Mateo's father, served as the informant for this evaluation. Prior to the evaluation, Mr. Rivero reported Mateo was diagnosed with an "articulation/phonological disorder in the past". Mateo has a history of speech therapy services at Rio Grande Therapy. Mateo's lip tie was released in March 2021. Due to COVID and Mateo's school not having a speech therapist, Mateo has been out of speech therapy for 1-2 years. Speech Medical History: Mateo was reported to have jaundice at birth, however no further complications were reported about his pregnancy/birth. Mateo had a lingual frenectomy in March 2021.

Speech Background Summary: Overall, Mateo demonstrates good social-communication skills. However, Mateo is difficult to understand and at time unintelligible due to his inaccurate speech sound production/use in conversational speech.

Mateo's father, served as the informant for this evaluation. Per parent report, Mateo is currently in the 2nd grade at Sonoma Elementary in Las Cruses, NM. Prior to the evaluation, Mr. Rivero reported Mateo was diagnosed with an "articulation/phonological disorder in the past". Mateo has a history of speech therapy services at Rio Grande Therapy. Mateo's lip tie was released in March 2021. Due to COVID and Mateo's school not having a speech therapist, Mateo has been out of speech therapy for 1-2 years. A formal oral mechanism examination was completed to determine if the structure and function of Mateo's speech mechanism is adequate for speech and language development. All structures are present and deemed functional except for Mateo's tongue. Mateo presents with a lingual tethered oral tissue (TOT) or "tongue tie", which may be hindering the lingual function needed for certain speech sounds. Mateo was unable to demonstrate adequate place of articulation for the /r/ and /l/ phonemes, which appeared to be due to his decreased lingual range of motion. When asked to demonstrate tongue lateralization, elevation, and protrusion, Mateo demonstrates decreased coordination and decreased range of motion. It is imperative that Mateo's lingual TOT be assessed by a pediatric dentist to determine the next course of action. Mateo's articulation skills were evaluated using the Clinical Assessment of Articulation and Phonology - Second Edition (CAAP-2). Mateo received the following standard scores on the CAAP-2: Consonant Inventory <55 (percentile: <1, age equivalent: <2;6) and School-Age Sentences <55 (percentile: <1, age equivalent: <2;6). According to the standard scores on the CAAP-2 where a standard score from 85 to 115 is considered within functional limits, Mateo presents with a PROFOUND PHONOLOGICAL PROCESS/ARTICULATION DISORDER. Mateo is observed to perform the following phonological processes: gliding of /r, l/ (i.e., ring turns into wing) and consonant cluster reduction with /r, l/ blends (i.e., CLOWN turns into COWN). Mateo was also observed to require speech therapy services for the following phonemes: /th (voiced/voiceless), sh/. Per clinician's clinical observations, Mateo is highly unintelligible at the phrase level and in conversational speech. At this age, Mateo should be 90-100% intelligible to familiar and unfamiliar listeners. Additionally, all of Mateo's speech sounds should be developed and adequate used in conversational speech. Mateo's lingual TOT is inadequate for speech sound development and may be the cause of his articulation disorder and unintelligible speech to unfamiliar listeners. Speech therapy is highly recommended at this time as Mateo's speech sound production and use are not within functional limits for his age/safety. Speech therapy will utilize evidence-based methods and target Mateo's areas of difficulty to promote success and generalization of accurate speech sound production/use. Without speech therapy, safety risks include but are not limited to Mateo presenting with difficulty communicating basic wants, needs, and thoughts to those in his immediate environment especially in the case of an emergency. Speech Assessment Summary: Mateo is a 10 year, 5 month old male that has been receiving speech and language therapy at New Beginnings Pediatric Therapy Services and was re-evaluated to determine need for continued services. Mateo was re-evaluated using the Goldman Fristoe Test of Articulation-3 (GFTA-3). Mateo received the following scores: Sounds in words: raw score: 7, standard score: 72, percentile rank: 3, age equivalent: 5:6-5:7, and Sounds-in-Sentences: raw score: 4, standard score: 83, percentile rank: 13, age equivalent:7:3-7:5. Based on scores, Mateo currently presents with a PHONOLOGICAL/ARTICULATION DISORDER. During assessment, it was observed that Mateo still presents with difficulty with vocalic ‘R’ sounds and ‘th’ sound at sentences level. Since his previous re-evaluation, Mateo has made significant progress such as a reduction in errors in both sounds-in-words and sounds-in-sentences and an increase in standard scores in both Sounds-in-words and Sounds-in-sentences. During this authorization period slower progress was made due to limited access to speech therapy as he is only seen twice per month. Mateo only received speech therapy while in father’s custody which is why Mateo is seen every other weekend. Due to the above mentioned, not all goals were able to be targeted due to Mateo only being able to be seen approximately 5 times this authorization period. Similar goals to be targeted during new authorization due to not sufficient time to target goals. Based on assessment, it is recommended that Mateo continue speech therapy services in order to promote successful generalization of production and use of target speech sounds. This will help promote Mateo’s speech intelligibility which will assist Mateo in expressing his wants and needs. Mateo’s father will continue to be responsible for Mateo’s Home Management Intervention Program (HMI).

