Feeding Background History: Ricardo is a 10-month-old male referred for a feeding evaluation by his pediatrician due to concerns regarding feeding difficulties and limited acceptance of solid foods. His parents, Mr. and Mrs. Serrano, served as the primary informants and provided detailed medical and developmental history. Parents report that concerns regarding feeding began following the introduction of solids at approximately 6.5 months of age. Since that time, Ricardo has not demonstrated a consistent preferred food and continues to show limited interest in solids, including purees. He frequently avoids foods, may gag with both purees and soft solids, and often engages in play-based interaction with food rather than consumption, even when appearing hungry. Parents report that Ricardo strongly prefers breast milk and will often choose breastfeeding over solid food intake. Ricardo has a history of reflux, which parents report appears closely linked to food allergy exposure. He is currently followed by a gastroenterologist to support management of reflux symptoms. His pediatrician has not expressed concerns regarding growth at this time. Bottle feeding was attempted; however, Ricardo did not demonstrate consistent acceptance and did not gravitate toward bottle feeding. Parents report that sleep patterns are currently stable and age-appropriate. Ricardo's feeding and swallowing abilities were assessed via a bedside swallow evaluation and structured mealtime observation to evaluate his current functional feeding skills and identify contributing factors impacting feeding performance.
Feeding Medical History: Ricardo was born at 40 weeks gestation via vaginal delivery. The pregnancy and delivery were largely unremarkable. Maternal blood pressure fluctuations were reported during labor; however, no neonatal complications or need for medical intervention were noted following birth. Ricardo did not require a NICU stay. Ricardo’s medical history is significant for multiple food allergies, including dairy, egg, and soy. These allergies were initially identified during breastfeeding, as Ricardo exhibited reflux symptoms and dermatologic reactions (rash) when his mother consumed these foods. Elimination of dairy, egg, and soy from the maternal diet resulted in noticeable improvement in symptoms. Parents additionally report suspected seafood allergy, as Ricardo develops a widespread rash following exposure. Soy exposure continues to be associated with reflux symptoms.

His Blue Cross Blue Shield allows 30 visits per year for feeding therapy. The evaluating therapist has recommended therapy once a week for 30 minutes. If we proceed with this treatment plan, Ricardo will use all authorized visits before the end of the year.
You have a couple of options moving forward:

Continue with the recommended treatment plan. Once insurance visits are exhausted, you may continue therapy on a self-pay basis. Self-pay rates are as follows:
30-minute treatment session: $65.00
Re-evaluation: $150.00
or Adjust the therapy frequency to twice per month. This option would allow the patient to stay within the insurance visit limit for the year.

The evaluation typically lasts about an hour. I recommend arriving 15 minutes early to allow time for paperwork. As far as food he doesn't like to eat much at all. Will take two to three spoons of fruit gerber. I constantly offer solids like beef and chicken as finger foods but will only play with them hardly even tries them.
Refuses to eat any vegetables in puree or solid form.

He also has dairy, egg and soy allergy.
