Speech Medical History: Emma was born at 36 weeks via a twin birth. Mother stated that Emma was in the NICU since Emma was not eating, she had insufficient oxygen, her sugar was low, and she was not finishing her bottles "fast enough." About 2 years ago, Emma had an ear infection. Mother stated that the antibiotics that were initially prescribed were not working.

Speech Assessment Summary: Emma is a 6 year old, 3 month old female that has been receiving speech and language therapy at New Beginnings Pediatric Therapy services. She was re-evaluated to determine need for continued services. Emma was re-evaluated using the Preschool Language Scales Fifth Edition (PLS-5) and the Clinical Assessment of Articulation and Phonology Second Edition (CAAP-2). Emma’s language skills were evaluated using the PLS-5 and revealed the following standard scores: Auditory Comprehension 124 (raw score: 65, percentile rank: 95, age equivalent: >7-11), Expressive communication 97 (raw score: 60, percentile rank: 42, and age equivalent: 6-0), and Total Language 111 (raw score: 125, percentile rank: 77, and age equivalent: 6-11). Scores between 85-115 indicate language skills within functional limits. Based on assessment results, Emma’s language skills are within functional limits for age. However, based on clinical observation Emma presents with difficulty using pronouns/nouns and possessive pronouns/nouns when formulating grammatically correct sentences and during conversation. Despite Emma’s language skills being within normal limits, this area of concern is crucial for Emma’s functional communication and goals will be created to target area of need. Continued monitoring and re-evaluation of language skills will continue to be warranted as needed. Emma’s articulation and phonological skills were assessed using the CAAP-2 and revealed the following standard score: 63 (raw score: 12, percentile rank: 2, age equivalent: 4;6). Based on score reported above, Emma presents with a SEVERE PHONOLOGICAL/ARTICULATION DISORDER. During assessment, Emma was observed to make following errors: omission of /ng/ in final word position, substitution of /th/ in initial/final word position, substitution of /l/ in final word position, phonological process of vowelization and gliding. In her language skills, Emma has demonstrated increase improvements as evidenced by an increase in PLS-5 raw scores and standard scores. However, it is observed that Emma still requires assistance with use of pronouns/nouns and possessive pronouns/nouns during sentence formulation and during conversation. In her articulation skills Emma, has made noticeable gains as evidenced by a reduction in errors during assessment. Although current standardized severity rating reflects a decrease, this change is partially attributed to the patient transitioning into a higher age range for scoring. However, patients raw score improvement demonstrates functional progress. Emma has also made measurable progress as evidenced by meeting 4/5 targeted goals. Despite functional progress, therapy should continue to target functional expressive language concepts as well as age-appropriate speech sounds to help support Emma’s functional communication. Based on overall assessment, speech therapy continues to be warranted. Based on previous re-evaluation, it is recommended that Emma’s speech session range from 30-45 minutes based on mode of delivery. 30-minute sessions should be advised for telehealth sessions, as observed by Emma’s distractibility resulting in decreased participation during virtual sessions. In person sessions should remain 45 minutes to maximize therapy session which allows more time to target all session activities required. Speech therapy still warranted to promote Emma’s safety and independence due to her safety risk. Emma’s parents to remain responsible for implementing the Home Management Intervention Plan (HMIP).

Tricare is currently experiencing technical difficulties with their technology systems and initial and re-authorizations. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications. We still have time on her old authorization so we will continue with our regular therapy sessions on Tuesdays. Her new therapy plan will focus on speech sounds L, SH, CH, and use of possessive nouns/pronouns. :)

Her receptive and expressive language skills are still within the average range. During observation I still do see she has a bit of difficulty with possessive pronouns during sentences. In the articulation examination, Emma had a decrease in errors on evaluation. During this new authorization period we will be focusing on: formulating sentences using possessive pronouns OR possessive nouns, producing /l/ sound in initial word position at the phrase/sentence level, producing /l/ sound in final word position at the phrase level, producing voiced /th/ sound in initial and final word position at the word level, and producing voiceless /th/ sound in initial and final word position at the word level. We targeted her /l/ sound at the beginning and at the end of the word in sentences. She did good there were a few instances where she required some modeling. We also worked on her sounds /Th/ voiced and voiceless both at the beginning and at the end of the word at word level.
