Speech Background History: Luke is a 4-year, 3-month male who was see at New Beginnings Pediatrics on February 27th, 2026 from 10:00am to 11:00am for an initial speech and language evaluation. Luke was accompanied by his mother who served at his primary informant regarding his medical history. Parent reports there is no history of speech and language delays in the family. Luke was referred due to parental and physician concerns with his receptive and expressive language skills. Currently, Luke produces approximately 100Β­words (colors, clothing items, what is that). He is reported to have a recent increase word and animal/environmental noise imitation. Luke previously received speech therapy services through ECI but has since then aged Β­out. There are no concerns with fluency or voice at this time. Parent expressed concerns about feeding difficulties. Luke's primary language is English, therefore, this evaluation was conducted in English.
Speech Medical History: There is no significant medical history or chronic conditions reported. Luke was born at 35 weeks and 5 days gestation via vaginal delivery, weighing 6 pounds, 0 ounces. During delivery, Luke reportedly turned blue for approximately one minute and required brief oxygen administration by nursing staff. The parent also reported that the placenta appeared to be ripped at the time of delivery; however, this was not medically confirmed. There were no additional reported perinatal complications, and Luke did not require admission to the NICU. Luke’s hearing and vision are within normal limits (WNL), with no current concerns or impairments noted. There is no reported history of allergies. No other surgical procedures or hospitalizations are documented in his medical history.
Speech Medications: Luke is currently prescribed leucovorin (folinic acid) 5 mg, which he has been taking for the past five months. The parent was unsure of the specific medical indication for this medication at the time of reporting.

Difficulties with pragmatics can manifest as challenges in social interactions, problemΒ­-solving, and understanding figurative language. Luke presented as reserved and passive. He demonstrated appropriate and consistent eye contact; however, he did not independently initiate conversations or engage in social routines such as greetings or farewells. Participation in both structured and unstructured interactions was limited, and he required prompting to engage. Luke exhibited fleeting attention to verbal and nonverbal cues and required frequent verbal and gestural prompts, as well as occasional tactile cues, to respond to simple directions. Overall, his pragmatic language skills appear to be emerging but remain below age-level expectations, particularly in the areas of social initiation and reciprocal interaction.
Speech Diagnosis/Impressions: Luke is a 4-year, 3-month old male who is demonstrating a Moderate Mixed Receptive and Expressive Language Disorder (F80.2). These deficits impact Luke's ability to effectively communicate with family, peers, and community members, contributing to communication breakdowns and frustration. It is recommended Luke receives services 2x/week for up to 60-Β­minute sessions. Therapy should be conducted in patient’s dominant language. Parent is aware of the diagnosis and prognosis with and without treatment. Parents are also aware of New Beginnings Pediatric Therapy's attendance policy of not falling below 90% monthly attendance. If the patient does not demonstrate consistent, regular attendance in accordance with New Beginnings Pediatric Therapy's policy, they are subject to having speech therapy services reduced or placed on hold. Luke is a strong candidate for speech therapy intervention. Parent understands the importance of carryover and the need for continuum of care within the home environment. A frequency of 2x/week for 6 months is recommended due to the Luke's need for intervention in order to prevent regression and/or failure to retain newly learned skills. Telehealth services may also be provided as clinically appropriate and according to board rules. To not warrant services at this frequency will hinder the patient’s progress and development. Discharge date is to be determined based on the patient's progress towards his goals (criteria must be met for all goals) and demonstrating a standard score of 85 or higher on the most current standardized test. The overall goal is to demonstrate age-Β­appropriate (average) and/or functional speech and language skills. Medical necessity for skilled speech and language therapy services is indicated as Luke is not able to clearly or effectively communicate his needs and wants for his personal health and safety. Unclear speech can prevent individuals from having their physical and psychosocial needs met, potentially leading to feelings of isolation, depression, and social difficulties. Therefore, early intervention is crucial to address these issues and reduce the need for more intensive, longterm services in the future. Developmental conditions refer to specific impairments that differ from the normal condition and meet the definition of medical necessity. Development is a natural state but when paired with disorder, disability or delay it indicates an abnormal state. A diagnosis of a severe speech impairment in a patient indicates an abnormal state of function, and speech-Β­language treatment services are as medically necessary for the patient as therapy are for an adult who has suffered a stroke and lost speech and language function. Impaired speech and language, loss of hearing, and swallowing difficulties all reflect deficit or loss of body functions and therefore services to treat such impairments meet the definition of medical necessity. Therefore, Luke will be seen for skilled therapy services at the recommended frequency, however treatment will not begin until the referring physician has signed this plan of care and the insurance authorization is received. - WH-questions (who, what (do), where, when): is he able to understand and answer WH-questions?
- labeling pictured or tangible items: is he able?
- identifying pictured or tangible items: is he able?
- pronouns (I, mine, he, she, yours): is he able to use?
- how many words is he able to string together to make a sentence? phrase?
- how many words does he have in his vocabulary?
- perseverations or self-stimulatory behaviors?
02-25-2026 03:45 PMLuke MannLuke (Prim) Replied: He is able to identify certain things but he does struggle with this and who what where when questions as well. He is good with puzzles (wooden puzzles, matching animals, fruits etc) regarding vocabulary I would say he has about 50 words in his vocabulary and I have heard him put up to 5 words together to say a sentence. He is good a repeating but I hope this doesn't sound strange some moments very verbal and some moments more silent.

I can show you the videos when we see you all in person perhaps?

It sounds like he may be demonstrating a mixed receptive and expressive language delay, but once I evaluate him, I will be able to provide more once I see and hear him. For understanding, receptive language is our ability to understand (comprehend wh-questions, follow directions) and expressive is our ability to answer (wh-questions, comment, decline, request).

Do you have any other concers outside of his language? like articulation?

Drinking from a straw or open cup are superior to traditional sippy cups for speech development, as they promote mature swallowing and strengthen oral muscles. Straws are excellent for strengthening lips, cheeks, and tongue retraction, while open cups encourage proper tongue elevation and swallowing patterns. Do you have other feeding concerns? Limited food and drink diet intake, choking or coughing when eating or drinking?

You would have to cal BCBS for more information:)

Yes, we can do Mondays and Wednesdays at 10:00am. As long as front office is able to input everything into the system for his authorization, he can be seen as early as Monday, March 2nd :)

I also wanted to mention that we have this wonderful new hire, her office is right next door to mine, who is able to offer you Tuesdays and Thursdays at 10:00am if that schedule works better for you.

We can also offer split days between the two of us if you preferred one day on with me! I wanted to give you this option so you know you are not just limited to my currently availability :) settling in on a schedule can sometimes be tricky, so whatever is easiest for you and your husband, you have another option for days and times :)

I wanted to inform you that we are currently experiencing an issue with our medical documentation system that is preventing both myself and our front office staff from submitting Luke’s plan of care. As a result, I may not be able to see him today. The system has been generating an error code for the past 24 hours and is not registering the plan of care submission. Our IT specialist has been working to resolve the issue and had anticipated it would be fixed this morning; however, it appears the problem is still ongoing.
