Occupational Background History: Adilyn is an 18 month old girl who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. (Parent), was present in order to serve as the primary informant throughout the evaluation. Per (parent's) report child was born at 40 weeks gestation. Child did not experience any complications at birth, does not have any visual or hearing deficits. Child does not have history of aggression and seasonal allergies.

Child currently demonstrates attention span approximately 5 seconds before attempting to flee. Child struggled to initiate and/or complete simple one step directions with max assistance via visual demonstrations, and verbal prompting for motivation, task initiation and redirection to task. Child exhibits limited attention evident by fleeing/escaping activities presented.

Assessment of Goals/Response to Treatment: The patient presented as eager to participate in today's skilled occupational therapy session, which focused on improving cognitive, visual-motor, and self-care skills. The patient required frequent verbal reminders to maintain sustained attention on therapeutic tasks. Intervention strategies included positive reinforcement and structured breaks to facilitate successful transitions between activities. Progress was noted in cognitive and visual processing skills during a structured play activity; with the provision of initial visual cues, the patient successfully sorted shapes and colors and then transitioned to correctly placing body parts on a Mr. Potato Head toy, demonstrating improved problem-solving and matching to sample. During the session, the patient exhibited instances of agitation, including hitting and pinching a caregiver, which required verbal cues for redirection to de-escalate the behavior. Significant gains were observed toward the goal of increased independence with self-care. The patient actively participated in the handwashing routine, successfully completing 2 of the 3 targeted steps (rubbing hands with soap and rinsing hands) with moderate verbal and physical cues. This marks a notable improvement from requiring maximal assistance at baseline and represents 75% progress toward the stated goal.
