During the evaluation, Truce demonstrated minimal interest in toys or therapist directed activities and did not engage in functional or reciprocal play. Parent reported he typically prefers sensory based items and may engage in self-stimulatory behaviors, such as holding electronic devices directly to his ear when videos or music are playing. He showed a strong preference for remaining in close physical proximity to his mother, frequently sitting in her lap and reaching for her phone. Receptively, Truce demonstrated limited response to social cues and did not respond to his name when called by the therapist or parent. Parent reported inconsistent responses at home, most often when called by his father. Turn-taking is reported to be emerging in ABA therapy (attended five days per week from 9:00 AM–3:00 PM); however, this skill was not observed during the evaluation. Based on clinical observation and caregiver report, Truce presents with a severe mixed receptive-expressive language disorder characterized by the absence of functional verbal communication, limited response to auditory stimuli, poor joint attention, and significantly reduced social engagement. His communication deficits fall well below age expectations and substantially interfere with his ability to express wants and needs, participate in daily routines, and develop age appropriate social relationships. Without intensive intervention, he is at high risk for continued communication failure, increased frustration, and long-term academic and social challenges. Skilled speech language therapy is medically necessary to address Truce’s severe communication deficits and to establish a reliable functional communication system. Due to the complexity of his needs and the severity of his disorder, intervention requires the clinical expertise of a speech-language pathologist to develop individualized strategies, implement evidence-based augmentative and alternative communication (AAC) supports, and train caregivers to promote generalization across environments. Speech therapy will focus on improving functional communication, increasing joint attention, strengthening receptive language skills, and facilitating social engagement to enhance safety, independence, and overall quality of life. Based on formal assessment, parent interview, and clinical observation, Truce presents with a PROFOUND Mixed Receptive and Expressive Language Delay. We would like to inform you of an important update regarding therapy services. Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy.

Please note:
-The referral end date provided by your PCP/PCM will determine the authorization period for therapy services.
-If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits.
-If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly.

This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services.

If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services.

We appreciate your patience and cooperation during this transition. If you have any questions, please don’t hesitate to contact our office.