Speech Progress to Date: PROGRESS-TO-DATE: ***Due to custody between Mateo’s parents, Mateo is only able to attend speech therapy sessions twice per month when he is with his father. During this previous 6-month authorization, Mateo was only seen 5 times. The number of reduced sessions was attributed by holidays and events which limited availability for therapy. Upon inquiring, clinician proceeded to ask patient where he was. Patient stated that they were out of town in Tucson. Clinician then proceeded to talk to dad. Clinician informed parent that patient has to be in the state of Texas to be able to be seen through zoom. Parent reported to clinician that he is "mentally in Texas". Clinician proceeded to explain to parent legalities of having to be licensed in specific state to be able to see the patient. Parent demonstrated and verbalized understanding. Clinician offered parent a makeup for when patient is back with him and offered to see him twice that week. Zoom meeting was then concluded.
05-15-2025	Jordan Orozco	Patient will not be seen until week of July 7th due to him being with his mother for half of his summer break.
04-07-2025		Patient’s schedule under his father’s custody is changing due to holidays/breaks. Patient will be with his father only on April 12th, May 3, May 17, and June 29-July 27. Patient’s attendance will be impacted by this changed schedule.
03-04-2025	Jordan	Father notified on conversation with school SLP. The following message was sent: Jordan Orozco 03-04-2025 09:20 AM “I was planning to give you a recap on Saturday, but I did share with Malorie what I am currently working on, including my rationale and the therapeutic techniques I’m using. She was very receptive and mentioned that she would also implement these strategies and work on similar sounds for Mateo’s new goals. I explained to her that Mateo continues to need speech therapy. While he can be intelligible in short phrases or isolated words, his intelligibility decreases in connected or conversational speech due to articulation errors. I emphasized that his conversational intelligibility is crucial for his development, communication growth, and confidence. I also reassured her that Mateo is still young and has time to develop his speech and articulation. There was some misunderstanding on her part regarding his weekend activities. She thought that speech therapy with me would prevent him from doing other things, but I clarified that this is a 45-minute biweekly session, and there are times we cancel due to Mateo’s other activities. She was also under the impression that this therapy was being implemented due to parental critiques, but I reassured her that this therapy is necessary and is intended to support Mateo’s future. I am not sure what information mom has given her but I encouraged her to schedule a conversation with you to gain more transparency and input, as she mentioned that she has not had a conversation with you yet.”
02-21-2025	Jordan	Clinician had a approved conversation with Mateo’s school SLP Malorie. Malorie requested info on current target goals for patient and therapy techniques/activities to implement. Clinician educated Malorie on why speech sounds continue to be targeted and that is because in conversational speech Mateo can still be unintelligible. Malorie stated that Mateo’s father is reportedly “very critical of Mateo’s speech” and Mateo’s mother is stating that he “cannot do things on the weekends like soccer or go with friends because he has speech”. Clinician stated that father cancels sessions often due to Mateo’s extracurricular needs. Malorie stated that all of her information is coming from one source, which is Mateo’s mother.

