Speech Assessment Summary: PATIENT HX: Adaluna, who goes by “Luna”, is a 2 year, 0 month old female who was referred for a speech and language evaluation by her pediatrician for concerns with speech delay. Adaluna’s mother and father served as the informants for this evaluation. Mrs. Sanchez reported that Adaluna uses less than 10 words and primarily uses pointing/vocalizations to express her wants/needs. According to clinical observations, Adaluna’s communication is characterized as minimal to no functional words and pointing/grabbing to communicate his wants/needs. Adaluna’s speech and language skills were evaluated using the Preschool Language Scales 5th Edition English version (PLS 5). ORAL MECH EXAM: Adaluna’s oral structure and function appears functional. However, Adaluna is reportedly a mouth breather at night and snores occasionally. Adaluna was also observed to be somewhat of a mouth breather during the evaluation. The clinician advised Adaluna’s parents to seek further consultation with an ENT if snoring and mouth breathing persists. Adaluna appears to have labial tie, however it is unknown if she has a lingual tie. The clinician advised parents to request for Adaluna’s ties to be checked at her next dentist appointment. HEARING: Adaluna reportedly passed her newborn hearing screening and has no history of ear infections. Adaluna has an audiological evaluation in November 2024. Adaluna’s assigned treatment therapist should ensure to follow up with her audiological findings. BEHAVIORAL OBSERVATIONS: The evaluation took place on the clinic room floor where Adaluna required numerous prompts for cooperation and redirection. Adaluna demonstrates decreased social communication skills, which may be attributed to her noted pragmatic weaknesses. The following behavioral observations were also noted: INCONSISTENT attending skills, INTENT too communicate, WNL social interactions, ADEQUATE awareness of others/environment, MILD DELAYS in response rate, need for NUMEROUS PROMPTS to cooperate, and WNL level of activity. LANGUAGE: The following are Adaluna’s standard scores on the PLS 5 - Auditory Comprehension 69 (raw score: 20, percentile: 2, age equivalent: 1 year 4 months), Expressive Communication 64 (raw score: 17, percentile: 1, age equivalent: 1 year 0 months), and Total Language Score 64 (raw score: 37, percentile: 1, age equivalent: 1 year 2 months). According to Adaluna’s standard scores on the PLS 5, where a score from 85 to 115 is considered within functional limits, Adaluna presents with a SEVERE MIXED RECEPTIVE EXPRESSIVE LANGUAGE DISORDER. Speech Assessment Summary: Adaluna is a 3 year old, 4 month old female that has been receiving speech and language therapy at New Beginnings Pediatric Therapy services and was re-evaluated to determine need for continued services. During the evaluation, the patient demonstrated limited willingness to participate in evaluation activities as observed by patient perseverating on obtaining ball. Pt was observed to be crying and screaming which prompted clinician to allow mother into therapy room to assist with redirection. Mother and clinician presented with difficulty redirecting patient. Some portions of the PLS-5 were completed based on caregiver report and previous observation in sessions. During assessment, Adaluna was re-evaluated using the Preschool Language Scales Fifth Edition. On the PLS-5, Adaluna obtained the following standard scores: Auditory comprehension 67 (raw score: 26, percentile rank: 1, age equivalent: 1-11) Expressive Communication: 70 (raw score: 25, percentile rank: 2, age equivalent: 1-10), and Total Language: 67 (raw score: 51, percentile rank: 1, age equivalent: 1-9). Adaluna’s score on standard deviation is at least -2 standard deviations below the mean. Based on scores reported above, Adaluna presents with a SEVERE MIXED RECEPTIVE/EXPRESSIVE LANGUAGE DISORDER. Adaluna has made measurable progress as evidenced by meeting all of her targeted therapy goals and an increased performance on raw score in standardized assessment. A decrease in standard score should be interpreted with caution as performance during the re-evaluation may have been influenced by reduced participation and recent change in treating therapist. Adaluna’s expressive and receptive language skills are still not functional for her age. Speech therapy continues to be warranted to promote safety and independence due to Adaluna’s high safety risk. Parents will remain responsible for implementing the Home Management Intervention Plan (HMIP).
