Speech Communication Weaknesses:
Speech Background Summary: During the evaluation, Jerimyah was alert and attentive and participated fully in all tasks presented. He demonstrated appropriate social engagement throughout the session. He was friendly and cooperative. He displayed adequate eye contact, turn-taking skills, and topic maintenance during structured and unstructured interactions.
Speech Diagnosis/Impressions: Based on formal assessment results, clinical observations, and language sampling, Jerimyah presents with a PROFOUND speech sound disorder characterized by significant articulatory imprecision, reduced intelligibility, and atypical speech patterns. Errors are noted across multiple phonemes and word positions, including final consonant deletion, consonant cluster reduction, medial consonant deletion, and distortions of /s/. Speech is further impacted by slurred production, reduced speech clarity, and inconsistent pacing. Oral mechanism observations revealed limited lingual and labial range of motion, which likely contribute to reduced articulatory precision. These deficits significantly interfere with effective verbal communication and warrant continued skilled speech-language intervention. At this time, speech and language therapy is considered to be medically necessary.

Occupational Background History: Jerimiyah is a 17.4 year old boy who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Parents were present in order to serve as the primary informants throughout the evaluation. Per parents' report child was born at 40 weeks gestation. Child did not experience any complications at birth, does not have any visual or hearing deficits. Child does not have history of aggression and seasonal allergies.
Occupational Medical History: 0. Parents report a diagnosis of epilepsy and autism. Parents report last seizure was approximately a year ago. Parents also report a diagnosis of scoliosis for which he sees an orthopedic doctor.
Occupational Medications: None at this time
Occupational Behavioral Observations: Attending Skills: Distractible, Response Rate: Moderate delays, Level of Activity: Passive, Cooperation: With some prompts, Behavior: Present, Awareness of Others: Fair, Reliability of Scores: Questionable, Awarencess of Environmental Events: Moderately, Social Interactions: Withdrawn, Prognosis for Improved Communicative Functioning: Good,
Occupational Communication Strengths:
Occupational Communication Weaknesses: ADL's, Anxiety, Attention to Task, Body Awareness, Endurance, Eye-Hand Coordination, Grasp, Manual Dexterity, Problem Solving, Self-Esteem, Sensory Process/Mod, Visual Motor Skills, Visual Perceptual Skills,
Occupational Background Summary: Jerimiyah demos difficulty with in-hand manipulation skills due to delayed finger dexterity, finger isolation and palm translation skills. Fine motor skills involved the small muscles of the body that enable such functions as writing, grasping small objects, and fastening clothing. Weaknesses in fine motor skills can affect a child's ability to eat, write legibly, use a computer, turn pages in a book, and perform personal care tasks such as dressing and grooming. Jerimiyah demonstrates cognitive delays hindering executive functioning skills such as processing, problem solving, attention, sequencing and completion of tasks. Child will stop task due to difficulty processing what was asked limiting his engagement. Child also demonstrates fine motor and grasp delays in BUE evident by premature grasp patterns, spillage and difficulty with manual dexterity tasks.
Occupational Diagnosis/Impressions: Jerimiyah is a 17-year, 5-month-old male with diagnoses of autism, epilepsy, and scoliosis who presents for an occupational therapy evaluation to determine his current level of functional performance and identify the need for skilled therapeutic services. Clinical observation and formal assessment reveal profound deficits across multiple functional domains that significantly impede his ability to participate in age-appropriate daily activities. Assessment findings indicate that Jerimiyah’s functional abilities are severely impacted by underlying deficits in cognitive processing, sensory modulation, and adaptive skills. He consistently demonstrates difficulty attending to tasks, is often distractible, and follows only one-step commands, which represents a cognitive processing delay of greater than 180 months and is functionally equivalent to performance more than -3.0 standard deviations below the mean for his chronological age. This profound deficit in comprehension directly limits his ability to learn new routines, engage in pre-vocational tasks, and participate safely in his home, school, and community environments.Formal assessment of daily living skills, utilizing The Roll Evaluation of Activities of Life (REAL), yielded a standard score of 62.7, placing Jerimiyah below the 1st percentile and reflecting performance more than -2.0 standard deviations below the age-expected mean. This score quantifies significant functional impairment in his ability to perform essential self-care routines. These deficits are further compounded by significant challenges with sensory processing. Clinical observations revealed sensory defensiveness to auditory and tactile stimuli, resulting in poor self-regulation, anxiety, and social withdrawal. This difficulty modulating sensory input is well below the 50th percentile and represents a functional delay of several years, impeding his ability to tolerate academic and community settings. The combined impact of these cognitive, adaptive, and sensory processing deficits results in a significant functional impairment across all occupations. His dependence on others for basic ADLs, poor safety awareness, and inability to follow multi-step directions severely compromise his independence and place him at considerable risk for harm, preventing participation in instrumental activities of daily living necessary for the transition to adulthood.Based on the comprehensive findings of this evaluation, occupational therapy is deemed medically necessary and reasonable to address the significant and measurable deficits identified. Jerimiyah demonstrates profound delays that severely restrict his functional independence and pose ongoing safety concerns. Without skilled intervention, he is at high risk for continued functional regression and a widening of the developmental gap as he enters adulthood, leading to increased caregiver dependency and limited participation in vocational, educational, and community life. Therefore, it is the clinical recommendation of this therapist that Jerimiyah receives skilled occupational therapy two times per week. This intensive frequency is justified by the severity of his global deficits, particularly the profound cognitive processing delays, the significant safety risks associated with his poor awareness and impulsivity, and the critical need to establish foundational self-care and self-regulation skills to support a safe and functional transition to adulthood.

Assessment of Goals/Response to Treatment: Patient presented as alert and cooperative, demonstrating full engagement and participation in all therapeutic activities. The session focused on improving cognitive integration skills for enhanced functional independence. Progress was noted toward the goal of recalling emergency contact information. Following a single visual demonstration, the patient successfully recalled the first three digits of the phone number independently on 3/3 trials when provided with the established mnemonic verbal cue. Intervention then progressed to learning the subsequent three digits (8-0-8) utilizing a new mnemonic strategy paired with a visual illustration. The patient demonstrated successful acquisition of this new information, recalling the sequence independently for 3/3 trials within the session. To further address goals of sustained attention, visual processing, and problem-solving, the patient completed a 5-word search puzzle. The patient effectively utilized a compensatory strategy of verbalizing the letters of each target word while visually scanning the grid, successfully locating all 5 words. The patient's ability to learn and immediately apply new memory strategies is a positive indicator for continued progress.
