Feeding Background History: Kyro is a 11 month 19 day old male who was referred by his pediatrician concerns regarding feeding aversion. Kyro’s mother and father served as the informants for this evaluation. Ms. Rosas reported Kyro is not up to date with eating solids, rejects purees, and will only lick or suck on solid foods but will not chew or swallow solids. Kyro primarily gets all nutrition via bottle. Ms. Rosas also reported that Kyro struggled to breastfeed and only takes bottles. A bedside feeding evaluation and skilled meal feeding observation were utilized to assess Kyro’s feeding/swallowing skills.
Feeding Medical History: Kyro was born at 39 weeks via c section and complications included: marginal cord insertion, anemia, and cholestasis.

Feeding Diagnosis/Impressions: PATIENT HX: Kyro is a 11 month 19 day old male who was referred by his pediatrician concerns regarding feeding aversion. Kyro’s mother and father served as the informants for this evaluation. Ms. Rosas reported Kyro is not up to date with eating solids, rejects purees, and will only lick or suck on solid foods but will not chew or swallow solids. Kyro primarily gets all nutrition via bottle. Ms. Rosas also reported that Kyro struggled to breastfeed and only takes bottles. A bedside feeding evaluation and skilled meal feeding observation were utilized to assess Kyro’s feeding/swallowing skills. ORAL MECH EXAM: A formal oral peripheral exam could not be completed due to Kyro’s young age. However, the following observations were made - possible lingual/labial tethered oral tissues (TOTs), shallow palate, and some dentition. At this time Kyro’s oral structure/function is considered inadequate due to his possible TOTs . Clinician educated parent to seek further assessment/treatment with a pediatric dentist. FEEDING/SWALLOWING: A bedside swallow evaluation and skilled meal feeding observation revealed that Kyro presents with a CHRONIC PEDIATRIC FEEDING DISORDER as characterized by impaired oral intake that is not age appropriate and is associated with medical diagnosis (TOTs), nutritional status (no oral intake of solid foods), feeding skill (sensory difficulties with foods, poor oral motor skills for eating, physiological difficulties with foods), and/or psychosocial dysfunction (problematic mealtime behaviors). At this time, the pharyngeal and esophageal phase of Kyro’s swallow could not be formally assessed as he refused any trials of solids/liquids. The esophageal/pharyngeal phase of Kyro’s swallow function should be monitored to determine if further assessment/treatment is needed. However, Ms. Rosas does not report unexplained fevers or history of aspiration pneumonia. Kyro is reported to have no known food allergies. Kyro is reported to prefer crunchy/smooth/hard foods, tart/sweet flavored foods, and have an overall poor appetite. Ms. Rosas completed the Pediatric Eating Assessment Tool (PediEAT) which is a questionnaire used to subjectively assess Kyro’s feeding behavior. Kyro received an overall rating of "High Concern", with physiologic symptoms and problematic mealtime behavior being of highest concern. Kyro’s ratings on the PediEAT are consistent with parent reports and with his performance in the evaluation and diagnosis. Purée via spoon and puffs were offered to Kyro, which were both rejected. Kyro mouthed a puff and immediately took it away from his mouth after it touched his lips. Kyro refused to open his mouth for any trials of foods which is also reportedly consistent in the home. After sitting in the high chair for a few minutes, Kyro began to scream and cry. Kyro was removed from the high chair and more foods were trialed in his fathers lap, however he refused all trials as well. Kyro currently drinks 8oz of formula via bottle every 3-4 hours. Kyro is in the 50th percentile for his weight and height. Kyro is reportedly given opportunities to eat every time his parents eat solids. Ms. Rosas completed a food inventory for Kyro, however all food items listen are foods that Kyro will mouth or lick but will not actually chew and swallow. RECOMMENDATIONS: Feeding therapy is highly recommended at this time, and should target increasing Kyro’s tolerance to solid food items via steps to eating hierarchy and desensitization methods, as well as improved family mealtime behavior/routine. Without feeding therapy, safety risks include but are not limited to: lack of appropriate nutrition due to restricted diet characterized by no solid foods and food jagging on limited foods in Kyro’s food inventory. Ms. Rosas and Mr. Butler will be responsible for Kyro’s Home Management Intervention Program (HMIP).

