Mother stated her concerns are with the pronunciations of /s/ and stuttering when she is excited and telling a story. Patient stated that she does not want to sound the way she does anymore, and is excited to work on it. A child Hayden's age should be 100% intelligible to all communication partners, and no errors should be made. The Goldman Fristoe Test of Articulation Third edition (GFTA3) was administered to assess Hayden's articulation/phonological skills. In Sounds In Words, she obtained a Raw Score of 8 (number of errors) with a standard score of 64, resulting in a percentile rank of 1, which is more than 2.5 standard deviations below the mean when compared to her same aged peers. In Sounds In Sentences Hayden obtained a Raw Score of 18 (number of errors), with a Standard Deviation of 58, and a Percentile Rank of 0.3, which is more than 2.5 standard deviations when compared to her same aged peers. These scores are indicating that her scores are in the PROFOUND range, due to her age and the amount of errors. The only error patient demonstrated throughout the evaluation was as follows: frontal lisp for /s/ and /z/ in all word positions. It was more consistently apparent in sentences as opposed to word level. The Stuttering Severity Instrument Fourth Edition (SSI4) was administered to determine stuttering severity based on frequency, duration, and physical concomitants of disfluencies. Speech samples were obtained through reading and conversational language samples. A frequency score was derived from the percentage of stuttered syllable (%SS). During the speech samples, there was an average of 4.3% SS resulting in a task score of 10. A duration score was derived from the average length of the three longest stuttering events (.5 seconds or less), resulting in a Duration Score of 2. A physical concomitant score was derived from observations of distracting sounds, facial grimaces, head movements, and movement of the extremities. The physical concomitants observed were some facial grimaces, nostril flares, and leg or arm movements, therefore there was a physical concomitant score of 3.
