She reported that the patient previously underwent surgical intervention in September, including a tonsillectomy, adenoidectomy, and correction of a deviated septum. Parent was messaged at 3:35 pm to notify therapist when they arrived. A minute later mother checked in, therefore therapist went outside to retrieve patient. They were not in the parking lot or the lobby. Therapist messaged parent letting her know that she did not see them. Mother did not respond, nor was he visible in the parking lot or lobby. Therapist messaged mother at 3:43 stating she would have to reschedule the session. Shortly after, father called clinic to inform that they were in the parking lot, however when front desk spoke to him, he stated he was at the wrong address. Session will be rescheduled.

We would like to inform you of an important update regarding therapy services. Due to a recent change in Tricare’s process, all patients are now required to be seen by their Primary Care Provider (PCP/PCM) and have an updated referral submitted to Tricare before we can request authorization for therapy.

Please note:
-The referral end date provided by your PCP/PCM will determine the authorization period for therapy services.
-If possible, we recommend requesting a referral period of up to one year, as this will help reduce the number of required follow-up visits.
-If you are unsure whether a current referral is on file, please contact your PCP/PCM or Tricare Referral Management directly.

This change began earlier this month. While we are actively reviewing all existing referrals, we kindly ask for your assistance in scheduling an appointment with your PCP/PCM and ensuring an updated referral is submitted to help avoid any delays in therapy services.

If you are able to view your referral end date, please be aware that your child will need to be seen again before that date to continue services.

We appreciate your patience and cooperation during this transition. If you have any questions, please don’t hesitate to contact our office.

So when you have an opportunity can you answer these questions for me: 1) Any complications with pregnancy or birth? How many weeks was he born at? 2) Major illnesses or accidents since last check up? Any medical diagnoses (i.e., Autism, ADHD, down syndrome, etc)? 3) developmental milestones met within normal limits? or delayed? What age did he begin to use his first single words/combined words? 4) sleeping or breathing issues? does patient get 8-10 hours of sleep? Wake up several times a night? Snoring? sleep with mouth open or closed? Enlarged tonsils? 5) medications? 6) education? (i.e., daycare, preschool) 7) vision or hearing issues? 8) In a few words write your parental concerns with SPEECH and LANGUAGE. 9) when did your concerns begin? 10) Any history of previous therapy (i.e., speech, occupational therapy, physical therapy, counseling, etc).

He didn't walk until 3 weeks after his 1st bday and didn't talk until he was almost 3 What age did he begin to use his first single words/combined words? He said Moma and dada at 3 combined words was at 4 I thought him essential words in sign language, such as food, cup, hungry and more because he would just cry and didnt talk
4) sleeping or breathing issues? does patient get 8-10 hours of sleep? Wake up several times a night? Snoring? sleep with mouth open or closed? Enlarged tonsils? He used to snore due to enlarged tonsils and othe obstruction but since having them reduced he sleeps fine no snoring and well rested
5) medications? He takes vyvanse 40 MG
6) education? He started a program at 2 called babies can't wait due to him not talking which started as in home speech therapy then early head start and now in the 6th grade he has an IOP due to ADHD and dyslexia.
7) vision or hearing issues? No
8) In a few words write your parental concerns with SPEECH and LANGUAGE. Joshua had always been a mouth breather due to not being able to breathe out of his nose, so he would talk congested, and even though he is no longer congested or having those issues breathing, he still talks as if he is a mouth breather

The only thing is that on the following week, December 16th, I already have a patient scheduled for a makeup visit at that time, so I can move his visit just for that week. I can either see him twice next week - one on Monday December 8th at 3:30 and then Tuesday December 9th at 3:30, or I can see him the following week on Monday December 15th at 4:30, but I know AJ's speech sessions are at 3:00 on Mondays so there would be a gap between her and Josh. Let me know what works best for you all :)
