Speech Background History: Christian is a 5 year 2 month old who came into the offices with mother and younger sister, with a referral from the pediatrician. Mother served as informant for this report in answering questions concerning language, speech, and development for Christian (CJ), Mother reports that CJ failed his New Born Hearing Screening and it took at least 18 months after birth before any services were implemented for CJ. Paperwork was difficult and did not help to get things in motion for CJ. After his second fail in hearing test, hearing aids were attempted but CJ did not progress with language or listening and it was decided by Cochlear Implant team in Washington for CJ to be implanted at 2 years old. Mother reports because they are a military family CJ lost time during moves and it was difficult to get him into therapy. CJ has been in and out of Auditory-Verbal therapy and his last therapist in California shared that CJ made great progress with onomatopoeias and was beginning to use words while working with her. It was decided that CJ would take the Comprehensive Assessment of Spoken Language-Second Edition (CASL-2), as family speaks English in the home and CJ listens to videos and parents voices in English and attempts to repeat what they say. Evaluating therapist is a Listening and Spoken Language Specialist for Auditory-Verbal Therapy and attempted a Speech Intelligibility test with the Goldman-Fristoe Test of Articulation-Third Edition.
Speech Medical History: Per parent report, medical history is not remarkable. CJ has recently gone through the RSV virus and has had no other issues with his health. He failed his New Born Hearing Screening and received Hearing Aids at 18 months for use of his residual hearing, only later to be implanted with bilateral Cochlear Implants.

CJ did not understand and could not follow directions given. Sign language was limited as well as verbal output but he enjoyed play and looking at pictures with therapist. Any onomatopoeias he knew, he imitated and used sign language he knew to help support what he was attempted to say. Rapport was built quickly with CJ and therapist.

CJ was unintelligible while naming items was done. CJ can name colors and count to 5, although to an unknown listener, it would not be understood without support. A standard score could not be determined for CJ. Christian (CJ) was given the Comprehensive Assessment of Spoken Language-Second Edition by Auditory-Verbal means upon first presentation and Signed Exact English (sign language) upon the second presentation. CJ was able to point to accurate picture when assessment begin but soon relied on the signs for words in order to show understanding by pointing to picture in a field-of-four. Christian's raw score on the Receptive Language sub-test was 8 as he listened and showed understanding by point to simple CORE words that labeled common household items. i.e. CAT, DOG, CUP. Christian's Standard Score for his Receptive Language is 59, at least 3 standard deviations below that of average score and below that of his same-aged hearing peers. Christian's (CJ) raw score on the Expressive Language sub-test was 2 as he actually used two real words by pointing and naming from a picture scene. On other items, CJ was able to use some onomatopoeias to convey what he knew and then imitated therapist. Christian's Standard Score for his Expressive Language is 60, at least 3 standard deviations below that of average score and of his same-aged hearing peers. STRENGTHS: CJ is able to name a few items using CORE Words and his voice. When Christian's standard score on a standardized language assessment is within at least one standard deviation of average development, than Christian will no longer be in need of speech therapy services.

