Speech Background History: Kesandu is a 2 year, 10 month old female referred for a speech and language evaluation by her pediatrician. Her mother, Ms. Ezeogu, served as the primary informant. Kesandu previously received speech therapy and was discharged after meeting all goals and demonstrating age appropriate communication skills. However, since discontinuation, her mother has noticed a regression in attention span and increased frustration, which may be related to her parents’ recent divorce. Currently, Kesandu uses approximately 200 words and speaks in 3 to 4 word phrases. Clinical observation indicates her communication is functional but sometimes accompanied by tantrums and emotional outbursts. The Preschool Language Scales, Fifth Edition (PLS-5), English version was used to assess her skills, and English was determined to be the appropriate language for intervention based on her language background and usage.

Speech Background Summary: Kesandu was accompanied by her mother to the evaluation, which was conducted at a clinic room table. She required frequent prompts and redirection to participate in certain tasks; however, she demonstrated appropriate social communication skills when engaged. Throughout the session, Kesandu exhibited several emotional outbursts, all of which were manageable with redirection. Her attention and behavioral challenges observed during the evaluation appear consistent with a need for occupational therapy support.

Speech Assessment Summary: PATIENT HX: Kesandu is a 1 year, 7 month old female who was referred for a speech and language evaluation by her pediatrician for concerns with speech and language. Kesandu’s mother served as the informant for this evaluation. Mrs. Ezeogu reported Kesandu has about 10 words but does not consistently use her words. According to clinical observations, Kesandu’s communication is characterized as some functional words, pointing/reaching, and whining/crying to communicate her wants/needs. Kesandu’s speech and language skills were evaluated using the Preschool Language Scales 5th Edition English version (PLS 5). ORAL MECH EXAM: A formal oral mechanism assessment could not be completed due to Kesandu’s unfamiliarity with the clinician. However, the following observations were made: open mouth posture at rest, mouth breathing, labial tethered oral tissue (TOT) due to teeth gapping, and possible lingual (TOT) due to low and protruding tongue carriage. It is recommended that a formal oral mechanism assessment be completed when Kesandu allows for it. Due to Kesandu’s mouth posture and possible TOTs, the clinician recommended that she seek further assessment/treatment from a pediatric dentist. Kesandu may also benefit from a referral to an ENT due to her mouth breathing and reported snoring at night. As of right now, Kesandu’s oral mechanism is considered inadequate at this time for speech and language development. HEARING: Per parent report, Kesandu passed her newborn hearing screening and a history of 1 ear infection. Clinician recommended Kesandu receive a formal audiological evaluation to ensure that her hearing is adequate for speech and language development. BEHAVIORAL OBSERVATIONS: Kesandu was accompanied by her mother to the evaluation. The evaluation took place on the clinic room floor where Kesandu required numerous prompts to cooperate for certain evaluation tasks. Kesandu demonstrates good social?­ communication skills, however Kesandu’s diminished vocabulary may be hindering the quality of her pragmatic skills. The following behavioral observations were also observed: ADEQUATE attending skills, INTENT to communicate, ADEQUATE awareness of others/environment, SHY social interactions, APPROPRIATE response rate, and needs NUMEROUS PROMPTS to cooperate. LANGUAGE: The following are Kesandu’s standard scores on the PLS 5 - Auditory Comprehension 77 (raw score: 18, percentile: 6, age equivalent: 1 year 2 months), Expressive Communication 81 (raw score: 20, percentile: 10, age equivalent: 1 year 3 months), and Total Language Score 78 (raw score: 38, percentile: 7, age equivalent: 1 year 2 months). According to standard scores on the PLS 5, Kesandu presents with a MILD to MODERATE MIXED RECEPTIVE EXPRESSIVE LANGUAGE DISORDER. Kesandu was greatly benefit from speech therapy services that address the following functionally appropriate concepts: follow routine directions, identify familiar objects, turn taking, imitating multi syllabic words, imitating 1-2 word phrases, and using functional communication to express wants/needs. RECOMMENDATIONS: Speech therapy is highly recommended at this time as Kesandu’s receptive and expressive language skills are NOT WITHIN FUNCTIONAL OR DEVELOPMENTAL LIMITS for her age/safety. Without speech therapy, Kesandu’s safety risks are high since she will likely not be understood if she needs to communicate pain or the need for assistance. Additionally, Kesandu may not understand instruction from safety personnel (i.e. name). Kesandu’s mother will be responsible for her Home Education Program (HEP). Due to Kesandu’s language background and her reported language input/output, the language of intervention will be English. *** It is imperative that Kesandu’s assigned treatment therapist follow up with all recommendations: TOTs assessment with a pediatric dentist, ENT consultation, and Audiological consult.
Speech Rehabilitation Potential: Good. with consistent Speech Therapy and parental support. Prognosis in the area of improvement is good. Kesandu will be discharged when she achieves a standard score above ­- 1.0 standard deviations from the mean for all impairments, indicating that her speech and language skills will be in the average range and she will have met long term therapy goals.

