Patient/Family Education: Discussed the finding of the examination, reviewed the POC and goals with the parent. Discussed activities/strategies available to address current concerns and how to assist patient in achieving optimal developmental milestones. Parent states understanding and agrees to provide support at home in order to progress outcomes for patient. Parent provided opportunity to ask questions and all questions were answered to the parent's satisfaction.

Physical Therapy Assessment: Child assessed using a Standardized Assessment Clinical Observations: limited muscular girth, limited balance, lacks coordination, very posture/round back and forward upper trunk and head, decreased activity tolerance Parental Report: wants to see him move better, better posture, better balance, coordination The patient is a 12-year-old male functioning at an estimated developmental level of approximately 4–5 years on the BOT 2 indicating a below average and well below average status with simple and complex tasks thus moderately to severely impacting life skills if left untreated. The patient was referred to physical therapy secondary to movement dysfunction. The patient demonstrates significant deficits in gross motor skills, including generalized weakness, decreased coordination during both static and dynamic activities, impaired dynamic balance, atypical mobility patterns, decreased activity tolerance, and reduced kinesthetic awareness. These impairments limit the patient’s ability to safely and efficiently perform age-appropriate functional mobility tasks and activities of daily living. As a result, the patient is unable to meet expected developmental milestones, which impacts participation in daily routines and negatively affects overall quality of life. Without skilled physical therapy, the patient is at risk for developing further impairments that may negatively impact the ability to perform both simple and complex daily tasks, sustain proper posture, fulfill ADLs , and limit overall quality of life. Skilled physical therapy services are required to address and promote optimal level of developmental function, minimize risk for permanent disability, improve overall strength throughout, improve flexibility/AROM, promote functional mobility to habilitate optimal and overall functional development to keep up with peers. Patient would greatly benefit from skilled PT interventions in order to address established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Results of the evaluation were reported and explained to parent(s), along w/ the diagnosis, prognosis, including the frequency, time and duration of treatment and contents of the plan of care. Family will be provided w/ a verbal report of progress noted along w/ any homework for the family as part of a home program (if applicable) at the end of every session. Parent(s) verbalized understanding of treatment plan and agreed to the initiation of skilled therapy. Within 3 months, the patient will improved joint coordination by performing smooth, coordinated movements of the upper and lower extremities during functional tasks (e.g., bouncing a ball, walking on balance beam, jumping over hurdles, tree pose on BOSU, etc ) with no more than one verbal cue in 4 out of 5 trials. Within 3 months, the patient will jump forward at least 40 inches and land safely without loss of balance or need for external support in 4 out of 5 attempts. Within 3 months, the patient will demonstrate improved lower extremity strength as evidenced by the ability to perform 20 consecutive sit-to-stand repetitions without use of upper extremity support and no loss of balance. Within 6 months, the patient will sustain one leg stance independently for 20 seconds for each side without loss of balance in 3 out of 5 trials. Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy.

Please note:
-The referral end date provided by your PCP/PCM will determine the authorization period for therapy services.
-If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits.
-If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly.

This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services.

If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services.

We appreciate your patience and cooperation during this transition. If you have any questions, please don’t hesitate to contact our office.

My name is Dolly Molina, and I am the Physical Therapist at New Beginnings. I was recently provided with your information and am reaching out to schedule your Physical Therapy Evaluation.
I currently have availability on one of the following dates:
Tuesday, January 27, 2026 at 8:00 AM (I understand this is short notice), or
Tuesday, February 3, 2026 at 1:00 PM
I hope one of these times works for you. Due to my schedule at other facilities, I am only able to offer evaluation appointments on Tuesdays during daytime hours. Once the evaluation is completed, you will be assigned to a Physical Therapist Assistant (PTA) who has greater flexibility for ongoing therapy sessions.
Please let me know if either of these times is convenient for you, or if we need to explore an alternative appointment.
