Mother reported a full-term pregnancy and delivery with no complications. No surgeries or significant medical history were reported. Mother stated that Elaine experienced RSV during infancy; however, no ongoing medical concerns, accidents, or illnesses were reported. Developmentally, mother reported that Elaine did not crawl but began walking early. Mother also reported that early speech development was present. Elaine is currently enrolled in school and is not receiving any special services at this time; however, mother reported that her teacher has recommended speech-language services due to concerns with communication skills. Socially, mother stated that Elaine attempts to make friends but experiences difficulty interacting with peers due to reduced intelligibility and others having difficulty understanding her speech. Assessment of Goals/Response to Treatment: The patient presented as alert and oriented, and was an active participant in the session. Skilled occupational therapy intervention focused on enhancing psychosocial, cognitive, and self-care skills. During a simulated toileting task designed to improve problem-solving and sequencing for self-care, the patient experienced an episode of incontinence, soiling themself. This event highlights a significant deficit in functional toileting performance. To address sensory integration deficits, the session also included a messy play activity with wet and sticky textures. The patient demonstrated significant tactile defensiveness, exhibiting avoidant behaviors such as throwing the sensory materials to the floor. Maximum hand-over-hand assistance was required to facilitate even brief interaction with the textures, indicating a lack of progress toward the goal of tolerating these stimuli. This avoidant behavior also negatively impacted the patient's ability to sustain attention and appropriately initiate and terminate the activity, requiring maximal therapist support to complete the task.

Following the assessment, clinician provided parent education regarding bilingual language exposure. Parent was counseled to continue exposing the child to both languages, provided that appropriate and consistent language models are used in each language. Clinician discussed current evidence-based research indicating that bilingualism does not cause language confusion or delay. It was explained that children can successfully learn two languages when given meaningful exposure, quality input, and functional opportunities to communicate. Parent was encouraged to model language naturally, avoid limiting to one language out of concern for confusion, and focus on clear, rich language interactions in daily routines. Parent verbalized understanding and agreement with recommendations. Continued monitoring and support for bilingual language development will be provided as part of the plan of care.

The evaluation is scheduled for tomorrow, February 9th at 12:00pm. I wanted to reach out ahead of the evaluation to learn more about your primary concerns for Elaine. Please answer the following questions at your earliest convenience: 1.) Length of pregnancy, please list any complications during pregnancy or birth. 2.) Please list any major illnesses, accidents, surgeries, and/or medical diagnoses (e.g., Autism, ADHD, down syndrome, etc.) 3.) Were developmental milestones met as expected or delayed? 4.) Does your child wake up several times a night or are there instances of snoring? Does your child’s pediatrician have any concerns with tonsils? 5.) Please list any medications your child takes (e.g., prescribed, over the counter, vitamins) 6.) Is your child currently enrolled in daycare or school? 7.) Mother and pt attended evaluation and intake information was gathered during the evaluation. Elaine’s mother reported that she is exposed to Spanish when with her and is currently enrolled in a dual language pre-k class. Therapist informed mother that she was not a bilingual therapist and stated evaluation completed by bilingual therapist would be essential to accurately distinguish between a language difference vs. a true language disorder. Mother verbalized understanding. Pt will be rescheduled with Steph on 2/16/26 at 2pm.
