Speech Background History: Zhacchaeus Centillas is a 2-year 6-month-old male who was referred by his pediatrician for a comprehensive speech and language evaluation due to limited language skills and delayed milestones. Zhacchaeus’s mother served as the informants for the evaluation. The evaluation was performed in English, as that is the primary language at home. Zhacchaeus’s mother reported no concerns during pregnancy or at birth. Mother reported patient was born at 38 weeks via natural birth. Mother reported patient is using some words however it is inconsistent and not reported approximately 30-40 words. Mother reported he is not using words in spontenous speech and has been observed to only “copy” or imitate what other people are saying around him. Mother reported patient is impulsive and will have frequent tantrums and has been observed to self-harm himself. Self harming behavior such as hitting himself against the wall on his head was observed during the evaluation. Mother reported she is very concerned due to limited communication. Zhacchaeus is very self-directed and attempts to be independent in the home setting as evidence by climbing on furniture to get what he wants/needs. Mother reported patient is still on the bottle due to patient requesting bottle. Mother stated he is bottle fed approximately 2-3 times a day. Mother reported she is very concerned due to Zhacchaeus not communicating any words and have tantrums or get upset when he is not understood. Mother stated behavior displayed during evaluation was consistent with what is observed at home.

Speech Background Summary: During evaluation Zhacchaeus was observed to require high levels of encouragement to participate in task. Scores should be inteprted with caustion as patient was observed to require high levels of cues to participate in tasks. Mother reported the same is observed at home. Patient was observed to continuously get out of seat hide under his chair and attempt to go with mother. Mother reported this is very consistent with what is seen at home. Patient was observed to explore room as he walked in and attempted to play with toys however patient was observed to throw items on the floor and had difficulty picking them up. Patient attempted to play with a ball tower however when clinician tried to play along with him patient continued to demonstrate shy behavior or would run with mother. Mother stated this was a very common behavior in unfamiliar settings. Patient was observed to climb furniture during evaluation and mother stated the same was observed at home. When patient wants/needs something patient will attempt to get item on his own or climb furniture or physically grab mother. Mother stated patient uses some words such as “milk” when he wants milk however will only use a couple of words independently. Patient was observed to be very self directed and required numerous prompts to sit in the kinder chair. Mother is very concerned as patient does not use words consistently and will depend on other to anticipate his wants/needs.

Child is easily distracted, and it is difficult for her to be redirected back to the task. Child has difficulty transitioning away from mother and to non-preferred tasks, during evaluation child required to be soothed by mother to re-engage in task. Child demonstrates very limited sustained attention requiring constant supervision and redirection to task.

Subjective: Patient was friendly and cooperative,

Assessment of Goals/Response to Treatment: Zac participated in structured play-based activities targeting attention to task, following directions, task initiation, and coping strategies. Zac maintained attention to therapist-led activities for approximately 3–5 minutes with moderate verbal and visual cues. Demonstrated increased distractibility during less-preferred tasks. Zac followed 1-step directions consistently (80% accuracy) and 2-step directions with moderate prompting (60% accuracy due to task avoidance). He required mod verbal prompts to initiate familiar activities (clean-up, starting a puzzle) but was able to begin tasks within 5–10 seconds after cueing. Zac practiced deep breathing and “pause and count” strategies with modeling. Required prompting to use strategies when mildly frustrated but was able to engage with support. Zac would continue to benefit from skilled ot services to improve short term and long term goals.
