Speech Background History: Hailey is a 1-year, 7-month old female who was seen at New Beginnings Pediatrics on February 6th, 2025 from 9:00am to 10:00am for an initial speech and language evaluation. Hailey was accompanied by her mother who served as her primary informant regarding her medical history. Parent reports there is a history of speech and language delays within the family. Hailey was referred due to parental and physician concerns with her receptive and expressive language skills. Currently, Hailey does not produce words but it reported to babble. She is not reported to imitate word and/or animal/environmental/play noises. There are no concerns with fluency, voice, or feeding at this time. Hailey's primary language is English, therefore, this evaluation was conducted in English.
Speech Medical History: There is no significant medical history or chronic conditions. Hailey was born at 39 weeks’ gestation via emergency cesarean section due to breech presentation, weighing 7 pounds 14 ounces. At birth, Hailey experienced respiratory distress and required a four-day stay in the NICU for monitoring and stabilization. Prenatally, small cardiac septal defects (“holes in the heart”) were identified; however, follow-up with cardiology at one year of age indicated spontaneous resolution, and no ongoing cardiology care is required. Hailey's hearing and vision are within normal limits (WNL), with no current concerns or impairments noted. There is no history of allergies. No other surgical procedures or hospitalizations are reported in her history.
Speech Medications: Hailey is not taking any prescribed or over­-the-­counter medications at this time.

In a speech­-language assessment, evaluation of pragmatic skills involves observing a child’s ability to initiate and maintain interactions, participate in turn-taking, interpret verbal and nonverbal cues, and use language appropriately across social contexts. Hailey demonstrates emerging pragmatic skills, including appropriate eye contact and engagement during social interactions. She actively participates in both formal and informal social exchanges and demonstrates good emerging attention to verbal and nonverbal cues. She is able to follow basic commands with verbal and gestural prompts as needed.
Speech Diagnosis/Impressions: Hailey is 1-year, 7-month old female who is demonstrating a Moderate to Severe Mixed Receptive and Expressive Language Disorder (F80.2). These deficits impact Hailye's ability to effectively communicate with family, peers, and community members, contributing to communication breakdowns and frustration. It is recommended Hailey receives services 2x/week for up to 60minute sessions. Therapy should be conducted in patient’s dominant language. Parent is aware of the diagnosis and prognosis with and without treatment. Parents are also aware of New Beginnings Pediatric Therapy's attendance policy of not falling below 90% monthly attendance. If the patient does not demonstrate consistent, regular attendance in accordance with New Beginnings Pediatric Therapy's policy, they are subject to having speech therapy services reduced or placed on hold. Hailey is a strong candidate for speech therapy intervention. Parent understands the importance of carryover and the need for continuum of care within the home environment. A frequency of 2x/week for 6 months is recommended due to the Hailey's need for intervention in order to prevent regression and/or failure to retain newly learned skills. Telehealth services may also be provided as clinically appropriate and according to board rules. To not warrant services at this frequency will hinder the patient’s progress and development. Discharge date is to be determined based on the patient's progress towards her goals (criteria must be met for all goals) and demonstrating a standard score of 85 or higher on the most current standardized test. The overall goal is to demonstrate age­-appropriate (average) and/or functional speech and language skills. Medical necessity for skilled speech and language therapy services is indicated as Hailey is not able to clearly or effectively communicate her needs and wants for her personal health and safety. Unclear speech can prevent individuals from having their physical and psychosocial needs met, potentially leading to feelings of isolation, depression, and social difficulties. Therefore, early intervention is crucial to address these issues and reduce the need for more intensive, long­term services in the future. Developmental conditions refer to specific impairments that differ from the normal condition and meet the definition of medical necessity. Development is a natural state but when paired with disorder, disability or delay it indicates an abnormal state. A diagnosis of a severe speech impairment in a patient indicates an abnormal state of function, and speech-­language treatment services are as medically necessary for the patient as therapy are for an adult who has suffered a stroke and lost speech and language function. Impaired speech and language, loss of hearing, and swallowing difficulties all reflect deficit or loss of body functions and therefore services to treat such impairments meet the definition of medical necessity. Therefore, Hailey will be seen for skilled therapy services at the recommended frequency, however treatment will not begin until the referring physician has signed this plan of care and the insurance authorization is received. Once insurance authorization is approved, the patient will receive therapy services as recommended herein unless the frequency, duration or amount of therapy is changed by the payer or at the parent/patient’s request. All missed or cancelled visits will be made up in the current authorization period. Speech Rehabilitation Potential: Good. with consistent Speech Therapy and parental support. The prognosis with therapy is good, prognosis without therapy is guarded. Hailey will be discharged from speech therapy when all of her goals are met and she receives a standard score above ­1.5 standard deviations for all impairments, indicating that her expressive and receptive language abilities are within the mild or average range. The parents/caregivers will be responsible for incorporating her Home Management Intervention Program at home. The frequency of therapy sessions will be 2x/week for 30­-45 minutes.

Hailey was observed to attend to most models and carryover in <15-20% of opportunities. Observed imitations include but are not limited to: more, open, colors, and common animals, etc. with verbal approximation. She IND produced 'bye, down, ball, huh, oh, okay, done' etc with verbal approximation. #1.3: Within 6 months, Hailey will produce a variety of C-V combinations (CVC, CVCV, CVCVC) using age appropriate consonants with 30% accuracy across 3 consecutive sessions, improving her overall speech sound production skills, as measured by therapist collected data and observations. - Utilized Kaufman Kit 1, CVCV. Hailey was observed to attend to some models with MIN-MOD imitation. This is Rachael, CCC-SLP with New Beginnings Pediatrics. This is a friendly reminder that my office is moving from 1510 Zaragosa, Building A to 1510 Zaragosa Building B.

DIRECTIONS: When you first pull into the parking lot, drive straight all the way to the end. On your right, you will see a sign that reads “1510 Zaragoza, Building A” — turn right there. If you turn left at the sign, it will take you to the main office, so please be sure to turn right. Continue driving to the end of the parking lot; the last building is my new office location.

Please note that all future appointments will take place in Building B. If you have any questions or concerns, please contact me or front office at (915) 855-0601.