Kyro is due for a feeding re evaluation to determine if continued feeding therapy is warranted. A bedside feeding evaluation and skilled meal feeding observation were utilized to re assess Kyro’s feeding/swallowing skills. ORAL MECH EXAM: Kyro had his lip and tongue ties released in July 2024. At this time, Kyro’s oral structure/function is considered adequate for feeding/swallowing development. FEEDING/SWALLOWING: A bedside swallow evaluation and skilled meal feeding observation revealed that Kyro continues to present with a CHRONIC PEDIATRIC FEEDING DISORDER as characterized by impaired oral intake that is not age appropriate and is associated with medical diagnosis (history of TOTs), nutritional status (poor variety of solids), feeding skill (sensory difficulties with foods, decreased oral motor skills for eating, physiological difficulties with foods), and/or psychosocial dysfunction (problematic mealtime behaviors, selective eating). At this time, the pharyngeal and esophageal phase of Kyro’s swallow are not of concern. Additionally, Ms. Rosas does not report unexplained fevers or history of aspiration pneumonia. Kyro has no known food allergies. PROGRESS TO DATE: Since Kyro’s last re-evaluation, Ms. Rosas reports that he has expanded his food repertoire. However, she would like to continue working on introducing a wider variety of foods, particularly savory and healthy options. It is important to note that Kyro has experienced significant environmental changes, as he now splits time between his mother’s and father’s homes. These changes may be contributing to the frequent emotional outbursts observed during sessions. Differences in feeding schedules and food options between his two households may also be impacting his performance. Due to these environmental factors and decreased consistency in therapy, Kyro did not meet the majority of his target goals during this authorization period. RECOMMENDATIONS: Feeding therapy continues to be highly recommended at this time, and should target increasing Kyro’s tolerance to age appropriate solids as well as adding nutritional variety to his food inventory via steps to eating hierarchy and desensitization methods. Kyro’s new goals will also focus on food chaining techniques to support his performance and progress. Kyro’s oral motor skills should also continue to be targeted to support chewing/swallowing age appropriate consistencies. Without feeding therapy, safety risks include but are not limited to: lack of appropriate nutrition due to restricted diet characterized by no solid foods and food jagging on limited foods in Kyro’s food inventory. At this age, Kyro should be completely transitioned to table foods for all meals and should have a variety of nutritious foods in his food inventory. Kyro should also be able to chew and swallow hard mechanical textures. Ms. Rosas and Mr. Butler will be responsible for Kyro’s Home Management Intervention Program (HMIP).

His mother and I agreed to work on savory or plain foods in the categories of proteins, starches, and vegetables. We have been working on foods that are soft cubed consistency or soft mechanical consistency to support him eating the food if he does put it in his mouth. We have been working on chicken/meat, green beans, and rice/noodles. Even if he does not eat these foods at home it is important to offer routine opportunities daily for him to play and explore these foods without the demand or expectation that he has to consume the food. He did try and throw himself back several times and he did bump his head on the ground. I tried to offer a safe environment so he would not hurt himself. Next week will be our last week on our authorization until the new authorization comes in from insurance. Will Tuesday at 1045AM be ok? My schedule is changing a lot right now I am sorry I have some personal matters going on.

