Occupational Background History: Daniela is a 8.3 year old girl who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born at 20 weeks gestation, just at the 5th month mark as a pre-mature infant. Child did experience complications at birth which required a 3 month NICU stay. Child may possibly have a L side eye deficit, specialist appointment scheduled; Child does not have hearing deficits. Child does not have hx of aggression; Child does have a hx of seasonal allergies.

Occupational Background Summary: Daniela demos difficulty with in-hand manipulation skills due to delayed finger dexterity, finger isolation and palm translation skills all affected by LUE weakness and spasticity due to CP diagnosis. Fine motor skills involved the small muscles of the body that enable such functions as writing, grasping small objects, and fastening clothing. Weaknesses in fine motor skills can affect a child's ability to eat, write legibly, use a computer, turn pages in a book, and perform personal care tasks such as dressing and grooming.
Occupational Diagnosis/Impressions: Daniela was assessed via: Standardized Assessment The REAL (The Roll Evaluation of Activities of Life), ROM test, MMT test, Stereognosis test, Clinical Observations and Parental Report Per The Real Assessment: Daniela's ADL standard score was less than 81.9 (lowest available score) and percentile rank was 3%, which indicates a delay in self-care abilities compared to her peers; Daniela's IADL standard score was less than 100.7 (lowest available score) and percentile rank was 50%, which indicates a delay in self-care abilities compared to her peers. Per parent report, pt displays difficulty with initiating, sequencing and termination of tasks due to high distractibility and decreased focus, as well as her L sided weakness and spastic musculature. Scores obtained from age equivalency of 8:0 to 8:11 years compared to her chronological age of 8:3 years old. Per MMT test: RUE is at MMT of 3+ throughout LUE is at MMT of 2 throughout with 2- at grip and pinch strength; did not test BLEs, however it is noted there is LLE weakness and decreased AROM/PROM and atypical gait pattern "crouch gait" (L hip drop) Per AROM: RUE: appeared WNLs throughout LUE: shoulder limitation at flex/ext, IR/ER and abd/add by 10-15 degrees each; elbow limitation in flex/ext and IR/ER by ~5-10 degrees each; wrist limitation ~2-5 degrees in supination and pronation and flex/ext; 100% limitation in serial opposition of L hand; PROM throughout the L UE triggers spasticity Daniela demonstrates deficits in fine motor precision, fine motor integration, visual motor integration, manual dexterity, manual coordination, UB and LB strength, increased muscular tone, atypical gait pattern and decreased bilateral coordination, which have impaired her ability to complete activities of daily living and instrumental activities of daily living that compromise her routines and roles at home and in the community.