Parents are divorced and have a communication coordinator who has now requested to speak with clinician (Jordan). 07-30-2023	Jordan	After therapy on 7-28-23, father informed clinician that during his most recent court hearing with his ex wife - he stated that the clinician stated that Mateo's reattached TOTs (that were released in 2021) may be attributed to after care (i.e., not completing massages, not following up with dentist, not following up with speech therapy, etc.). However it is known that reattachment of TOTs may be attributed to a variety of factors outside of after care. This statement from the clinician was said verbally in the parking lot of the clinic after Mateo's initial evaluation at NB. Per father's report, after court, Mateo's mother accused Mateo's father of suggesting that she was negligent when he commented on why Mateo's TOTs may have re-attached. Mateo's father informed clinician of this incident in the case that Mateo's mother tries to contact the office and speak with clinician or delve further into this matter. Clinician to inform office manager and director of situation.
06-05-2023	Jordan	Patient's attendance is low due to custody issues. Patient is only able to attend speech therapy when he is with his father as his mother does not agree with patient needing speech therapy. Due to holidays, recent family death, and summer beginning - Mateo's attendance has been very inconsistent. Front office is aware of patient's custody situation. Next scheduled visit is set for 6-23 at 4pm. Father stated he would try to schedule 2 extra visits the following week to help with attendance.

I do have another option - not sure if you are interested. I can offer to see Mateo Saturday 4-20-24 online via telehealth. Basically you get a link the morning of the appointment and log him in. The session will be about 40-45 min and he would just need to be set up on a phone or laptop in a quiet area.

His goals target vocalic R - specifically /ar/, /er/, /or/, and /ire/. He should remember how to correctly pronounce those but for reference /ar/ sounds like AHHHH-R, /er/ sounds like EEEEE-R, /or/ is OHHHH-R, and /ire/ is iiiii- R. :)

The re evaluation will only take 15-20 minutes to complete since we just have to run through the articulation battery of words and some sentences. He has shown gains in his articulation productions by meeting his therapy goals for the previous authorization as well as 2-4 less speech sound errors on standardized measures. I will let you know once we have his new authorization in and once my NM license comes in so we can try to hit weekly visits. I think the weekly visits will help him too with his articulation.

Unfortunately, his mother does not agree with speech therapy outside of school. So for now, I want to try and keep the session every other Friday or Saturday like we have been doing. Once she sees some documentation, maybe then she will decide to help Mateo attend some sessions when he is with her.

Whenever you have weekends with him I recommend going to home-speech-home.com and looking up his target sounds through there - vocalic er, th in final position, and r blends (pr, cr, br) so that he can practice his sounds and articulation with you! The Clinical Evaluation of Language Fundamentals 5 (CELF 5) was used to formally assess Mateo’s language skills. The following are Mateo’s standard scores on the CELF 5 - Core Language Score 102 (percentile: 55), Receptive Language Index 105 (percentile: 63), Expressive Language Index 98 (percentile: 45), Language Content Index 100 (percentile: 50), and Language Memory Index 105 (percentile: 63). According to Mateo’s standard scores on the CELF 5, where a score from 85 to 115 is considered within functional limits, Mateo’s receptive and expressive language skills are considered within functional limits for his age and do not warrant treatment.

