Fernanda no longer benefits from skilled OT interventions as child has reached functional independence and performance. Occupational Progress to Date: Fernanda demonstrates consistent progress with occupational therapy services evident by meeting 100% of previous goals with at least 70% accuracy, improves standardized assessment scores on the PDMS-2, per clinical observations and parental report. Parent conveyed there are NO concerns with child's development and participation of ADL activities. Parent understood, child will continue to utilize strategies at home to self-regulate and attend to tasks. Child is IND in ADLs and pre-academic tasks; mother mentioned child will start school fall 2026. In 3 months, Fernanda will demonstrate functional crossing of midline during self help tasks (e.g., reaching across to pull up pants, don a shirt, or wipe face) with MOD assist or use of compensatory strategies, to increase bilateral hand use and independence in ADL participation, 3/4 trials. GOAL MET- 4/4 trials In 3 months, Fernanda will attend to and complete a 2-4 step familiar task (e.g., pick up toy and put it in the bin, get a tissue and wipe nose) with no more than MIN VCs, to improve task sequencing and attention during daily routines, 3/4 trials. GOAL MET- 4/4 trials In 3 months, Fernanda will demonstrate social reciprocity by engaging in at least 1-3 verbal or nonverbal back-and-forth exchange (e.g., greeting, sharing, responding to name, or commenting) with a peer or adult during structured play or daily routines, with MIN cueing, 3/4 trials. Assessment of Goals/Response to Treatment: During the session Fernanda completed PDMS-2 subtests to gather data for upcoming re-evaluation with direct supervision from OTD/OTR/L. She sustained attention to each subtest with only MIN cues for redirection at times to demo increased self-regulation/attention. Fernanda demonstrated functional crossing of midline during a cooperative play task with kinetic sand with only MIN assist during 3/4 trials, to demo increased bilateral hand use for independence in ADL participation.Fernanda increased social reciprocity when completing the cooperative play activity and engaged in verbal and nonverbal back and forth exchanges when responding to questions, following 2 step verbal directions, and commenting back and forth during the activity with MIN cues for assistance. 07-02-2025 11:52 AM	Kaitlin Marsland	She did really well. I think the visual timer and schedule are helping a lot. She was able to complete 3 activities, even one she wasn't very interested in so I asked if we could do 2 minutes and I set the timer and she did :) She only got upset once during the session because we started with play-doh and she cleaned it up after she heard the timer go off but I moved it off the table and I think that's what upset her. I used co-regulation calming strategies of using a soft voice to state "calm body" and modeled deep breathing and she imitated me once. I also gave her "squeezes" or compressions to her arms and said "squeeze squeeze squeeze" and she pushed her arm into my hand again and that helped her calm as well. She came back to the table and completed the next activity and I asked if she wanted play-doh again to try and get her to initiate requesting. She completed a 12 piece puzzle, a magnetic dot art activity, and a sequencing hamburger for me today and we finished with some time on the swing/trampoline :)

She needed some input today to sustain attention to tasks but she sequenced some gross motor activities with me. We had a new dentist play set so she was enjoying that at the table and we took turns with items and worked on some fine motor and social skills with that activity. She should be regulated and ready for speech :)

She did need redirection for minimal behavior I had not witnessed in our past sessions. She attempted to scratch and bite. I redirected with a quick "no Fer, safe hands, no scratch/bite" or " Calm body" and she was able to redirect the behavior. She followed directions. She was naming and singing. Great session overall. Transition to Ot was smooth as well. Thank you for bringing her in, we will continue to work on our goals next week. TY! =)
