Speech Background History: Matthew Dominguez is a 3-year-old male who was referred for a speech and language evaluation due to concerns regarding speech development. Per parent report, pregnancy and delivery were unremarkable. Matthew was born full term via natural delivery with no reported complications. Parent reported that major illnesses, accidents, and medical concerns are unremarkable at this time. Developmental milestones for walking and crawling were met within expected timeframes; however, speech and language development has been inconsistent. Parent reported that Matthew attempts to talk but is not yet speaking consistently. Sleep patterns were reported to be within normal limits, with parent indicating that Matthew receives adequate sleep and does not typically wake throughout the night. No medications are currently reported. Matthew stays at home with his father during the day and is not receiving additional educational or therapeutic services at this time. Vision and hearing concerns were denied per parent report. Feeding and swallowing skills were not reported as an area of concern. Socially, Matthew reportedly attempts to play with peers and generally gets along well with other children. Primary parental concerns include speech development, as pediatrician recommendations prompted further evaluation due to concerns regarding speech and language skills. Parent reported no previous history of speech-language therapy. Mother reported that Matthew is exposed to both English and Spanish in the home environment; however, during the evaluation he was observed to primarily speak and demonstrate comprehension in English, and English appeared to be his dominant language at this time.

We just received authorization from insurance to begin services. Matthew was approved for the frequency of 2x a week starting the week of 03/16/2026.
