His speech was described as choppy, fragmented, and difficult to follow. These difficulties negatively impact his ability to express thoughts, participate in conversations, and understand spoken language in both academic and social environments. Sebastian’s receptive and expressive language skills were evaluated using the Clinical Evaluation of Language Fundamentals. On the Sentence Comprehension subtest, Sebastian received a raw score of 16, a scaled score of 5, a percentile rank of 5 percent, and an age equivalent of 5 years and 3 months, indicating significantly below average receptive language abilities. He demonstrated difficulty understanding complex sentence structures, including direct and indirect requests, compound and interrogative sentences, sentences containing direct objects, and sentences with prepositional phrases, which interferes with his ability to process spoken instructions and classroom language demands. Expressive language assessment revealed significant deficits. On the Word Structure subtest, Sebastian received a raw score of 12, a scaled score of 3, a percentile rank of 1 percent, and an age equivalent of 3 years and 7 months. He demonstrated difficulty producing grammatical forms including regular and irregular plurals, third person singular verbs, possessive nouns and pronouns, duration of nouns, regular and irregular past tense verbs, comparative and superlative forms, and subjective pronouns. These deficits significantly impact his ability to communicate clearly and effectively. On the Formulated Sentences subtest, Sebastian received a raw score of 0, a scaled score of 1, a percentile rank of less than 1 percent, and an age equivalent of less than 3 years. This was Sebastian’s most challenging area. He was unable to generate simple, grammatically correct sentences when provided with visual stimuli, instead producing fragmented and syntactically disorganized utterances such as β€œCan walking all the way with kids,” β€œairplane flying at the sky,” β€œshe is scrubbing rinsing,” and β€œthe cat inside the fish.” These responses reflect severe expressive language deficits that limit his ability to convey basic ideas and participate meaningfully in age appropriate communication tasks. On the Recalling Sentences subtest, Sebastian demonstrated the ability to accurately recall sentences containing up to four words; however, he exhibited significant difficulty recalling sentences exceeding four words in length. This indicates weaknesses in auditory memory, sentence processing, and syntactic organization, which further impacts his ability to comprehend and reproduce spoken language. Sebastian’s Core Language Score yielded a sum of scaled scores of 13, resulting in a standard score of 61 and a percentile rank of 0.5. These results INDICATE A PROFOUND MIXED RECEPTIVE AND EXPRESSIVE LANGUAGE DISORDER when compared to same-aged peers. At seven years of age, children are expected to demonstrate well-developed receptive and expressive language skills, including the ability to understand multi-step verbal directions, comprehend complex sentences, answer WH-questions, understand grade-level vocabulary, and make inferences based on spoken language. Expressively, a typically developing 7-year-old uses grammatically complete and complex sentences, clearly expresses ideas, retells events with appropriate sequencing and detail, participates in reciprocal conversations, and uses language to problem solve, self-advocate, and engage in academic tasks. In contrast, Sebastian’s receptive and expressive language abilities are significantly below age expectations. The severity of Sebastian’s language impairment significantly interferes with his academic performance, social interactions, and functional communication skills across settings. Sebastian presents with a PROFOUND MIXED RECEPTIVE LANGUAGE and EXPRESSIVE LANGUAGE DISORDER secondary to his diagnosis of autism spectrum disorder and ADHD. Based on standardized assessment results, clinical observations, and parent report, Sebastian presents with a PROFOUND MIXED RECEPTIVE AND EXPRESSIVE LANGUAGE DISORDER that requires skilled intervention. His language deficits are not expected to resolve without direct treatment by a licensed speech language pathologist. Without ongoing speech therapy services, Sebastian is at high risk for continued academic difficulties, reduced social participation, and long term communication impairments. Due to the severity, chronic nature, and functional impact of his language disorder, speech therapy services are medically necessary.
