Speech Background History: Ellena came into the offices with parents and referral from pediatrician and pediatric audiologist, Dr. Amanda Chiao. Parents and family have moved in from Oklahoma were Ellena was born. She has begun to talk and uses good language skills and beginning development but is difficult to understand due to underlying disorder of the acoustinc nerve. History of Diagnosis and language assessment given will be discussed below.
Speech Medical History: Per parent report, pregnancy was good but delivery was complicated. Ellena was born with fluid in her lungs and they had a difficult time intubating her. When this was finally done, she had to go into NICU as she would need to be kept cool for 72 hours. She underwent some brain damage due to HIE and she remained in NICU for 3 months. Ellena also had a g-tube placed for nutrition and understand surgery for a cleft larnyx. After New Born Hearing Screening, and ABR; Ellena was diagnosed with Auditory Neuropathy Spectrum Disorder.

Speech Communication Strengths: Appropriate utterances, Easily separated from Parents, Follows verbal command with cues, Responds to greetings,
Speech Communication Weaknesses: Difficult to understand, Poor use of eye contact,
Speech Background Summary: Ellena played appropriately although became frustrated easily when not understood or when she did not know how to express for herself help with toys and/or tasks for play. Mother prompted and therapist gave examples to ask for help and after practice Ellena was able to request when help was needed. Ellena would answer parent and therapist when answers were needed for questions and/or test items.
Speech Diagnosis/Impressions: At this time, Ellena's Standardized Score for Language Development is 91 and is considered within normal limits for children within Ellena's age group. Although her articulation and oral motor skills were moderately delayed with phonemes that should be age appropriate currently. On the Pre-School Language Scales-Fifth Edition (PLS-5), Ellena had more than 6 errors, which should be noted and considered for speech therapy. Due to Ellena's diagnosis for Auditory Neuropathy Spectrum Disorder. It is strongly recommended that Ellena be provided speech therapy services at this time, to keep up with her ever changing hearing and noise within her daily routine. It should be noted that on any given day, Ellena will miss sounds in her environment and miss a word or group of words spoken to her due to her Neuropathy and not being able to note noise correctly. On this particular day, Ellena was difficult to understand with this evaluation, and her inability to fix her production of phonemes is reason to note the need for Articulation therapy. With the Auditory Neuropathy Spectrum Disorder, Ellena has good hearing days and days that her hearing deteriorates and language and speaking will be very difficult for her and her communication partner. Currently, the Articulation of phonemes within her developmental age are inconsistently produced for her communication partner to understand her. Ellena frustrates easily when others do not understand her, as well as not having her needs and wants met due to others inability to understand her. It is crucial that she be provided therapy services to help in her intelligibility for others to understand her and for Ellena to learn listening strategies and advocating strategies (i.e. I can't hear Mommy, or What is that noise?) in order to be successful in her daily routine. Speech Assessment Summary: Ellena is now 3 years old and is due for a review on her language and if her Auditory Neuropathy is interfering with her communicative intent or language development. Ellena has been attending speech therapy for approximately six months now and has good steady growth with communication, although speech productions continue to be a challenge for Ellena. There have been sessions that due to how she is filtering sounds or how she is hearing, or using her listening acuity; is having difficulty for that day. Ellena was given the Pre-School Language Scales-Fifth Edition (PLS-5), English. Although Ellena's grandparents speak to her in Spanish and she has begun to use Spanish with some of her communication, Ellena is using code-switching, between English and Spanish, and often in the speech therapy sessions. She continues to show dominance in English and use of asking about what she heard. On the Pre-School Language Scales-Fifth Edition (PLS-5) Screener for Articulation, Ellena had more than 9 errors, which should be noted and considered for speech therapy. Due to Ellena's diagnosis for Auditory Neuropathy Spectrum Disorder, it continues to be recommended that Ellena be provided speech therapy services, in order to keep up with her ever changing hearing and noise within her daily routine. Ellena has begun to drop endings on words and runs words together, in that she hears all the speech run together. On any given day, Ellena will miss sounds in her environment as well as words, due to her Neuropathy and not being able to note sounds correctly. On this particular day, Ellena did well with this evaluation, and her ability to fix her production of phonemes. But Ellena will only clarify if her communication partner asks her to say the word again. She had basic difficulties with 'What' and 'Who', whereas in previous sessions she has been able to provide answers. With the Auditory Neuropathy Spectrum Disorder, Ellena has good hearing days and days that her hearing deteriorates and language and speaking can be difficult for her and her communication partner. Ellena frustrates easily when others do not understand her, as well as not having her needs and wants met due to others inability to understand her. Mother continues to note difficulties with words and her inability to follow directives from parents. Overall her Total Language Score remains within the average ranges of language with an 85 (percentile 16; age equivalent 2 years 4 months). Her STANDARD SCORE IS NOT THE SAME AS HER PEERS, her standard score is at ONE STANDARD DEVIATION BELOW THAT OF HER PEERS. It is crucial that she continue speech therapy and auditory --verbal therapy services to help in her intelligibility for others to understand her and for Ellena to learn listening strategies and advocating strategies (i.e. I can't hear Mommy, or What is that noise?) in order to be successful in her daily routine. At this time, it is recommended that Ellena continue with speech therapy three times a month for 30-45 minutes to continue to note and instruct for her hearing acuity and to use auditory-verbal information to help her identify difficulties for the day; and/or to learn to produce phonemes in words accurately. Ellena's functional communication and individual safety are at HIGH risk, in that she may not interpret sound and alarms within her daily routine appropriately, as well as hearing urgency in voice or words said to her when there is danger. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Medical staff was unable to help her breath and had a hard time intubating her. Once intubated she started having seizures because of the lack of oxygen. She was transported to ou childrens hospital where she was cooled for 72 hours. MRI of brain showed severe HIE and damage. Where told by neurologist the part of her brain that was injured was the part that controlled coordination and balance and that she would have CP

A gtube was placed for us to be able to go home, but then she has trauma to the site, and her gtube hole became large and so this complication has us at the hospital for another month. She had a revision surgery . And then we where sent to a rehab facility with an ngtube while we waited for her gtube site to heal . Total hospital stay before taking her home was 3 months

The paper with the Doctors names are all developmental pediatricians and may be able to help in evaluating Miss Elena for the possibility of being a child on the spectrum. It is what we have on file, although Ms. Fonseca may be retired. I'm not sure. The checklist is to better look at some areas of development and to note if we see Miss Elena presenting with any of these characteristics. I am more familiar with "stimming" as the rocking, hand flapping, and spinning. It can be a signal for autism, but not always. Your indication of her stopping and not going back to that behavior and not doing so when she is busy makes me think it may be something they can address through OT. Different ways to show her self-regulating rather than laying down. I would start there with OT and PT regarding the balance. Let me know if you have any questions.
