Skilled physical therapy services are required to promote optimal level of developmental function, minimize risk for permanent disability, improve overall strength throughout, improve flexibility/AROM, promote functional mobility to habilitate optimal and overall functional development to keep up with peers. Patient would greatly benefit from skilled PT interventions in order to address established delays and facilitate functional independence and performance. Assessment: Child assessed using a Standardized Assessment Clinical Observations: intermittent toe walking, lack of focal attention, weakness in LE, decreased coordination Parental Report: mother would like to see child not toe walk Patient demonstrates deficits in gross motor skills. Patient referred to physical therapy due to toe walking. Patient presents with weakness in LE, difficulty with regulation and stimming, limited balance, lacks coordination, atypical gait, and decreased kinesthetic sense which impairs patient’s ability to meet developmental milestones with activities of daily living, routines and for overall quality of life. The patient is a 3 year 11 month old male who scored at 18 month level in Gross Motor area on the PDMS-2 assessment indicating a poor status with simple and complex skills; thus severely impacting development. Skilled physical therapy services are required to promote optimal level of developmental function, minimize risk for permanent disability, improve overall strength throughout, improve flexibility/AROM, promote functional mobility to habilitate optimal and overall functional development to keep up with peers. Patient would greatly benefit from skilled PT interventions in order to address established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Results of the evaluation were reported and explained to parent(s), along w/ the diagnosis, prognosis, including the frequency, time and duration of treatment and contents of the plan of care. Family will be provided w/ a verbal report of progress noted along w/ any homework for the family as part of a home program (if applicable) at the end of every session. Parent(s) verbalized understanding of treatment plan and agreed to the initiation of skilled therapy.

ST offered to change session times to 6 pm starting Wednesday (3/11) and mom agreed. Sessions with ST will be on Mondays and Wednesdays @ 6 pm starting week of 3/16.
03-03-2026Julie MoraParent called on 3/2/26, regarding concerns about scheduling her son’s appointments. She asked who she could speak with about changing his schedule. I informed her that each therapist manages their own schedule and that she would need to speak directly with her son’s therapist regarding any changes. Parent stated that the current Speech Therapy (ST) appointment on Mondays at 5:00 PM no longer works due to traffic difficulties, making it challenging to arrive on time for a 30-minute session. She expressed that the commute is not worthwhile under the circumstances and requested a later time on Mondays. If a later Monday appointment is not available, she would prefer to keep only the Saturday appointments. Parent also expressed frustration, stating that we have many therapists, and she does not understand why she must accommodate the therapist’s schedule rather than the therapist accommodating her son. I reminded parent that we have made efforts to accommodate her schedule as best as possible. However, if the therapist does not have a later Monday time available, the patient may need to attend therapy once per week for the time being. Therapist will coordinate available times, pending new schedule.
03-02-2026Lailani ChehedehOn Saturday, 2/28, Mother inquired with therapist Lailani regarding additional availability during the week for Lucas’ speech therapy sessions. Mother was informed that, at this time, Lailani’s only availability to see Lucas remains on Saturdays. Mother expressed that Saturday sessions are becoming increasingly difficult due to her daughter’s volleyball games. As previously discussed, Mother confirmed plans to transition services to Lauren after 3/16 to accommodate weekday scheduling needs. Mother continues to report that weekday appointments work better for her family’s schedule. Lauren has since provided available weekday time slots and dates, which Mother has agreed to. Please refer to patient messages for further details regarding confirmed scheduling.
02-28-2026Lailani ChehedehMother was previously provided with the available Saturday schedule prior to the initiation of speech therapy services and agreed to the assigned time at that time. After services had begun, mother requested a change . Mother will attend the next two scheduled Saturday sessions as originally assigned, after which services will transfer fully to Lauren SLPA on 3/16. Mother was informed of the transition plan and acknowledged understanding of the scheduling change.

Therapist reminded mother that ST and Feeding could not take place on the same day limiting the days pt could be seen. Mother asked if Wednesdays at 5pm were available. Therapist informed mother that she did not have that availability. Mother wanted to know if there was another therapist that had that time. Therapist told mother that she would relay information to front office. Spoke to front office/Teresa; no availability. 02-21-2026Lailani ChehedehMother was informed that the current Saturday appointment time will be at 1:30 p.m., and she agreed to the updated schedule. During the session, behaviors were noted throughout. Lukas removed his shirt, threw items, and softly hit the clinician. Behaviors were redirected, and calming strategies were implemented, including decompression breaks, allowing him to go under the table, minimizing lighting, and placing a visual timer. Mother was provided with verbal feedback regarding how the session went and agreed that these behaviors are consistent with those observed at home.

At this time, we do not have any after-school appointments available. However, we do have daytime availability between 9:00 a.m. and 1:00 p.m.

I hope you’re doing well. I wanted to kindly follow up regarding the recent change request to your child’s feeding therapy schedule.
I also want to share that we truly care about our patients and their families. We completely understand that work, school, and other responsibilities can sometimes make it difficult to attend therapy consistently. We always try to be as accommodating as possible to help make scheduling easier for you. However, as mentioned previously, our feeding therapists do not currently have afternoon availability. Our clinic hours are 8:00 AM–6:00 PM, and any schedule adjustments must remain within those hours.
One option we can offer is Telehealth for feeding therapy. To give you some peace of mind, we have seen great success and meaningful progress with our Telehealth sessions. If you’re open to this option, we would be happy to check whether one of our feeding therapists has later Telehealth appointments available.

If Telehealth is not something you would like to pursue at this time, we can place your child on a short hold until a more suitable time becomes available with our therapist.
Please let us know how you would like to move forward. We’re here to support you and want to work together to find the best solution for your family. Thank you for your time!

The evaluation typically lasts about an hour. I recommend arriving 15 minutes early to allow time for paperwork. Alternatively, I can email you the documents in advance so you can complete them ahead of time and bring them with you on the day of the evaluation.
For this upcoming week, I currently have an opening on 12/10/2025 at 10:00 AM and 12/10/2025 at 5:15 PM. Please let me know if this time works for you. If not, I’ll be happy to offer additional availability for the following week.

2. No major illnesses or accident since last check up. He was diagnosed with/Autism and ADHD on March of 2024.
3. He is delayed with his milestones. Evaluation from PT she mentioned he was about an 18 month old.
4. No breathing issues but he does struggle with his sleep he can be a night owl at times or wakes up at random times like 3, 4, 5 am and has full energy. For the most part he does sometimes get 6 or 8 hrs sleep it depends how tired he is. No snoring. I believe he sleeps with his mouth closed. No enlarge tonsils that I’m aware of.
5. No medications, I do give him a powdered probiotic for tummy and drops of magnesium for sleep but they don’t work all the time.
6. Yes, I’ve enrolled him in daycare and they have a PK-3 program like school between 8am-12pm. The it rolls over to daycare for the day.
7. I just took him to his yearly optomology exam on 01/19/2026 and everything came out fine he doesn’t need glasses yet. He did test in 2025 with myopia and stigmatism in his eyes but not strongly to wear glasses.
8. My son is non-verbal at 4 years old so my concerns as a mother is will he talk ? and if? I’m just wondering if he will need an AAC translator and will we be approved for one? He is a very smart boy and as a mother it’s just naturally that I do understand him even tho he doesn’t use actual words. His strengths right now is a little sign language β€œbasic words” like more, please, all done when he wants something in particular he usually just gets it and brings it to me and says more. I’m teaching him to say β€œplease” I do

There may also be times when I will need to reschedule due to time out of the office and those visits will need to be made up as well. But yes, some kids are brand specific for food items.