Speech Assessment Summary: Christian “CJ” is now 6 years 4 months old and is due for a review of language growth and improvement, as well as an update on the use of his auditory acuity for listening and language since these last 6 months of speech and language services. Mother has noticed a significant growth with Christian (CJ), as he is increasing the use of verbal output, supplemented with the sign of a word to aid in his expressive communication wants/needs. Christian (CJ) was given the Comprehensive Assessment of Spoken Language-Second Edition (CASL-2), and it will continue to be used as the tool of assessment to measure growth and progress since these last 6 months of speech therapy. Christian (CJ) participated fully in the assessment, but needed numerous prompts and/or redirection to attend and/or return to the assessment. Christian (CJ) participated fully in the assessment, and continued to need prompts and/or redirection to attend and/or return to the assessment. Christian continued to utilize signs to support his verbal output, but decreased the number of signs used as compared to the previous re-evaluation as Christian increased his use of verbal output during this assessment. Christian pointed to accurate pictures and selectively utilized sign to support his verbal output. Christian received the following scores on the CASL-2: Receptive Language sub-test raw score was 35, as he listened and showed understanding by pointing to simple CORE words that labeled common household items (i.e., cat, dog, cup). Christian’s standard score for Receptive Language was 92 (percentile: 30, age-equivalent: 5;6-5;8). Christian displayed continual steady gains with listening and understanding to what is needed. On the CASL-2: Expressive Language sub-test, Christian received a raw score of 17 as he could tell and give information about the pictures he saw in the assessment. Christian’s Standard Score on Expressive Language is 81 (percentile: 10, age-equivalent: 4;6-4;8). Christian has continued to make continual gains as he has increased his use of verbal output. Christian’s Standard Score on Expressive Language is at least 1.5 STANDARD DEVIATION BELOW that of average score and BELOW that of his same-aged hearing peers. On the CASL-2: Sentence Expression sub-test, Christian received a raw score of 5 as he could tell and give simple information about the pictures he saw in the assessment. Christian’s Standard Score on Sentence Expression is 63 (percentile: 1, age-equivalent: less than start age). Christian’s Standard Score on Sentence Expression is at least 2 STANDARD DEVIATION BELOW that of average score and BELOW that of his same-aged hearing peers. On the CASL-2: Sentence Comprehension sub-test, Christian received a raw score of 7, as he listened and showed understanding by pointing to simple CORE words that identified common core vocabulary words and were in response to simple questions. Christian’s Standard Score on Sentence Comprehension is 68 (percentile: 2, age-equivalent: less than start age). Christian’s Standard Score on Sentence Expression is at least 2 STANDARD DEVIATION BELOW that of average score and BELOW that of his same-aged hearing peers. STRENGTHS: Christian will name, label and request items using CORE Words and his voice; labeling and commenting increasing his use of attributes (i.e., color, size, shape). He has continued to increase his ability to follow therapist and understands social greetings and uses sign language and gestures for communication support at this time. Christian will verbalize 1-2 word phrases utilizing common core vocabulary words, and has increased his use of multisyllable words. He gains attention by including names of the desired item or using the phrase “over there” or “I know” as he gestures toward the item, as well as grabbing, and pointing. DIFFICULTIES: Christian does not consistently make his wants and needs known with intelligible words, rather uses a mixture of 1-2 word phrases mixed with word approximations, aided by pointing and gesturing to his communication partner. He still cannot verbalize clear ideas and/or use 2+ words to comment in conversation unless guided by communication partner, but more independence continues to be noted. Christian required constant and consistent redirection. The Goldman-Fristoe Test of Articulation Third Edition (GFTA-3) English was attempted once again to evaluate Christian’s speech production. Although Christian named all objects on his own without support, the articulation test was stopped before the Sounds-in-Sentences sub-test. A standard score was determined for the Sounds-in-Words sub-test. Christian received the following standard score: Sounds-in-Sentences 40 (raw score/errors: 78, percentile: <0.1, age-equivalent: <2 years 0 months). Christian was estimated, by this examiner, to be 50-60% intelligible. A typically developing child should be 75% intelligible or higher to familiar and unfamiliar communication partners who are of the same age. Christian was observed to have difficulties with multisyllable words, weak syllable deletion, and final consonant deletion. Christian (CJ) now presented with a MIXED MILD TO SEVERE RECEPTIVE/EXPRESSIVE LANGUAGE DELAY, as well as SEVERE ARTICULATION DELAY secondary to his hearing loss. Speech therapy with Auditory-Verbal strategies to support his language development is showing progress and continues to be warranted. Without Speech therapy, Christian’s safety risk and functional health risk is HIGH, as well as his independence; in that he cannot convey that he needs help or assistance if in need/ill, or danger. He also would not understand those who are giving him direction to move to a location or give help without a communication partner to guide and use sign language to support his needs. 1. Appointment Scheduling & Cancellations:
To provide the best care for all our clients, we kindly request at least [24 hours] notice for cancellations or

3. Billing & Payment:
Please ensure all insurance information is correct with our front office for billing.

For those with private insurance, your yearly deductibles and co-pays started anew on January 1, 2025.
If you have questions, you are welcome to contact your insurance to learn more about your particular
Healthplan benefits.
4. Policies:
Do not leave our premises while your child is in therapy for any reason
We require 90% attendance for all therapies, make-ups are necessary
Children under 6 must be accompanied to the restroom
Co-pays are due at the time of service

We wanted to take a moment to keep you informed about an issue that may affect your experience with us. Tricare is currently experiencing technical difficulties with their technology systems and initial and re-authorizations. As a result, there may be temporary delays in authorizations for your child’s therapy.
Please know that we are closely monitoring the situation and working with Tricare to resolve the issue as quickly as possible. They have stated a need of 4-8 weeks for their system issues to be resolved as their subcontractor insurance switched from Healthnet Federal Services to Triwest Alliance on 01/01/2025.

We sincerely apologize for any inconvenience this may cause and appreciate your understanding during this time.

As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

I would like to set up Christian Jackson's initial evaluation if possible. Do you have time next week? I know school is out but if you will be busy with preparations for Thanksgiving, I understand. I have a morning opening on Wednesday at 9:00AM, and if you prefer afternoon, please let me know. We will be open Monday/Tuesday/Wednesday next week - or, we can look at afternoon times the first week in December. The initial evaluation shouldn't take more than 45 to 60 minutes, depending on how well Christian Jackson does. Let me know when you have a moment and I will work with you on scheduling as best I can. Thank you
