Speech Background History: Israel is a 4 year, 2 month old child who was referred for a speech and language evaluation due to concerns with overall language. Parent expressed possible concerns with autism. Parent reported the following: only pretend plays with dinosaurs, plays alone with dinosaurs, does not engage with other children, hits himself, bites on clothes, lines up toys, and has minimal use of words. Mother also reported that he will imitate phrases in movies or songs.
Speech Medical History: Per parent report, medical history is not remarkable
Speech Medications: None at this time
Speech Behavioral Observations: Attending Skills: Adequate, Response Rate: Severe delays, Level of Activity: Active, Cooperation: Active, Awareness of Others: , Reliability of Scores: Adequate, Awarencess of Environmental Events: Good, Social Interactions: Usually, Prognosis for Improved Communicative Functioning: Withdrawn,
Speech Communication Strengths:
Speech Communication Weaknesses:
Speech Background Summary: Based on parent report and clinical observation, Israel demonstrates significant difficulties in social communication and pragmatic language skills. He primarily engages in solitary, repetitive play (e.g., lining up dinosaurs) and shows limited initiation or response to peer interactions. Pretend play appears limited. Israel was observed to demonstrate reduced functional verbal communication, limited spontaneous language use, and reliance on echolalic phrases (e.g., imitating lines from movies or songs). Additional behaviors impacting social engagement include humming, self-directed behaviors (e.g., hitting himself, placing hand in mouth, biting clothing), and difficulty sustaining reciprocal interaction. These characteristics interfere with his ability to participate in age-appropriate peer play, engage in shared attention, and use language for a variety of social-pragmatic functions.
Speech Diagnosis/Impressions: Based on Israel’s standard scores on the PLS-5, parent report, and clinical observations, he presents with a PROFOUND expressive/receptive language disorder characterized by minimal functional verbal communication, limited spontaneous word use, reliance on echolalic phrases, and reduced use of language for pragmatic purposes. Israel is not consistently using words, gestures, or other communicative means to effectively express wants, needs, or thoughts. His current expressive language abilities are significantly below developmental expectations for his age. Speech therapy is medically necessary and habilitative in nature, as Israel has not yet developed foundational expressive or receptive language skills that are expected at his age. Skilled intervention is required to teach functional communication, increase spontaneous language use, support pragmatic development, and reduce maladaptive behaviors. Without intervention, Israel is at significant risk for persistent communication impairment and limited social participation.

Speech Progress to Date: Initial evaluation was administered.
Speech Rehabilitation Potential: Israel demonstrates fair to good rehabilitation potential. Although his language skills are profoundly delayed, he shows emerging imitation skills and benefits from structured support and caregiver involvement. With consistent, skilled speech-language intervention and caregiver carryover, he is expected to make measurable progress in developing functional communication skills. Prognosis for improvement is dependent upon therapy consistency, implementation of home strategies, and ongoing developmental support.
Speech and Language Long Term Goals:

Pragmatic LanguageIn 6 months, Israel will engage in reciprocal turn-taking or joint attention activities (e.g., rolling a ball, shared book reading, play routines) for at least 3 consecutive exchanges, given visual and verbal supports, in 3 out of 5 opportunities, across 3 sessions, to increase social engagement and interactive communication.
