The primary language of the home is English. Based on the home language all testing was conducted in English. Speech Medical History: Kai was born a twin at 34 weeks and had to stay in the NICU for 3 weeks following his birth. Parent reported that Kai had feeding difficulties as a baby. Parent reports no current feeding difficulties.

Speech Diagnosis/Impressions: Kai is a 4;4 year old male who was seen on the above date for an initial evaluation at New Beginnings Pediatric Therapy per referral of PCP to determine his current level of functioning and if skilled speech intervention is warranted at this time. Per parent report, Kai has no previous history of speech therapy services. Kai was referred by his PCP after the doctor heard him speak during a routine visit. The primary language of the home is English. Based on the home language all testing was conducted in English. Kai completed the Preschool Language Scales -5th edition (PLS-5) in English and the Clinical Assessment of Articulation and Phonology-2nd edition (CAAP-2). The Preschool Language Scales Fifth Edition (PLS–5) was administered on the above date in order to determine current eligibility for speech-language services. Kai was given credit for behaviors that were elicited, produced spontaneously, or confirmed by parental report. Kai earned a Total Language Standard Score of 88, indicating that overall receptive/expressive language skills are WNL. At this time, Kai presents with receptive and expressive language skills that are WFL. At this time direct language intervention is not warranted. However, Kai was observed to have difficulties forming grammatically correct sentences. Mother received recommendation to closely monitor Kai's expressive language. His treating therapist is recommended to re-administration of the PLS-5 after 6 months if continued difficulties are observed during articulation therapy. The CAAP-2 was administered to determine Kai's articulation skills following observations during language testing. The CAAP-2 is a standardized diagnostic tool used to assess articulation. It allows the test administrator to sample spontaneous and imitative phoneme production in single words and conversational speech. The Consonant Inventory Score portion of this assessment evaluates target sounds and consonant clusters in the initial, final, and medial positions of words. For speech and language standardized tests, a standard score of 100 is average, and scores falling between 85 and 115 are considered to be within the normal range of development. Scores falling between 78-84 are considered to be marginal/borderline/mild, scores falling between 71-77 are considered to be low range/moderate, and scores of 70 or below are considered to be very low range/severe. Kai received a Consonant Inventory standard score of <55 indicating a SEVERE ARTICULATION/ PHONOLOGICAL DISORDER. Speech Assessment Summary: Kai is a 6 year, 3 month old male who was initially referred by his PCP after the doctor heard him speak during a routine visit. Kai is due for a re­-evaluation to determine if continued speech therapy services are warranted. Kai's articulation skills were re-­evaluated using the Clinical Assessment of Articulation and Phonology - Second Edition (CAAP-­2) and his language skills were re­-evaluated using the Preschool Language Scales - Fifth Edition (PLS-­5). The tests were administered in English as it is the primary language spoken in the home. Kai received the following scores on the CAAP-­2: Consonant Inventory Standard Score of 104 (Consonant Inventory Raw Score: 2, Percentile Rank: 46, Age Equivalent: 6;2) and School-Age Sentences Standard Score of 58 (School-Age Sentences Raw Score: 21, Percentile Rank: 2, Age Equivalent: <5;0). Kai's scores from the CAAP­-2 indicate that he presents with a PROFOUND ARTICULATION/PHONOLOGICAL PROCESS DISORDER at the SENTENCE AND CONVERSATION LEVEL. Based on information obtained through the re-evaluation, Kai would benefit from speech therapy services continuing to target increased independence with accurately producing the following phonemes: voiceless “th,” /l/ and /v/. This is a general broadcast to all parents, do not text this number to reply.

As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

We wanted to take a moment to keep you informed about an issue that may affect your experience with us. Tricare is currently experiencing technical difficulties with their technology systems and initial and re-authorizations. As a result, there may be temporary delays in authorizations for your child’s therapy.
Please know that we are closely monitoring the situation and working with Tricare to resolve the issue as quickly as possible. They have stated a need of 4-8 weeks for their system issues to be resolved as their subcontractor insurance switched from Healthnet Federal Services to Triwest Alliance on 01/01/2025.

We sincerely apologize for any inconvenience this may cause and appreciate your understanding during this time.

We are excited to announce that Physical Therapy (PT) services are now being offered exclusively at our Zaragoza location for Tricare, Superior, Wellpoint, and self-pay patients!
Our licensed pediatric physical therapist, with close to 30 years of experience, is here to support your child’s movement, strength, balance, and coordination needs. All patients will need a new referral for PT from their pediatrician, and an initial evaluation by our PT.

As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

Based on scores received on standardized testing administered, Kai presents with a profound articulation/phonological process disorder at the sentence level and a mild expressive (use of language) language disorder. Kai continues to present with receptive (understanding of language) language skills within the average range for his chronological age. Kai also received scores within the average range for productions of sounds at the single word level during testing administered yesterday - way to go Kai! We can plan to review and discuss his updated plan of care at his next scheduled session. Please let me know if you have any questions or concerns. I also hope his open house and meet the teacher night goes well at his new school tonight!
