Speech and Language Evaluation:

IDDateTypeEvaluation Diagnosis
2620402-05-2026Speech Initial EvaluationF80.2
Speech Background History: Claudia is a 2-year, 2-month old female who was seen at New Beginnings Pediatrics on February 5th, 2025 from 12:00pm to 1:00pm for an initial speech and language evaluation. Claudia was accompanied by her mother and father who served as her primary informant regarding her medical history. Parents report there is a history of speech and language delays within the family. Claudia was referred due to parental and physician concerns with her receptive and expressive language skills. Currently, Claudia produces approximately 5-words (mama, dada). She is not reported to imitate word and/or animal/environmental/play noises. There are no concerns with fluency, voice, or feeding at this time. Claudia's primary language is English, therefore, this evaluation was conducted in English.
Speech Medical History: Claudia is a fraternal twin and older of the pair, born one minute before her sister at 32 weeks’ gestation via cesarean section. She weighed 4 pounds at birth. The pregnancy was complicated by maternal preeclampsia. Claudia required an approximately four-week stay in the NICU but no significant perinatal complications were reported. Her hearing and vision are within normal limits, with no current concerns or impairments noted. There is no history of allergies, and no additional surgeries or hospitalizations have been reported.
Speech Medications: Claudia is not taking any prescribed or overΒ-the-Βcounter medications at this time.
Speech Behavioral Observations: Attending Skills: Inconsistent, Response Rate: Severe delays, Level of Activity: Active, Cooperation: Active, Communicative Intent: Resists, Awareness of Others: Present, Reliability of Scores: Poor, Awarencess of Environmental Events: Good, Social Interactions: Occasionally, Prognosis for Improved Communicative Functioning: Withdrawn,
Speech Communication Strengths: Follows verbal command with cues, Understood inhibitory words with signs,
Speech Communication Weaknesses: Difficult to understand, Diminished vocabulary, Irrelevent replies to questions, Poor use of eye contact, Poor vocabulary, Problems with organization and relevance, Self stimulation behaviors, Unable to answer questions, Unable to follow commands, Unable to participate in a conversation, Unable to take turns, Unintelligible, Unususal body language, Weakness is conversational repair (failure to request clarification, singal non-comprehension, apologize), Word finding difficulties,
Speech Background Summary: Pragmatics refers to the social use of language, including the ability to engage in effective communication, understand social cues, and adjust language based on context and audience. In a speech-language assessment, evaluation of pragmatic skills involves observing a child’s ability to initiate and maintain interactions, participate in turn-taking, interpret verbal and nonverbal cues, and use language appropriately across social contexts. Claudia demonstrates poor pragmatic skills, characterized by limited and inconsistent eye contact, difficulty with turn-taking, and challenges engaging in both formal and informal social exchanges. She frequently seeks desired items by grabbing them from others and may engage in hitting or biting behaviors when access is denied or when demands are placed. Claudia exhibits fleeting attention to both verbal and nonverbal cues and demonstrates difficulty responding to social bids. She requires frequent verbal and gestural prompts, as well as occasional tactile prompts, to follow basic commands. These pragmatic and social communication behaviors are consistent with characteristics commonly associated with Autism Spectrum Disorder (ASD). Based on these observed social communication behaviors, a comprehensive developmental evaluation is recommended to further assess for possible concerns related to Autism Spectrum Disorder. The parent verbalized concerns regarding possible ASD, noting differences in Claudia’s social communication skills when compared to her twin, who does not present with similar characteristics.
Speech Diagnosis/Impressions: Claudia is a 2-year, 2-month old female who is demonstrating a ΒSevere Mixed Receptive and Expressive Language Disorder (F80.2). These deficits impact Claudia's ability to effectively communicate with family, peers, and community members, contributing to communication breakdowns and frustration. It is recommended Claudia receives services 2x/week for up to 60Βminute sessions. Therapy should be conducted in patient’s dominant language. Parent is aware of the diagnosis and prognosis with and without treatment. Parents are also aware of New Beginnings Pediatric Therapy's attendance policy of not falling below 90% monthly attendance. If the patient does not demonstrate consistent, regular attendance in accordance with New Beginnings Pediatric Therapy's policy, they are subject to having speech therapy services reduced or placed on hold. Claudia is a strong candidate for speech therapy intervention. Parent understands the importance of carryover and the need for continuum of care within the home environment. A frequency of 2x/week for 6 months is recommended due to the Claudia's need for intervention in order to prevent regression and/or failure to retain newly learned skills. Telehealth services may also be provided as clinically appropriate and according to board rules. To not warrant services at this frequency will hinder the patient’s progress and development. Discharge date is to be determined based on the patient's progress towards her goals (criteria must be met for all goals) and demonstrating a standard score of 85 or higher on the most current standardized test. The overall goal is to demonstrate age-Βappropriate (average) and/or functional speech and language skills. Medical necessity for skilled speech and language therapy services is indicated as Claudia is not able to clearly or effectively communicate her needs and wants for her personal health and safety. Unclear speech can prevent individuals from having their physical and psychosocial needs met, potentially leading to feelings of isolation, depression, and social difficulties. Therefore, early intervention is crucial to address these issues and reduce the need for more intensive, long-term services in the future. Developmental conditions refer to specific impairments that differ from the normal condition and meet the definition of medical necessity. Development is a natural state but when paired with disorder, disability or delay it indicates an abnormal state. A diagnosis of a severe speech impairment in a patient indicates an abnormal state of function, and speech-Βlanguage treatment services are as medically necessary for the patient as therapy are for an adult who has suffered a stroke and lost speech and language function. Impaired speech and language, loss of hearing, and swallowing difficulties all reflect deficit or loss of body functions and therefore services to treat such impairments meet the definition of medical necessity. Therefore, Claudia will be seen for skilled therapy services at the recommended frequency, however treatment will not begin until the referring physician has signed this plan of care and the insurance authorization is received. Once insurance authorization is approved, the patient will receive therapy services as recommended herein unless the frequency, duration or amount of therapy is changed by the payer or at the parent/patient’s request. All missed or cancelled visits will be made up in the current authorization period. Any changes to this treatment recommendation will be reported to the referring physician in the next progress summary or evaluation report whichever comes due first as per state/federal regulations.

