Uzziah mother served as the informant for this evaluation. Mrs. Usiel reported Uzziah memorizes things well and repeats words/phrases constantly. However, Mrs. Usiel reported that Uzziah uses the same repetitive communication and does not use new words in sentences. Uzziah was pending an Autism/ADHD evaluation in Washington, but never received an appointment before moving. Mrs. Usiel is working on obtaining insurance for Uzziah and a referral for an Autism evaluation. According to clinical observations, Uzziah’s communication is characterized as phrases and grabbing/crying to express wants/needs. Uzziah’s speech and language skills were evaluated using the Preschool Language Scales 5th Edition English version (PLS 5). Due to Uzziah’s language background and his reported language input/output, the language of intervention will be English.

Assessment of Goals/Response to Treatment: The patient presented as alert, oriented, and cooperative for the therapeutic session. Intervention targeted the identification and production of regular plurals at the word, phrase, and sentence levels. The patient demonstrated significant progress, achieving 70% accuracy with minimal cues, thereby surpassing the initial objective of 20% accuracy. Further work addressed receptive language skills, specifically the ability to answer simple WH questions. The patient responded with 70% accuracy requiring minimal to no cueing, showing substantial gains toward the goal of 80% accuracy. Expressive language intervention also focused on the use of grammatically correct sentences of four or more words to communicate information, requests, and descriptions. The patient successfully utilized targeted sentence structures with 70% accuracy given minimal clinical prompting, which greatly exceeds the established objective of 20% accuracy. Overall, the patient displayed notable improvement across all targeted domains.

As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

4.) When were these developmental milestones met: Crawl, sat-up, stood up, walked, fed self, dressed self, potty trained?
5.) Sleeping or breathing issues? Does patient get 8-10 hrs of sleep? Wake up several times a night? Snoring? Sleeps with mouth open or closed? Enlarged tonsils?
6.) Education history (i.e., attends daycare, preschool, etc.)?
7.) Vision or hearing issues?
8.) In a few words write your parental concerns with patient’s speech and/or language, and how you would like it to improve within a year.

2) I was transported from one birthing facility to another hospital due to baby''s heart rate being low, had a c-section, Uzziah was put into a different room with other babies for 2 days till the 3rd discharge day.
3) We recently tried to check if he was autistic or had adhd but the wait list for him to be seen after registering would take 10 months from now.
4) Crawling, sat-up, stood up, walked before he turned 1 year old. Feeding himself, he can eat snacks and food hes used to with his hands, he can use a spoon to eat cereal but hes picky on food hes not used to eating so its a struggle to get him to use utensils on other foods besides cereal. He drinks liquids through straw. He won''t drink anything if there''s no cover or lids.
5) No sleeping or breathing issue. He gets 8-10 hours of sleep. Doesn''t wake up during the night. He sometimes snores. He sleeps sometimes with his mouth open and sometimes closed. No enlarged tonsils.
6) None
7) No hearing or vision issues
8) Okay, so I want to say that my son is pretty good at memorizing things, which is how he knows some words. He repeats words, phrases that he hears constantly. He doesn''t use words in sentences?! His communication is repetitive.

Everyone in our household tested negative besides him. His doctor said if hes not showing signs of symptoms and no flu then hes able to leave the house by the 21st as long as hes wearing a face mask. We will continue to give him medicines and monitor his flu till then.
