Speech Background History: Kaeda is a 3 year, 6 month old female, who was referred by her pediatrician for a comprehensive speech and language evaluation due to concerns with delayed speech milestones. Kaeda was previously evaluated a year ago, however she did not qualify for services. Parents requested a new referral due to observing a plateau in Kaeda's language. Mr. and Mrs. Wimberly served as the informant for this evaluation. They indicated that Kaeda has regressed in her development in speech and language, and will not use as much language as she did before. It was observed that she is picking up on patterns and articulation errors that her brother makes. A child Kaeda's age should use sentences of 3–5 or more words, follow multi-step directions, ask and answer simple wh-questions, demonstrate an expanding vocabulary, engage in conversational turn-taking, and produce speech that is approximately 75–90% intelligible to unfamiliar listeners. She was evaluated using the Preschool Language Scales Fifth Edition (PLS5). This evaluation was performed in english as this is the primary language used in all settings.
Speech Medical History: Kaeda was born at 38 weeks and did not experience any difficulties during pregnancy and no stay in NICU.

Speech Diagnosis/Impressions: Kaeda is a 3 year, 6 month old female, who was referred by her pediatrician for a comprehensive speech and language evaluation due to concerns with delayed speech milestones. She was evaluated using the Preschool Language Scales Fifth Edition (PLS5). This evaluation was performed in english as this is the primary language used in all settings. The Preschool Language Scales Fifth Edition (PLS5) was administered to determine Kaeda's overall receptive and expressive skills. The following are Kaeda's standard scores on PLS5: Auditory Comprehension 82 (raw score: 36, percentile 12, age equivalent: 2;11), Expressive Communication 78 (raw score: 32, percentile 7, age equivalent: 2;6), and Total Language Score 79 (raw score: 160, percentile 8, age equivalent: 2;8). This scores indicate that Kaeda's language skills are approximately 1.5 standard deviations below the average compared to her same aged peers. Kaeda's expressive and receptive language strengths consisted of: understanding spatial concepts (in, on, out of, off) without gestural cues, understanding quantitative concepts (one, all), making inferences, understanding negatives in sentences, understanding sentences with post noun elaboration, naming objects in photographs, using words more often than gestures to communicate, using words for a variety of pragmatic functions, using different word combinations, naming a variety of pictured objects, combining three or four words in spontaneous speech, and using present progressive ing verbs. Kaeda's expressive and receptive language weaknesses consisted of: understanding quantitative concepts (some, rest), understanding analogies, identifying colors, understanding spatial concepts (under, in back of, next to), understanding pronouns (his, her, he, she, they), understanding quantitative concepts (more, most), identifying shapes (triangle, square, star, circle), pointing to letters, identifying advanced body parts, using a variety of nouns, verbs, modifiers and pronouns, producing one four or five words in a sentence, using plurals, answering what and where questions, naming described objects, answering questions logically, using possessives, and telling how an object is used. Speech therapy is recommended at this time as she is presenting with a MILD TO MODERATE MIXED RECEPTIVE AND EXPRESSIVE LANGUAGE DISORDER. The Articulation Screener was completed to assess his production of phonemes in single words. His Raw Score (number of correct productions) was 9, therefore based on scoring on PLS5, further evaluation is indicated. However, do to her limited verbal output, articulation will be targeted indirectly. Errors produced by Kaeda are as follows: Initial word position: /b/ for /p/, /t/ for "ch", /s/ for "sh", /s/ for /z/, /t/ for /k/, /w/ for /r/ Medial word position: schwa for /r/, /t/ for /k/, /d/ for voiced "th", /bw/ for /br/ Final word positions: /p/ for /t/, deletion of /t/, deletion of /g/, schwa for /r/, /t/ for /d/, deletion of /p/, /t/ for /d/ Based on formal assessment, parent interview, and clinical observation, Kaeda is presenting with a MILD TO MODERATE MIXED RECEPTIVE AND EXPRESSIVE LANGUAGE DISORDER. Speech therapy is recommended at this time as Kaeda does not communicate like other children her age. Without Speech Therapy Kenzo's safety risk is HIGH, as she cannot communicate pain or danger in emergency situations and cannot effectively communicate her wants and needs

03-23-2026 11:31 AMShekinah VelasquezHello Tricare Families,

We would like to inform you of an important update regarding therapy services. Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy.

Please note:
-The referral end date provided by your PCP/PCM will determine the authorization period for therapy services.
-If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits.
-If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly.

This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services.

If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services.

We appreciate your patience and cooperation during this transition. If you have any questions, please don’t hesitate to contact our office.

Thank you for your time and support.

03-04-2026 01:37 PMVictoria ChaconThank you so much for understanding! I know we haven't tried this before but can we schedule a zoom session for tomorrow for both Kaeda and Kenzo? We can make Kaeda's at 1:30 and Kenzo's at 3:00 if that works for you all? I have bronchitis so I wont be going into the office for the rest of the week, but I don't want to miss their sessions. Is that okay with you? :)
03-04-2026 01:49 PMVictoria ChaconSorry also, since Kenzo would be missing two sessions, we can also schedule a zoom for Friday like around 3:00. Does that work? :)
