help support his attention and self-regulation needs. In six months, we will reassess his progress and determine if further support from other professionals is necessary. In the meantime, please continue to sing songs with Enzo, narrate his actions when his attention is sustained during play, and offer hand-over-hand support for gesture use. 05-15-2025 04:48 PM	Janette Gomez	We asked the front office about the Hold, and they did say that a Hold is for 30 days, but that since he's been out of town for 2 weeks, he can be placed on Hold for 2 more weeks. After the 2 weeks, we can resume therapy. If he doesn't resume his therapies we would have to discharge him. He can always come back, he would just need to re-start the process again with the initial evaluations in order to begin therapy. If you can please let us know what you would like to do :) Thank you!
