Mother reported a history of diabetes during pregnancy; however, delivery was natural and without reported complications. Mother reported that Anthony achieved gross motor milestones, including crawling and walking, at approximately 2 years of age. Mother stated that Anthony was previously using some words; however, at approximately 2 years of age, he experienced a regression in speech and stopped using verbal language. Mother reported that approximately two years ago, Anthony underwent surgery to remove multiple teeth due to significant dental decay. Anthony is currently enrolled in full-time Applied Behavior Analysis (ABA) services Monday through Friday from 8:00 a.m. to 2:00 p.m. Mother reported that Anthony is a very selective eater and currently consumes a limited diet consisting primarily of fries and crackers; therefore, a feeding evaluation has been recommended. Socially, mother reported that Anthony does not consistently interact with peers or siblings. Mother’s primary concerns at this time are Anthony’s limited functional language, minimal spontaneous verbal output, and the presence of echolalia, which significantly impact his ability to communicate wants, needs, and ideas across environments.

Occupational Background History: Anthony is a 5.8 year old boy who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother Kenya, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born full term. Child did not experience any complications at birth, does not have any visual or hearing deficits. Per mother's report Anthony struggles with self regulation, following simple directions and with sensory integration. Mother reports high aversion to textures, smells limiting ADL such as bathing, handwashing and feeding. Mom reports child limits intake on most days to fries and peanut butter. During the evaluation Anthony demonstrated task aversion by attempting to flee testing area, falling to floor and hiding behind mother.

Subjective: Patient completed all required tasks,Patient was alert and cooperative,

Assessment of Goals/Response to Treatment: Patient presented as alert and cooperative, demonstrating good engagement in all therapeutic activities presented. During a structured tabletop task designed to address fine motor, cognitive, and psychosocial skills, the patient demonstrated progress in task initiation and completion. He initially required hand-over-hand assistance (HOHA) for 10 min to learn the motor plan for sorting colored dinosaurs into 6/6 corresponding colored bowls. Following this tactile and proprioceptive input, the patient successfully completed 10 repetitions of the sorting task independently, indicating improved motor planning and eye-hand coordination. However, deficits in self-control and sustained attention persist, as the patient required redirection for periods lasting between 30 seconds and 2 minutes to address frequent attempts to slide out of his chair and onto the floor. This behavior impacts his ability to remain seated and engaged for the duration of a functional task. In regard to self-care, the patient continues to require maximum assistance for handwashing ADLs, presenting with significant task aversion, which is consistent with his baseline. He has not yet met his goal of completing 3/4 steps with moderate assistance. Continued skilled occupational therapy is warranted to improve sustained attention, self-regulation, and tolerance for ADL tasks to increase functional independence.

Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy. Please note: -The referral end date provided by your PCP/PCM will determine the authorization period for therapy services. -If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits. -If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly. This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services. If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services. I will be seeing Anthony for his speech and language visits! :) I know he already see's Katie for feeding, so we just have to make sure we don't schedule his speech sessions on the same day as feeding. Anthony will be seen 2 times a week for speech for 30 minutes each. I have Mondays at 1:30-2:00 and on Thursdays I have 11:30-12:00 pm. Does these times work for you all? I know he also has OT with Lexi on Mondays at 9:00, but the earliest time I have isn't until 1:30. :(
