Abel's mother voiced concerns that Abel is not using enough words. Abel's mother estimated that Abel has approximately less than 100 words in his vocabulary and he uses 1 to 2 word utterances. A typically developing child that is Abel's age should have approximately 200 to 300 words and use 3 word utterances with 4 word utterances beginning to emerge. The language of intervention for Abel should be English. Abel's family only speaks English. Abel's mother stated that he only watches English movies and she only reads English stories to him. Speech Medical History: Abel was born at 37 weeks via a Cesarean section due to a previous Cesarean section performed on Abel's mother. Abel was the first born of a twin birth. Abel was in the NICU for 4 days because he aspirated amniotic fluid and blood. Abel has some sensory issues and times needs a deep pressure applied. Abel has experienced approximately two ear infections since birth.

Speech Assessment Summary: Abel is a 8 year 2 month old male who has been receiving speech and language therapy services at New Beginnings Pediatric Therapy Services. Abel was re-evaluated using the Goldman-Fristoe Test of Articulation Third Edition English (GFTA-3). Although Abel no longer qualifies for therapy for language, he continues to require assistance with possessive pronouns. Abel received the following standard score on the GFTA-3: Sounds-in-Words 51 (raw score: 22, percentile: 0.1, age equivalent: 3 years 10 months to 3 years 11 month) and Sounds-in-Sentences 66 (raw score: 22, percentile: 1, age equivalent: <6 years 11 months). According to the standard scores on the GFTA-3, Abel has a SEVERE PHONOLOGICAL PROCESSES/ARTICULATION DISORDER. This is the first time that Abel was able to complete the Sounds-in-Sentences section. Occupational Background History: Abel is a 3 year 11 month old male presented to initial occupational therapy evaluation with referral from Dr. Usen and formal diagnosis of other childhood disintegrative disorder (F84.3) in order to determine if skilled intervention is warranted at this time. Mother and twin bother (also evaluated on same date) present at beginning of evaluation in order to obtain pertinent information in regards to medical history, areas of concern, and current level of function. Mother reported child attended HeadStart during the previous year, however, received online schooling due to the global pandemic. Mother reported that child will be starting Pre-K in August. She uis currently unsure if child will be placed in regular education classes, combined classes, or SPED. Mother reported child will be rested of ASD the first week of August.
Occupational Medical History: Mother reported that Alex is the product of an uncomplicated pregancy. Child was born at 37 weeks via C-Section along with his twin bother. Child remained in the NICU for 4 days following birth and was discharged from hospital with mother. Mother reported child met most developmental milestones on time, however, did not crawl for very long and was able to walk until approx. 20 months (delay of approx. 8 months).

The PDMS2 was administered in order to obtain standardized testing scores and provide a baseline of function. During testing child demonstrated difficulty with the following: grasping a marker, scissor manipulation, and copying pre-handwriting strokes such as intersecting lines. Scores from the PDMS2 place child at least 1 standard deviations away from the norm as compared to peers of the same age. In addition, child's scores indicate an age equivalent of 40 months for grasping abilities and 37 months for visual motor integration skills as compared to her chronological age of 47 months. Child currently presents with a 7-10 month delay in functional abilities as compared to peers of the same age. At this time skilled occupational therapy intervention targeting visual motor integration, sensory modulation, attention, and fine motor abilities is warranted. Intervention 1x/wk for up to 4 units each with PRN visits as needed for make ups (total 104 units) is necessary in order to address the above stated concerns. Without intervention Alexander is placed at risk of falling further behind peers of the same age, poor ability to regulate emotions in her ever changing environments, and limited ability to successfully engage and participate within his environments. Occupational Assessment Summary: Abel demonstrates improved fine motor precision, visual motor integration, manual coordination, sensory processing and self­-regulation, which have increased his ability to complete activities of daily living and instrumental activities of daily living. Allowing him to participate in his routines and roles at home and in the community. At this time, skilled OT interventions are not necessary as Abel has met all of his goals and has scored ABOVE AVERAGE and AVERAGE scores on the standardized assessment Bruininks-­Oseretsky Test of Motor Proficiency (BOT­-2). Results of the evaluation were reported and explained to mother, and mother verbalized understanding of treatment plan and agreed to the discharge decision of skilled therapy.
Occupational Progress to Date: Abel demonstrates consistent progress with occupational therapy services evident by meeting 100% of previous goals with at least 70% accuracy, improved standardized assessment scores on the BOT­-2, and per clinical observations and parental report. Abel was evaluated using the Bruininks-­Oseretsky Test of Motor Proficiency (BOT-­2). Abel demonstrates age equivalency scores 4:9 years to 13:5 years compared to his chronological age of 6:2 years, placing Abel at average and above average levels on fine manual control and manual coordination of BOT­-2. Within 6 months, Able will be able to demonstrate a decrease of hypersensitivity to sensory input in order to listen to 9/10 new loud sounds without attempting to cover ears or fleeing provided with min A in order to engage in tabletop task. GOAL MET- No aversion to loud sounds Within 6 months, Able will be able to catch a ball with both hands during 9/10 attempts in 3 consecutive sessions with no verbal and physical cues in order to improve motor coordination and physical independence required to play with other peers his age. GOAL MET 10/10 consecutive sessions Within 6 months, Able will demonstrate improved auditory processing skills in order to follow at least 8/10 directions with only min A in order to increase independence in IADLS and ADLs at home. As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

For children 10 years and younger, Texas Medicaid requires a Texas Health Steps check-up every year to keep therapy going.

If your child has not had a check-up in the past year, please schedule one soon. Without this visit, your child’s therapy approval may be delayed.

Our licensed pediatric physical therapist, with close to 30 years of experience, is here to support your child’s movement, strength, balance, and coordination needs. All patients will need a new referral for PT from their pediatrician, and an initial evaluation by our PT.

As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.
