Speech Background History: Sebastian is a 4-year, 4-month old male who was seen at New Beginnings Pediatrics on February 12th, 2025 from 12:00pm to 1:00pm for an initial speech and language evaluation. Sebastian was accompanied by his mother who served as his primary informant regarding his medical history. Parent reports there is a history of speech and language delays within the family. Sebastian was referred due to parental and physician concerns with his receptive and expressive language skills. Currently, Sebastian produces approximately 3-words (ma, no, yeah) and consistently produce /d/ for all word and sound productions. He is not reported to imitate words and/or animal/environmental/play noises. There are no concerns with fluency or voice at this time, however, there are ongoing feeding concerns related to a limited and nutritionally inadequate diet, compounded by medication-related appetite suppression. Sebastian's primary language is English, therefore, this evaluation was conducted in English.
Speech Medical History: Sebastian has a diagnosis of Autism Spectrum Disorder (ASD), however, per parent report, the specific DSM-5 severity level (Level 1, 2, or 3) was not identified and/or is currently unknown. Sebastian was born at 37 weeksโ€™ gestation via cesarean section following induction of labor due to suspected macrosomia (estimated fetal weight greater than 9 pounds in utero). His birth weight was 8 pounds. At delivery, the umbilical cord was noted to be wrapped around his neck (nuchal cord), resulting in a brief period of oxygen deprivation. He required admission to the Neonatal Intensive Care Unit (NICU) for four days for monitoring and stabilization. Sebastian's hearing and vision are within normal limits (WNL), with no current concerns or impairments noted. There is no history of allergies. No other surgical procedures or hospitalizations are reported in his history.
Speech Medications: Sebastian is currently prescribed Risperidone 0.25 mg, administered orally once daily in the morning for management of aggression; Quillivant XR (methylphenidate extended-release) 4 mL oral suspension, administered once daily during school hours for Attention-Deficit/Hyperactivity Disorder (ADHD); and Clonidine 3 mL oral suspension, administered once daily for management of sleep disturbance/insomnia.

Speech Diagnosis/Impressions: Sebastian is a 4-year, 4-month old male who is demonstrating a Severe Mixed Receptive and Expressive Language Disorder (F80.2) secondary to Autism Spectrum Disorder (F84.0). These deficits impact Sebastian's ability to effectively communicate with family, peers, and community members, contributing to communication breakdowns and frustration. It is recommended Sebastian receives services 2x/week for up to 60minute sessions. Therapy should be conducted in patientโ€™s dominant language. Parent is aware of the diagnosis and prognosis with and without treatment. Parents are also aware of New Beginnings Pediatric Therapy's attendance policy of not falling below 90% monthly attendance. If the patient does not demonstrate consistent, regular attendance in accordance with New Beginnings Pediatric Therapy's policy, they are subject to having speech therapy services reduced or placed on hold. Sebastian is a strong candidate for speech therapy intervention. Parent understands the importance of carryover and the need for continuum of care within the home environment. A frequency of 2x/week for 6 months is recommended due to the Sebastian's need for intervention in order to prevent regression and/or failure to retain newly learned skills. Telehealth services may also be provided as clinically appropriate and according to board rules. To not warrant services at this frequency will hinder the patientโ€™s progress and development. Development is a natural state but when paired with disorder, disability or delay it indicates an abnormal state. A diagnosis of a severe speech impairment in a patient indicates an abnormal state of function, and speech-ยญlanguage treatment services are as medically necessary for the patient as therapy are for an adult who has suffered a stroke and lost speech and language function. Impaired speech and language, loss of hearing, and swallowing difficulties all reflect deficit or loss of body functions and therefore services to treat such impairments meet the definition of medical necessity. We just received authorization from insurance to begin services. Sebastian was approved for the frequency of 2x a week starting the week of 03/09/2026. I would like to go ahead and get him scheduled in for his set times every week. I was told that you wanted the appointments to occur at 5pm, and luckily I have that time slot available Monday-Thursday. Please let me know which 2 days you would like to be scheduled and if that time slot still works best for you. Thank you and I look forward to meeting you both!
