The evaluation typically lasts about an hour. I recommend arriving 15 minutes early to allow time for paperwork. Alternatively, I can email you the documents in advance so you can complete them ahead of time and bring them with you on the day of the evaluation. Please let me know what you would prefer. I currently have an opening on 2/7 at 11:30am or 12:30pm. Please let me know if these times works for you. If not, I’ll be happy to offer additional availability for the following week.

(1.) Name and Date of Birth (2.)Any complications during pregnancy or birth? (3.)Any major illnesses or accidents since the last check-up? Any medical diagnoses (e.g., Down syndrome, etc.)? (4.)Any hospitalizations or surgeries? (5.)Were developmental milestones met within normal limits or delayed? (6.)Known allergies? (7.)Any sleeping or breathing issues? (Does your child get 8–10 hours of sleep? Wake up several times at night? Snore? Sleep with mouth open or closed? Enlarged tonsils?) (8.)Current medications? (9.)Has the pediatrician expressed concerns about nutritional growth? (10.)Any gastrointestinal issues? (11.)In a few words, please describe your concerns regarding your child’s feeding.

She does have a heart murmur that was found at 1 year old. 4) No hospitalizations/surgeries 5) Speech was delayed, she is currently still a little behind according to her teachers in head start, but no other delays. 6) No known allergies 7) No. She does sleep 8-10 hours, sometimes sleeps through the whole night sometimes wakes up one time. No snoring. Sleep with mouth closed. No enlarged tonsils that anyone has mentioned to me. 8) No medications 9) The pediatrician wants here to start feeding therapy as she is such a bad eater and she lost weight from the last time she had seen her. 10) No 11) Mia has a hard time trying new foods and sometimes she can go half a day or more than half a day without eating anything. The only things she will eat is French fries, pizza, cereal, carnita, and barbacoa. She loves her cookies also.