Child currently demonstrates attention span approximately 60 seconds before attempting to flee. Child struggled to initiate and/or complete simple one step directions with max assistance via visual demonstrations, and verbal prompting for motivation, task initiation and redirection to task. Child exhibits limited attention evident by fleeing/escaping non­ preferred activities, and need for constant redirection. Child also demonstrates retention of primitive reflexes which have decreased her ability to engage in activities due to her nervous system continuously in a high alert state decreasing her body and safety awareness. Kesandu demonstrates deficits in fine motor precision, fine motor integration, visual motor integration, manual dexterity, manual coordination, sensory processing and self­regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Child would greatly benefit from skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Occupational Assessment Summary: Kesandu was assessed via: PDMS-2 Standardized Assessment and SP-2 assessment, Clinical Observations, Parental Report Per SP-2 Toddler Version: Kesandu falls under the AVOIDING and SENSITIVITY quadrants demonstrating that the child moves away from sensory input at a higher rate than others and notices sensory input at a higher rate than others, setting her at +2 standard deviations from the mean. These sensory processing difficulties hinder child's ability to participate in daily activities; per clinical observations child has not integrated the STNR and Babinski reflexes, which are enhancing her sensory processing difficulties as her body is not able to process sensory stimuli due to increase hyper-sensitivity. Kesandu demonstrates deficits in fine motor precision, fine motor integration, visual motor integration, manual dexterity, manual coordination, sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Bilateral CoordinationIn 3 months, Kesandu will execute a ­3-4 step functional activity, given only MOD verbal directions (to initiate, sequence and terminate task) to demonstrate improved ability to execute movement and functional independence during ADLs, 3/4 trials and per parental report. Subjective: Patient was friendly and cooperative,
Assessment of Goals/Response to Treatment: Kesandu increased body awareness and safety during functional mobility when transferring between 4 tasks with MOD assist. Kesandu only demonstrated 1 tantrum during a FM task secondary to frustration. However, she was able to demonstrate increased self-regulation skills when taking space and was able to be redirected back to the activity within 2 minutes. Kesandu initiated, sequenced, and completed all tasks during the session with MOD cues for redirection during 1/4 tasks to demo increased self-regulation skills. Kesandu executed a ­3 step functional activity, given MOD verbal cues for assistance when sequencing to increase her ability to execute movement and functional independence during ADLs. Kesandu also participated in FM/VMI tasks while in prone and quadruped with MIN A for compensations to increase integration of primitive reflexes to improve motor planning/bilateral coordination skills.

As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

We wanted to take a moment to keep you informed about an issue that may affect your experience with us. Tricare is currently experiencing technical difficulties with their technology systems and initial and re-authorizations. As a result, there may be temporary delays in authorizations for your child’s therapy.
Please know that we are closely monitoring the situation and working with Tricare to resolve the issue as quickly as possible. They have stated a need of 4-8 weeks for their system issues to be resolved as their subcontractor insurance switched from Healthnet Federal Services to Triwest Alliance on 01/01/2025.

We sincerely apologize for any inconvenience this may cause and appreciate your understanding during this time.

Another patient came into the room and played with the toys she had played with and she just watched she didn't approach her or anything. She did seem a little off when she first came in she clung to Dad which typically she comes with me no problem and she was quiet at first but after a few minutes she was talking and her happy self again. So I am not sure maybe its just environmental and something is happening at daycare that triggers her behavior?

She started the session a bit quiet, but after a few minutes, she became very talkative. We took care of the baby and played with Play-Doh. The transitions between toys went smoothly. We focused on one-step instructions, naming, and requesting. Overall, it was a good day for her. The only time she cried was when it was time to leave.

Happy moments. We have went cold turkey on her pacifier (she was using it for bed) and last night was the third night and it was an emotional peak for her. I've just been trying to encourage her to self soothe with her stuffed animal and blanket

She had a good day; it was only during the transition from the lobby to the room that she was crying, but in the room, she was super talkative and cooperative. We played doll house, and we used the little people to model some of the skills and strategies she is using when she is sad or mad. I pretended one of the dolls fell, and we would hug him; this was to model and remind her of the strategy. I asked your husband if maybe I could get her from the car next week and see if that works better because it seems that the transition from the lobby is what makes her cry.
