Speech Background History: Luka, a young male patient, was referred for a speech and language evaluation following concerns from his pediatrician regarding delays in expressive language development. He was accompanied to the evaluation by his mother, who provided relevant developmental and medical history. Luka was reported to be born full term through an induced delivery with no complications before, during, or after birth. Developmental milestones were generally within functional limits, as he began crawling around 6 months of age and initiated walking at approximately 18 months. His first word was produced around 2 years of age. Luka stays at home with his mother during the day. According to parent report, he gets along well with other children but tends to prefer playing alone, although he engages appropriately with his sister and cousins during familiar routines. Hearing was screened at birth, during which Luka initially failed in the left ear; however, a repeat hearing screen was completed shortly afterward, and he passed without further concern. Luka hears both English and Spanish at home; however, per mother report, he primarily attends to and understands English more consistently. During the evaluation session, Luka demonstrated understanding of English-directed tasks and responded more readily to English models. For this reason, therapy is recommended in English with Spanish support as needed to ensure successful comprehension and functional carryover within his bilingual home environment. At the time of the evaluation, Luka’s mother reported that he currently uses approximately four consistent words and is not yet producing phrases or combining words into two-word sentences. She expressed concern that Luka has difficulty communicating his wants and needs and continues to rely heavily on gestures or leading behaviors. Although Luka has recently begun to use more verbalizations, his expressive skills remain significantly below developmental expectations for his age. Observations during the evaluation were consistent with parent report, as Luka demonstrated limited spontaneous verbal output, reduced vocabulary, and minimal attempts to imitate modeled words.

I will also be scheduling in some make up visits to catch up. My daughter will be having surgery April 1st, so I am actually going to be working this coming Friday and Saturday for the days I will be out for her surgery. I will be adding him to my Friday for sure.
