Jerus sleeps approximately eight to ten hours per night but wakes multiple times seeking a bottle and continues to drink milk and Pediasure from a bottle with a nipple. The family resides in a bilingual household where both English and Spanish are spoken however, throughout the evaluation, he was primarily addressed in English by family members. Jerus is also pending evaluations for Physical Therapy and Occupational Therapy. Jerus attended the evaluation accompanied by his mother and grandparents. Regarding progress toward attention goals, the client demonstrated measurable improvement. He was able to sustain attention to the self-feeding task for periods of 8-10 seconds with moderate verbal and tactile cues for redirection on 3/5 opportunities. This represents a positive gain from his baseline of 5 seconds with maximum assistance. Deficits, however, persist in maintaining an optimal state of alertness without external support. So when you have an opportunity can you answer these questions for me:

1.) Any complications with pregnancy or birth?
2.) major illnesses or accident since last check up? Any medical diagnoses (i.e., Autism, ADHD, down syndrome, etc.)?
3.) developmental milestones met within normal limits? or delayed?
4.) sleeping or breathing issues? does patient get 8-10 hrs sleep? wake up several times a night? snoring? sleep with mouth open or closed? enlarged tonsils?
5.) medications?
6.) education? (i.e., day care, preschool)
7.) vision or hearing issues?
8.) In your a few words write your parental concerns with SPEECH and LANGUAGE along with your child’s strengths.

- Jerus was born at 31 weeks premature due to pregnancy complications.

-No

- Delay in speech for his age.

4.) sleeping or breathing issues? does patient get 8-10 hrs sleep? wake up several times a night? snoring? sleep with mouth open or closed? enlarged tonsils?
-Jerus gets about 10-11 hours a night. But he wakes up about 3 times a night.

5.) medications?
No prescribed medications, but Jerus is given vitamins and supplements daily.

6.) education? (i.e., day care, preschool)
-No.

7.) vision or hearing issues?
-Jerus sees an optometrist for a lazy eye.

8.) In your a few words write your parental concerns with SPEECH and LANGUAGE along with your child’s strengths.
-He still only says a few words. Rarely uses basic phrases to ask for things (More juice, open please, etc). Jerus has a really good memory, oftentimes being able to recite and "play out" his favorite movies/shows.

9.) when did your concerns begin?
-Right around his 2nd birthday. Jerus was saying words, but not putting them together.

10) Any history of previous therapy (i.e., speech, occupational therapy, physical therapy, counseling, etc.)
- no

11) Dominate Language ? English or Spanish ?
-English is Jerus's dominant language, but has some Spanish vocabulary.

PT evaluation at 1510 N Zaragosa Suite B 1
You will turn off of Zaragosa into a drive way path where Ernie’s Cafe Restaurant is located Continue down the drive way with trees you will see the first address board that says 1510 N Zaragosa Suites A, pass this entrance, pass the alley then at the next address board it will say 1510 N Zaragosa Suites B - make a right and continue all the way down to the end and it will be Suite B- 1 . There is signage that says OT for Tots but the door says New Beginnings.

Your Speech evaluation will be at 1512 N Zaragosa C-1 /C-2 which is in the same isle but back the way you came in . I will be able to help guide you that way when I am done.

We just received authorization from insurance to begin services. Jerus was approved for the frequency of 2x a week starting the week of 02/16/2026. I would like to go ahead and get him scheduled in for his set times every week. These are the days and times that I have avaliable: Monday from 8 am- 3:50 pm and 4:40 pm-6 pm Tuesday-Thursday 8 am - 6 pm. Please let me know which of these times works best for you.
