Mother reported that Leonardo previously required a tracheostomy for approximately one year, which has since been removed, and he is currently breathing and eating without difficulty. Parent report also indicates that Leonardo experienced facial paralysis on the right side following surgery and continues to demonstrate balance difficulties; however, he is currently receiving physical therapy services for these concerns.
Leonardo currently attends a Montessori school in Ciudad Juárez. Parent reports that he enjoys being around other children, although he has difficulty with sharing during peer interactions. Mother’s primary concerns relate to limited communication skills and expressive language development following his medical history.
All assessment procedures were administered in Spanish, as this is the primary language spoken in the home environment. Given Leonardo’s unilateral hearing loss and pending audiological intervention, it is recommended that speech-language therapy services be initiated or continued following the provision of the hearing aid in order to support development of functional communication skills and optimize auditory access for language learning. |

Patient demonstrates deficits in gross motor skills. Patient referred to physical therapy due to right sided weakness, decreased coordination, lacks balance. Patient presents with weakness, limited balance, lacks coordination, and decreased kinesthetic sense which impairs patient’s ability to meet developmental milestones with activities of daily living, routines and for overall quality of life. The patient is a 4 year 7 m onto old male who scored at 11 month age equivalence in Stationary and 13 month old age equivalence in Locomotion for Gross Motor skills on the PDMS-2 assessment indicating a poor status with skills; thus severely impacting development. Skilled physical therapy services are required to promote optimal level of developmental function, minimize risk for permanent disability, improve overall strength throughout, improve flexibility/AROM, promote functional mobility to habilitate optimal and overall functional development to keep up with peers. Patient would greatly benefit from skilled PT interventions in order to address established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved.
