Speech Background History: Iaan is a 6-year, 10-month-old male who was referred for a speech and language evaluation due to parental concerns regarding language development and social communication skills. Mother reported concerns that Iaan may experience bullying or social isolation due to difficulty interacting with peers and engaging in age-appropriate conversations. Father reported that Iaan often has difficulty getting along with other children and tends to be excluded during social situations such as family gatherings. Mother indicated that Iaan attempts to socialize with peers; however, these attempts are often unsuccessful, resulting in limited peer interactions. Per parent report, pregnancy was notable for maternal depression; however, no complications during delivery or neonatal intensive care unit (NICU) stay were reported. Medical history is otherwise unremarkable. Parents reported that Iaan has been evaluated by a neurologist, and no concerns related to fluid in the brain or other neurological abnormalities were identified. Iaan has not been diagnosed with Autism Spectrum Disorder at this time. Parents reported that he previously received speech-language services at another therapy facility. Primary parental concerns include Iaan’s difficulty with language skills, conversational engagement, and social interaction with peers. Mother stated Iaan is exposed to both languages and understands both however prefers to speak English.
Speech Medical History:
Speech Medications: None at this time
Speech Behavioral Observations: Attending Skills: Distractible, Response Rate: Mild delays, Level of Activity: Active, Cooperation: Active, Communicative Intent: With numerous prompts, Awareness of Others: Present, Reliability of Scores: Fair, Awarencess of Environmental Events: Good, Social Interactions: Moderately, Prognosis for Improved Communicative Functioning: Talkative,
Speech Communication Strengths: Easily separated from Parents, Initiate activity/dialogue, Interactive,
Speech Communication Weaknesses: Difficult to understand, Inappropriate Behaviors, Perseverations, Weakness is conversational repair (failure to request clarification, singal non-comprehension, apologize),
Speech Background Summary:
Speech Diagnosis/Impressions: Based on standardized assessment results, clinical observation, and parent report, Iaan presents with a moderate receptive-expressive language disorder with notable pragmatic language deficits. Standardized testing revealed language abilities below age expectations, particularly in areas related to conversational participation, answering questions, maintaining topic during interactions, and sustaining attention to communicative tasks. Although Iaan demonstrates emerging strengths in vocabulary and sentence production, his difficulty organizing language for conversational exchanges and responding appropriately within social interactions significantly impacts functional communication. Parents expressed significant concern regarding Iaan’s ability to interact with peers and reported that he frequently attempts to socialize but often struggles to maintain interactions with other children. According to parent report, Iaan has difficulty engaging in back-and-forth conversations and may become excluded during social situations such as family gatherings and peer interactions. Parents indicated concern that these communication challenges may place Iaan at increased risk for bullying, social isolation, and difficulty forming peer relationships as he continues to grow and navigate social environments. Skilled speech-language therapy is medically necessary to address deficits in receptive language comprehension, expressive language organization, and pragmatic communication skills. Intervention is required to support Iaan’s ability to participate in reciprocal conversations, answer questions appropriately, maintain conversational topics, and navigate social communication expectations. Without intervention, Iaan remains at risk for continued communication breakdowns, difficulty establishing peer relationships, reduced participation in social environments, and potential negative psychosocial outcomes related to social exclusion.

02-17-2026Julie MoraParent initially expressed interest in telehealth services due to the lack of available after-school in-person appointments. I confirmed with the telehealth therapist that she is available after 3:30 PM and can accommodate the family’s schedule. I contacted the parent to offer an initial evaluation (IE) appointment. Parent agreed to proceed with telehealth services while awaiting an available in-person after-school slot. Parent is aware and acknowledged that the initial evaluation (IE) will be conducted in person and treatments will be through telehealth.
