Speech Assessment Summary: PATIENT HX: Mateo is a 1 year 6 month old male that was referred for a speech and language evaluation by his pediatrician due to speech delay and delayed milestones. Mateo was evaluated using the Preschool Language Scales Fifth Edition (PLS 5) English. Mateo was accompanied by his mother to the evaluation and she served as the primary informant for the evaluation. Ms. Serrano voiced that Mateo used to say momma and papa, but he no longer does. Ms. Serrano stated that Mateo only babbles and is often very quiet. Mateo just experienced a life changing event, which was the birth of his sibling. Mateo’s mother stated his not enrolled in daycare and just recently began walking, which is apparent in his gate. Mateo’s current communication is characterized as use of no functional words and crying/whining/grabbing to express his wants/needs. ORAL MECH EXAM: A formal oral motor exam could not be completed due to Mateo’s young age and difficulty with instructions. Mateo’s oral motor cavity should continue to be monitored to ensure the structure and function is adequate for speech and language development. The clinician advised Ms. Serrano to seek TOTs consultation with a pediatric dentist. HEARING: Mateo passed his newborn hearing screening. Mateo has had two ear infections - one in October 2023 and another in January 2024. The clinician recommended Ms Serrano seek further assessment with a audiologist/ENT to ensure Mateo’s hearing is adequate for speech and language development. BEHAVIORAL OBSERVATIONS: ADEQUATE attending skills, INTENT to communicate, SHY social interactions, MILD DELAYS in response rate, ADEQUATE awareness of others/environment, and need for SOME PROMPTS to cooperate. Mateo was accompanied by his mother to the evaluation. The evaluation took place on the therapy room floor. Mateo required some prompts to redirect to the evaluation. However, Mateo’s weaknesses in pragmatics may be impacting his social­ communication skills. Mateo also demonstrated repetitive movements of his hands/wrists throughout the session especially when he appeared excited or happy. LANGUAGE: The following are Mateo’s standard scores on the PLS 5 - Auditory Comprehension 77 (percentile: 6, age equivalent: 1 year 2 months), Expressive Communication 60 (percentile: 1, age equivalent: 0 years 8 months), and Total Language Score 67 (percentile: 1, age equivalent: 0 years 11 months). According to Mateo standard scores on the PLS 5, Mateo presents with an overall MODERATE­-to­-SEVERE MIXED RECEPTIVE-EXPRESSIVE LANGUAGE DISORDER where his expressive language is more severe (SEVERE) than his receptive language (MODERATE). Speech therapy is required at this time and should target the following age appropriate concepts: functional/relational play skills, following directions, identifying common objects, imitating actions/gestures, imitating speech sounds, imitating words, and using functional communication to express his wants/needs. RECOMMENDATIONS: Speech therapy is highly recommended at this time to target Mateo’s language difficulties. Early intervention is imperative for Mateo as the critical period of language development is from 1-3 years old. Without speech therapy, Mateo’s safety risk is high since he will likely not be understood if he needs to communicate pain or the need for assistance. Mateo may also demonstrate difficulty following instruction from safety personnel. Mateo’s mother will be responsible for his Home Education Program (HEP). Per parent report and clinician observation, the language of intervention will be English. ***All recommendations should be followed up by assigned treatment therapist to ensure the best possible outcome for Mateo’s speech and language development/progress: ENT/audiology consult and TOTs consult.