Speech Rehabilitation Potential: Good. with consistent Speech Therapy and parental support. The prognosis with therapy is good, prognosis without therapy is guarded. Claudia will be discharged from speech therapy when all of her goals are met and she receives a standard score above Β1.5 standard deviations for all impairments, indicating that her expressive and receptive language abilities are within the mild or average range. The parents/caregivers will be responsible for incorporating her Home Management Intervention Program at home. The frequency of therapy sessions will be 2x/week for 30Β-45 minutes.
Speech and Language Long Term Goals:
CategoryGoal Description

Subjective: Patient needed frequent reminders to return to task,Patient was alert and oriented,Patient was distractible and inattentive,Patient was eager to participate,Patient was uncooperative,
Assessment of Goals/Response to Treatment: The patient presented as alert and oriented, initially appearing eager to participate in preferred activities. However, she demonstrated significant distractibility and inattention when presented with structured therapeutic tasks, becoming uncooperative and requiring frequent verbal and physical redirection. Progress was noted in receptive language, where she demonstrated an emerging ability to follow 1-step directions, achieving 20% accuracy (1/5 opportunities). She did not yet demonstrate the ability to identify targeted nouns and verbs, resulting in 0% accuracy for this objective. In the area of expressive language, the patient did not yet use 1-2 word utterances or signs for functional communication, with 0% accuracy observed for pragmatic functions such as requesting or labeling. Similarly, foundational pragmatic skills remain an area of significant need, as the patient was unable to maintain joint attention or participate in simple turn-taking routines for three reciprocal turns across all presented opportunities (0% accuracy).

Dear Parents,

This is Rachael, CCC-SLP with New Beginnings Pediatrics. This is a friendly reminder that my office is moving from 1510 Zaragosa, Building A to 1510 Zaragosa Building B.

DIRECTIONS: When you first pull into the parking lot, drive straight all the way to the end. On your right, you will see a sign that reads β€œ1510 Zaragoza, Building A” β€” turn right there. If you turn left at the sign, it will take you to the main office, so please be sure to turn right. Continue driving to the end of the parking lot; the last building is my new office location.

Please note that all future appointments will take place in Building B. If you have any questions or concerns, please contact me or front office at (915) 855-0601.

NOTICE: Just another friendly reminder that I will be out of the office tomorrow, Monday, February 2nd, but will still be available by text if needed.

Kindest,
Rachael, CCC-SLP

02-04-2026 02:21 PMRachael HaduckHello! This is Rachael, CCC-SLP with New Beginnings Pediatrics. I wanted to reach out ahead of the evaluation to learn more about your primary concerns for Claudia. Are your concerns related to language, articulation (speech sounds), stuttering, social skills, or something else not listed? This information helps me select the most appropriate testing materials.

Regarding their upcoming evaluation: will one or both parents be present? Just a reminder that I will be evaluating each child individually at their scheduled time to focus on their unique needs. :)
