Speech Medical History: Jaxxon was born prematurely at 34 weeks via c section. Jaxxon is a twin and spent 14 days in the NICU due to premature birth. No further complications arose after birth.

According to Jaxxon’s standard scores on the PLS 5, Jaxxon presents with a MODERATE EXPRESSIVE LANGUAGE DISORDER. Jaxxon would greatly benefit from speech therapy services that address the following functionally appropriate concepts: imitation of gestures, imitation of speech sounds, naming/labeling via word approximations, and use of functional communication to express wants/needs. Speech Assessment Summary: Jaxxon is now 4 years 0 months old and is due for a review of his speech and language skills since these last 6 months of speech therapy services. Jaxxon’s current communication was characterized as 3-4 word phrases, mixed with word approximations that were considered highly unintelligible. A typically developing child that is Jaxxon’s age should have approximately 500+ words in their expressive vocabulary and should be 75%- 100% intelligible. Jaxxon has incorporated leading behavior, pointing/gestures to aid his expressive communication wants/needs. According to mother, Jaxxon has made noticeable gains with his expressive communication not only at home, but at school and confirmed by his teacher. Jaxxon required moderate redirection during the assessment, as well as verbal, visual, and gestural cues to engage in the task. Jaxxon was evaluated using the Preschool Language Scales 5th Edition English version (PLS-5) and it will continue to be used as this was the initial assessment tool used. Jaxxon received the following standard scores on the PLS-5: Expressive Communication 88 (raw score: 41, percentile: 21, age-equivalent: 3 years 7 months). According to the standard scores on the PLS-5, where a score from 85-115 is considered within functional limits, Jaxxon NO LONGER presented with a moderate expressive language disorder. His language skills are now WITHIN FUNCTIONAL LIMITS for a child his age. Due to Jaxxon’s highly unintelligible speech production, Jaxxon’s speech was assessed with the Clinical Assessment of Articulation and Phonology Second Edition (CAAP-2) to measure his speech production on phonemes in words. Jaxxon received the following score on the CAAP-2: Consonant Inventory Score <55 (raw score: 62 errors, percentile: <1, age-equivalent: <2 years 6 months). Jaxxon’s standard score was reported to be BELOW AVERAGE for his age. According to the standard score that Jaxxon received on the CAAP-2, where a score from 85-115 is considered within functional limits, Jaxxon presented with a PROFOUND PHONOLOGICAL PROCESS/ARTICULATION DISORDER. Jaxxon was observed to perform the following processes: cluster reduction, syllable reduction, substitution processes: fronting in the medial and final word positions, fricatives, stopping in final word position, deaffrication of /j/, vowelization of /l/, and gliding of /l, r/ in all word positions. At this time, continued speech therapy is warranted to continue progress with production of phonemes in words and in Jaxxon’s conversation. Without speech therapy, Jaxxon’s SAFETY RISK is HIGH since he will likely not be understood if he needs to communicate pain or the need for assistance. Jaxxon’s mother will be responsible for his Home Management Intervention Program (HMIP).
