Speech Background History: Regina is a 3-year, 5-month old female who was seen at New Beginnings Pediatrics on March 4th, 2026 from 9:30am to 10:30am for an initial speech and language evaluation. Regina was accompanied by her mother who served as her primary informant regarding her medical history. Parent report there is no history of speech and language delays within the family. Regina ws referred due to parental and physcian concerns with her articulation skills. Regina demonstrates a rapid rate of speech that significantly impacts her intelligibility. Based on caregiver report, she is understood approximately 75% of the time by her parent and about 25% of the time by extended family members and unfamiliar listeners. There are no concerns with fluency, voice, or feeding at this time. Regina's primary language is English, therefore, this evaluation was conducted in English.
Speech Medical History: There is no significant medical history or chronic conditions. Regina was born full-term at 40 weeks gestation via vaginal delivery, weighing 6 pounds, 10 ounces with no reported complications or perinatal concerns. Regina's vision and hearing are within normal limits (WNL), with no current concerns or impairments noted. No other surgical procedures or hospitalizations are reported in her history.
Speech Medications: Regina is not taking any prescribed or over­-the­-counter medications at this time.

These deficits impact Regina's ability to effectively communicate with family, peers, and community members, contributing to communication breakdowns and frustration. It is recommended Regina receives services 2x/month for up to 60­-minute sessions. Given the parent’s current school schedule and limited availability. This frequency will allow continued progress while accommodating family constraints. Ongoing skilled intervention remains important to address her speech intelligibility and rate. Following the parent’s graduation in May 2026, the family plans to increase the frequency of therapy sessions as their availability becomes more flexible. Therapy should be conducted in patient’s dominant language. Parent is aware of the diagnosis and prognosis with and without treatment. Parents are also aware of New Beginnings Pediatric Therapy's attendance policy of not falling below 90% monthly attendance. If the patient does not demonstrate consistent, regular attendance in accordance with New Beginnings Pediatric Therapy's policy, they are subject to having speech therapy services reduced or placed on hold. Regina is a strong candidate for speech therapy intervention. Parent understands the importance of carryover and the need for continuum of care within the home environment. A frequency of 2x/month for 6 months is recommended due to the Regina need for intervention in order to prevent regression and/or failure to retain newly learned skills. Medical necessity for skilled speech and language therapy services is indicated as Regina is not able to clearly or effectively communicate her needs and wants for her personal health and safety. Unclear speech can prevent individuals from having their physical and psychosocial needs met, potentially leading to feelings of isolation, depression, and social difficulties. Therefore, early intervention is crucial to address these issues and reduce the need for more intensive, longterm services in the future. Developmental conditions refer to specific impairments that differ from the normal condition and meet the definition of medical necessity. Development is a natural state but when paired with disorder, disability or delay it indicates an abnormal state. A diagnosis of a severe speech impairment in a patient indicates an abnormal state of function, and speech-­language treatment services are as medically necessary for the patient as therapy are for an adult who has suffered a stroke and lost speech and language function. Impaired speech and language, loss of hearing, and swallowing difficulties all reflect deficit or loss of body functions and therefore services to treat such impairments meet the definition of medical necessity. I’m sorry for reaching out so late but I wanted to ask if you are open to moving your scheduled evaluation time down by 30 minutes due to a meeting conflict? It should end prior to the scheduled appointment time but to not risk it, is that okay with you? The new time would be 10:00am-11:00am instead of 9:30am-10:30am. If not, no worries and I understand.