He did put the chicken fries to his mouth twice and then refused. I was pacing him because he was trying to put too many pretzels in his mouth and he got upset so we had to work through his emotions for a bit. I recommend we start some new food chained items that we will consistently target for a month so maybe sticking with the chicken fries or with the corn dog variation or maybe moving on to a different protein all together? Let’s try a new starch, fruit, and vegetable to target for the month of October. What do you think? :)

I had back to back sessions this afternoon so I did not have a chance to message you back right away, but I will be sure to send a message weekly after his visits. Kyro did good today. He got a little upset because he was trying to overstuff his mouth and I had to pace him but he adjusted to the pacing. He took 2 small bite of the meat from the corn dog and ate the rest of the breading. He ate his yogurt but spit out the small pieces of fruit - I tried to show him how he can chew them and he did eat one small piece. We played with the fruit from his fruit cup and he licked the juice from the cup from his fingers. He seemed curious about the fruits. He marked his cheese stick with his teeth once. I told you mom how we are working on food chained corn dogs, a fruit, a vegetable, and another starch or protein. I told her how we should keep the fruit, vegetable, and starch or protein the same for a few sessions and continue targeting different food chained corn dogs. :) Also did you by chance see my message about Kaizen’s feeding therapy session for next week?

I am thinking this could be a helpful technique to use with him at home. It seems when the corn dog is whole he tends to just want the bread but when its in smaller pieces and mixed he is more receptive. He also has been doing a lot of imitation with me - we have been doing a my turn/your turn game and he imitated holding corn in his mouth and some of his fruit cup! He also bit and spit out his cheese today. Overall a productive session. :)

He lasted in the high chair for about 5 minutes. I told his dad to start working on feeding him in the high chair even if we start with smaller amounts of time. I also recommended eliminating tv or distractions so that he can focus on his eating. I also recommend to work on meltables or soft cube consistencies and pacing him by purposefully offering small pieces and placing them on the lateral sides of his mouth and modeling chewing pattern. I also recommend allowing food to sit in front of him even if it is to play with the food or look at it. I did assign him a chewy tube to also start using and working on to promote those oral motor skills. I will keep you posted as we continue his care. Today was more of an education session and getting him more comfortable with me. :)

He does eat the hot dog meat as long as it has a little bit of a preferred food on it. He also touched corn to his lips and he explored the fruit cup with his lips and tongue independently today. Overall very good session. :)

I noticed when i cut the strawberries into smaller cubed pieces he was able to chew and swallow them rather than the big pieces that would fall out of his mouth or he would spit out. We only played with corn with out hands and he refused his corn dog and fries. His grandmother and I agree to maybe air fry the corn dog and fries so that they don’t get soft and he is more likely to want to eat them. :)

Kyro did great today in session; he took several bites of the meat from the corn dog but he did need help to keep the corn dog together so he would not eat just the bread. He ate his banana, yogurt, and goldfish. We only played with corn and he did bite and spit out pieces of grapes. Next week will be Kyro’s re evaluation so I will send a questionnaire for you both to complete. The following week his new therapist will take over and I will be sure to list that you all would prefer an earlier time for therapy - I know that her schedule is a bit booked right now so I am unsure what time she will be able to offer but I did note your preference. Ms Andrea will be Kyro and Kaizen’s new feeding therapist and they will be in great hands. :)

He licked several corns but then pushed them away with his hands. Kaizen interacted by licking 2 cookies and touched the scrambled eggs and the corn. The last 5 minutes of the session were for regulation since he was a bit overwhelmed. At home, you can start introducing scrambled eggs with a little bit of cheese just to change the consistency and animal crackers. Just remember to model first that way they are receiving that visual input too. Overall, they both did good! I will see them again next week. Thank you :)

We are still working on regulation and tolerating the high chair for 3-4 minute intervals. Today he licked egg, cookies, string cheese, and sausages. At home, you can try feeding Kaizen soft table foods (soft cubes) such as avocado, banana, cooked apple, etc. Try modeling first; you can even have Kyro model feeding behaviors for Kaizen! 03-24-2026 07:30 PM	Andrea Rodriguez	Hello! We didn’t use any toys today, they were both engaged throughout the session. We used the high-chair for the entirety of the session. Kaizen took several bites of the sausage, avocado, goldfish and apples. We are still working on taking bites and pacing. Kyro took at least one bite of each item. We will be targeting the same food items for next week. Thank you!
