LANGUAGE: The following are Jameson’s standard scores on the PLS 5 - Auditory Comprehension 90 (percentile: 25, age equivalent: 2 years 4 months), Expressive Communication 77 (percentile: 6, age equivalent: 1 year 8 months), and Total Language Score 82 (percentile: 12, age equivalent: 2 years 0 months). According to Jameson’s standard scores on the PLS 5, where a score from 85 to 115 is considered within functional limits, Jameson presents with a MODERATE EXPRESSIVE LANGUAGE DISORDER. Jameson would greatly benefit from speech therapy services that address the following functionally appropriate concepts: imitation of speech sounds, naming/labeling, imitation of 1-2 word phrases, and use of functional communication to express wants/needs. Jameson’s receptive language skills will also be targeted to support her continued language development. Speech Assessment Summary: Jameson is a 4 year, 1 month old male that has been receiving speech and language therapy services at New Beginning Pediatric Therapy services and was re-evaluated to determine the need for continues services. The re-evaluation took place in clinic room table where maximum redirection was required in order to improve cooperation and participation. It was observed that Jameson was highly distracted due to Jameson getting out of seat and playing with chair and running around the room. During assessment, Jameson was evaluated using the Preschool Language Scales Fifth Edition. On the PLS-5, Jameson obtained the following scores: Auditory Comprehension 79 (raw score: 38, percentile rank: 8, age equivalent: 3-2), Expressive Communication 69 (raw score: 31, percentile rank: 2, age equivalent: 2-4), and Total Language Score 73 (raw score: 148, percentile rank: 4, age equivalent: 2-4). Based on reported scores, Jameson presents with a MILD RECEPTIVE/ SEVERE EXPRESSIVE LANGUAGE DISORDER. During re-evaluation, formal articulation assessment attempted. However, formal assessment unable to be completed due to patient refusal to participate. Due to refusal, PLS-5 articulation screener completed to gain further insight on current speech production. Developmentally appropriate sounds will be focused on in order to support his expressive language growth. Since previous re-evaluation, Jameson has made measurable progress as evidenced by an increase in raw scores and by meeting all targeted therapy goals. Based on overall assessment, speech therapy continues to be warranted due to Jameson’s high safety risk. Speech therapy warranted in order to target functional communication which will promote safety and independence. Parents will remain responsible for implementing the Home Management Intervention Program (HMIP).

Tricare is currently experiencing technical difficulties with their technology systems and initial and re-authorizations. As a result, there may be temporary delays in authorizations for your child’s therapy. We do have a permanent slot we can offer which is Mondays at 1230pm and that can begin next week 11-18-24. Beginning Dec 2 we can resume our Mondays at 1230pm for the kids. I hope this all makes sense.

I am writing to inform you that I have been asked to supervise a Speech-Language Pathology (SLP) graduate student for the upcoming Spring 2025 semester. This will be my second year teaching and mentoring graduate students. Angela Valera, a graduate student from Yeshiva University in New York, will be working with me as part of her clinical training. She is set to graduate with her Master’s degree in May 2025. As part of her program, Angela is required to complete direct clinical hours under the supervision of a certified speech pathologist. I would like to ask for your permission to have Angela work alongside me during therapy sessions with your child. If you are comfortable with this, please let me know.
