Mom acknowledged message and treating therapist that reached out as well. Called mom today again @11:05 to check in and asking if she had any questions or concerns about the message that was sent last night. Mom had a couple questions about when she would be starting therapy, but I did clear up her confusion and confirmed if she had any more questions. Mom stated she was happy with the change and with us being able to accommodate her and Reyna.

01-21-2026	Delia Gomez	Documentation Note – Parent of Reyna Gonzalez The first therapy session scheduled following the New Year was on 01/07; however, the session was canceled by the therapist due to illness (influenza) and loss of voice. On 01/08 at 2:30 PM, Reyna was scheduled for therapy; the parent was a no-call, no-show. Upon follow-up, the parent indicated availability only for sessions scheduled at 5:00 PM or later. The following week, Reyna was scheduled on 01/12 and 01/15 at 5:00 PM. The parent was again a no-call, no-show for both appointments. On 01/19, the therapist canceled the scheduled session due to a family emergency (daughter in the emergency room). Reyna was subsequently scheduled on 01/21 at 5:00 PM; the parent contacted the office that morning to cancel the session, citing concerns regarding inconsistent scheduling and stating that 5:00 PM sessions had not been provided. Documentation reflects that multiple 5:00 PM appointments were scheduled and offered, which were either not attended or canceled by the parent.
01-21-2026	Julie Mora	Parent called this morning asking for her daughter to be discharged from services. She said that she is having trouble with the therapist scheduling appointments. She said that the therapist schedules appointments without her confirmation and that she receives messages letting her know the date and time. I asked her if a change of therapist would help and she said yes. She requested if appointments can be after 5:00 and begin in February. She sounded very frustrated and ready to discharge. I let her know I would forward you this information to our director and see if we can accommodate her so she does not have to discharge. We found another therapist to accommodate the times needed. Both therapist will be notified.

We do have another therapist available who can accommodate your preferred times via telehealth. Her name is Daniela, and she will be reaching out to you soon to confirm treatment days and times. Please note that Daniela is expecting in April and will be taking the entire month off to spend time with her baby. She will resume therapy at the beginning of May and will be front-loading appointments, before her time off, to ensure Reyna continues to receive the recommended number of sessions. I would also like to take a moment to review our attendance and cancellation policies to help support Reyna’s treatment schedule. If a child’s attendance drops below 90% within the current authorization period and requested make-up sessions or other interventions are denied, the child may be discharged for noncompliance or poor attendance. Appointments canceled within 24 hours of the scheduled time will count as a noncompliance. After three non-compliances, an additional occurrence will result in immediate discharge. Exceptions to the 24-hour policy may be made in the event of an emergency involving you or the patient. You may review these policies at any time in the documents you signed during registration.