03-04-2025 09:20 AM	Jordan Orozco	I was planning to give you a recap on Saturday, but I did share with Malorie what I am currently working on, including my rationale and the therapeutic techniques I’m using. She was very receptive and mentioned that she would also implement these strategies and work on similar sounds for Mateo’s new goals. I explained to her that Mateo continues to need speech therapy. While he can be intelligible in short phrases or isolated words, his intelligibility decreases in connected or conversational speech due to articulation errors. I emphasized that his conversational intelligibility is crucial for his development, communication growth, and confidence. I also reassured her that Mateo is still young and has time to develop his speech and articulation. There was some misunderstanding on her part regarding his weekend activities. She thought that speech therapy with me would prevent him from doing other things, but I clarified that this is a 45-minute biweekly session, and there are times we cancel due to Mateo’s other activities. She had also assumed that this therapy was being implemented due to hypercritical parental beliefs, but I reassured her that this therapy is necessary and is intended to support Mateo’s future. I am not sure what information mom has given her but I encouraged her to schedule a conversation with you to gain more transparency and input, as she mentioned that she has not had a conversation with you yet.

We practiced them in phrases and conversational speech to help improve his speech intelligibility in real-life settings. One word we’re paying extra attention to is “girl.” I provided visual and tactile cues to help him smooth out the pronunciation of this word. I’ll see him again on March 22nd. He is demonstrating consistent accuracy with the /th/ sound at the end of words during conversation, as well as with his /dr/ and /fr/ blends at the phrase level. Today, we placed a strong focus on the vocalic /er/ and vocalic /ar/ sounds. One strategy we use to support correct production of the vocalic /er/ is to remind ourselves that we want words to end with an "ER" sound rather than an "UH" sound — for example, saying watER instead of watUH. For the vocalic /ar/, our technique involves encouraging Mateo to open his mouth wider when producing the "AR" sound in words to ensure clarity and accuracy. Have a good weekend! If you choose to continue, Mateo will be able to transition to a new therapist once I relocate. I’ll also be sending over a summary of the re evaluation findings along with my clinical recommendation for continued speech therapy. The following are Mateo's standard scores on the GFTA3: Sounds in Words 66 (errors: 10, percentile: 1, age equivalent: 5 years 0 months 5 years 1 month) and Sounds in Sentences 82 (errors: 5, percentile: 12, age equivalent: 7 years 0 months 7 years 2 months). According to Mateo's standard scores on the GFTA3 ,where a standard score from 85 to 115 is considered within functional limits, Mateo continues to present with PHONOLOGICAL PROCESS/ARTICULATION DISORDER. Mateo's increased severity on the Sounds in Words section vs the Sounds in Sentences section may be attributed to the increased articulatory demand of isolated words. Mateo is observed to require speech therapy services for the following phonemes in connected speech/sentence level: vocalic r /ar, er, or, ire, ear/ and /th/ in final word position. Per clinician's clinical observations, Mateo continues to demonstrate moments of unintelligibility at the sentence level and in conversational speech due to his speech sound errors. At this age, Mateo should be 90 to 100% intelligible to familiar and unfamiliar listeners. Additionally, at this age, all of Mateo's speech sounds should be developed and adequately used in conversational speech.

During this authorization period, Mateo successfully met 4 out of his 6 target therapy goals. However, his slower progress and two unmet goals may be attributed to limited access to speech therapy services, as he is only seen twice per month. At this time, Mateo primarily presents with articulation difficulty on the vocalic /r/ and occasional errors with /th/ in the final word position. Although he is approaching 10 years of age, continued speech therapy remains appropriate due to persistent errors in vocalic /r/ production. This sound is often one of the most challenging to master, given its complex articulatory demands and variability across contexts. For some children, support is needed beyond the typical developmental range. Ongoing therapy will help Mateo achieve accurate and consistent vocalic /r/ production, which is essential for overall speech intelligibility and age appropriate social communication with peers.

I’m so sorry for the last minute cancellation, but my little sister finally went into labor this morning and she asked me to be at the hospital with her. I really appreciate your understanding! I also wanted to give you a quick update that my home was finally appraised earlier this week, and it looks like we’ll be able to close our contract. My last day with New Beginnings will be November 13th. I am working on getting a new therapist lined up for Mateo and am working with the front office to make the transition as smooth as possible. My home went under contract on October 23rd, so everything has been moving rather quickly, but I’ll be sure to keep you updated on Mateo’s new therapist before his next visit on November 15th. Thank you again for understanding and it’s been an honor to work with Mateo and see him grow.
