Speech Background History: Alexander is a 5 year 11 months old male that was referred for a speech and language evaluation by his pediatrician due to speech sound errors. Alexander was accompanied by both his parents to the evaluation, and both served as the informants. Mr. Villarreal stated that Alexander was evaluated by a speech-language pathologist last year while they were stationed in New Mexico. However, due to the family leaving the duty station, Alexander did not begin speech therapy services. Per parent report, there are no concerns with Alexander’s receptive and expressive language skills, but Alexander has difficulty producing certain sounds, “s, l, d, and k”. Both parents reported that the family has difficulty understanding when Alexander is using single words, as well as sentences. According to clinical observations, Alexander’s current communication was characterized as 4-5 word utterances to label, comment and/or request, and he exhibited the following phonological processes: fronting and stopping. He became easily embarrassed by his word pronunciation, and hid his mouth with his hands, or looked down as he spoke to the clinician. The Clinical Assessment of Articulation and Phonology 2nd Edition (CAAP-2) was used as the tool of assessment to measure his speech production on phonemes in words and at the sentence level. Alexander received the following Standard Scores on the CAAP-2: Standard Score: <55 (Consonant Inventory Score 19 percentile rank 3, age equivalent <2;6), School-Age Sentence: <55 (School-age Sentence Score: 27 percentile rank <1, age equivalent <5:0). He demonstrated improvement compared to his previous evaluation (Consonant Inventory Score <40); however, he continues to sound unintelligible to unfamiliar listeners. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time. If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications. Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Tricare is currently experiencing technical difficulties with their technology systems and initial and re-authorizations. As a result, there may be temporary delays in authorizations for your child’s therapy. They have stated a need of 4-8 weeks for their system issues to be resolved as their subcontractor insurance switched from Healthnet Federal Services to Triwest Alliance on 01/01/2025. 03-20-2024 02:59 PM	Annette Barron	Good afternoon. As discussed during yesterday’s evaluation, here is a brief summary. Alexander is diagnosed with profound phonological/articulation disorder. His goals will target: understanding the difference between correct and incorrect productions, understanding and using the right articulators (tongue, teeth, lips, etc.) to produce sounds correctly, decreasing his use of incorrect productions, substitutions and reductions of consonant blends, as well as teaching him how to produce the sounds to make the “s, z, v, k, g” sounds. Once the office has processed the report, they obtain the needed signatures, and insurance approves the report, the treating clinician will reach out to you to schedule your first session. My 3:30 patient has agreed to switch his appointment, which means I will be able to see Alex every Tuesday at 3:30 or even 3:15. However, this change will take about three to four weeks to become effective. Another thing to consider is that Alex's authorization is set to finish on 8/11, so we only have this week until his authorization ends. We might have to wait a few days before the new authorization gets approved. One option for this week is to have therapy via telehealth. This is very simple; I will just send you a Zoom link. I think you would need to have an account for Zoom; you will click on the link I will send, and it will take you to the meeting. If this works for you, I can see Alex via telehealth on Friday at 4 pm.