Mateo was introduced to solids at approximately 7 months of age. According to mother, Mateo consumed a wide variety of foods but began displaying picky eating behaviors around 24 months of age. A bedside feeding evaluation and skilled meal observation were completed to further assess his feeding and swallowing skills. Mateo was born at 36 weeks. Mrs. Bussell reported frequent bleeding during the first 4 months of pregnancy. Mateo was diagnosed with Autism Spectrum Disorder in February of 2026. A formal oral mechanism examination revealed the absence of tethered oral tissues. Structurally, Mateo appears to have all anatomical components necessary for safe and efficient feeding and swallowing. However, the function of his oral mechanism appeared reduced, with decreased facial muscle tone, limited jaw stability, and inconsistent tongue movement and oral control observed during feeding. These oral mechanism findings are consistent with his reduced oral motor feeding skills and may contribute to his difficulty coordinating chewing and swallowing higher bolus consistencies. A bedside swallow evaluation and skilled meal observation revealed that Mateo presents with a CHRONIC PEDIATRIC FEEDING DISORDER (PFD) and ORAL DYSPHAGIA. His PFD is characterized by impaired oral intake that is not age?appropriate and appears to be influenced by multiple domains: medical (history of feeding difficulties), nutritional (limited food inventory), feeding skill (sensory sensitivities to food textures), and psychosocial (selective/restrictive eating). Mateo’s oral dysphagia is characterized by decreased oral motor skills required for efficient mastication. FOOD REPERTOIRE: According to mother, Mateo's appetite is variable day to day. Currently, his food inventory includes fewer than 15 accepted foods and 2 liquids. Accepted proteins include chicken strips (Tyson), chicken nuggets (Tyson), sausages, and ground beef or meatballs only when served with spaghetti marinara. Fruits include bananas. Vegetables include potatoes. Starches include saltines, Maria's, cheese puffs, tostadas, vegetable chips, white rice, Mexican rice, donuts, French fries, pancakes, breakfast bars, dry cereals, and spaghetti marinara. Feeding Assessment Summary: PATIENT HX: Mateo is a 3-year, 5-month-old male referred by his pediatrician for a feeding evaluation due to concerns with picky eating and limited nutritional variety. Mateo was introduced to solids at approximately 7 months of age. According to mother, Mateo consumed a wide variety of foods but began displaying picky eating behaviors around 24 months of age. A bedside feeding evaluation and skilled meal observation were completed to further assess his feeding and swallowing skills. MEDICAL HX: Mateo was born at 36 weeks. Mrs. Bussell reported frequent bleeding during the first 4 months of pregnancy. Mateo was diagnosed with Autism Spectrum Disorder in February of 2026. ORAL MECH EXAM: A formal oral mechanism examination revealed the absence of tethered oral tissues. Structurally, Mateo appears to have all anatomical components necessary for safe and efficient feeding and swallowing. However, the function of his oral mechanism appeared reduced, with decreased facial muscle tone, limited jaw stability, and inconsistent tongue movement and oral control observed during feeding. These oral mechanism findings are consistent with his reduced oral motor feeding skills and may contribute to his difficulty coordinating chewing and swallowing higher bolus consistencies. FEEDING/SWALLOWING: A bedside swallow evaluation and skilled meal observation revealed that Mateo presents with a CHRONIC PEDIATRIC FEEDING DISORDER (PFD) and ORAL DYSPHAGIA. His PFD is characterized by impaired oral intake that is not age appropriate and appears to be influenced by multiple domains: medical (history of feeding difficulties), nutritional (limited food inventory), feeding skill (sensory sensitivities to food textures), and psychosocial (selective/restrictive eating). Mateo’s oral dysphagia is characterized by decreased oral motor skills required for efficient mastication. FOOD REPERTOIRE: According to mother, Mateo's appetite is variable day to day. Currently, his food inventory includes fewer than 15 accepted foods and 2 liquids. Accepted proteins include chicken strips (Tyson), chicken nuggets (Tyson), sausages, and ground beef or meatballs only when served with spaghetti marinara. Fruits include bananas. Vegetables include potatoes. Starches include saltines, Maria's, cheese puffs, tostadas, vegetable chips, white rice, Mexican rice, donuts, French fries, pancakes, breakfast bars, dry cereals, and spaghetti marinara. He also required MOD A to engage bilateral hands during sensory bin activities (kinetic sand) to make sand castles to increase bilateral coordination skills. He also required MOD verbal cues to complete a 3 step task to make sand castles with kinetic sand to increase his ability to sequence multistep tasks. Mateo colored at table top in order to improve fine motor precision with coloring strokes with MAX assist via modeling and VCs to initiate. He engaged in coloring a circle during 1/4 trials. Mateo tolerate contact dry and wet textures on bilateral hands. Mateo babbles. He makes sounds but does not say actual words. At around 14 months he would attempt to say "mama" or "papa" but does not say them anymore. He communicates by grabbing our hand and leading us to what he wants. Or will hand us things he wants.

I am working with him on more bilateral coordination and fine motor/visual motor activities like the shape sorter and simple puzzles. He is trying to place the simple puzzle piece on his own I am just helping him to "turn" the piece so it fits and if he places it on the wrong spot I point to the correct one or cover the other ones so he can find the right slot. For bilateral coordination I introduced linking cars so he holds 2 of them and pushes them together. This required more hand over hand assistance because he was enjoying dropping them on the tile. But I want to keep introducing him to new ways of play now that he has mastered in/out!

5. No sleeping issues. Mateo gets around 9-11 hours of sleep. Does not wake up throughout the night. Sleeps with mouth closed. No snoring (only if really tired)

7. Mateo does not speak yet. But Spanish is another language spoken around him.
8. Mateo has lazy eye in left eye. Has not yet seen an eye doctor. Heating screening done last year in October. Passed hearing exam.
9. Mateo displays major speech delay. Uses some signs occasionally; more, all done, open, bath. He babbles and sometimes says mama or baba. I am seeing more frustration from him due to not being able to communicate his needs and wants. Does self-harm if frustrated or transitioning from task to task. (Hits his head against table, wall, floor)

We will be working on: tolerate and engage in sensory-based oral motor play activities (e.g., vibration, chewy tube), interact with non-preferred foods using his whole hands and face, add 1­​2 food-chained items of higher level consistencies, not overstuff his oral cavity, pace his solid/liquid intake, and home implementation plan at home. Once the office has processed the report, obtained the needed signatures, and insurance approves the report, I will reach out to you to schedule our first session. If you have any questions, please reach out.
