Speech Background History: Ava is a 5 year, 6 month old child with a history of a full-term pregnancy and no reported prenatal, perinatal, or postnatal complications. Developmental milestones for gross motor skills were reportedly achieved early, as Ava walked and crawled at an early age. Ava’s mother reported concerns regarding Ava’s communication skills beginning at an early age, specifically related to delayed language development. Ava is currently homeschooled and receives one-on-one services through the public school system. Mother reported concerns related to Ava’s previous school placement and stated she no longer felt comfortable returning Ava to that setting. Mother expressed ongoing concerns regarding Ava’s social communication skills, noting that Ava frequently prefers to play independently, demonstrates limited use of functional and meaningful words, and often produces scripted or repetitive speech rather than spontaneous language. These communication difficulties impact Ava’s ability to effectively express wants and needs, engage in age-appropriate interactions, and participate in daily routines. Ava has been diagnosed with Autism Spectrum Disorder and Speech Impairment through the school district. Mother’s primary concerns include Ava’s limited functional communication, reduced spontaneous language, and decreased engagement in meaningful verbal interactions across environments
Speech Medical History:
Speech Medications: None
Speech Behavioral Observations: Attending Skills: Distractible, Response Rate: Moderate delays, Level of Activity: Passive, Cooperation: Passive, Communicative Intent: With numerous prompts, Awareness of Others: Present, Reliability of Scores: Fair, Awarencess of Environmental Events: Good, Social Interactions: Usually, Prognosis for Improved Communicative Functioning: Withdrawn,
Speech Communication Strengths: Follows verbal command with cues,
Speech Communication Weaknesses: Delayed echolalia, Difficult to understand, Diminished vocabulary,
Speech Background Summary:
Speech Diagnosis/Impressions: Ava is a 5 year, 6 month old child who presents with a severe receptive and expressive language disorder as evidenced by significantly below age-level performance on standardized assessment measures, clinical observation, and caregiver report. Ava’s language abilities demonstrate a substantial deviation from expected developmental norms for a child of her chronological age. Ava exhibits limited comprehension of spoken language without the use of visual or verbal cues, minimal spontaneous and functional expressive language, and impaired social communication skills, including reduced joint attention and initiation of interaction. These deficits significantly restrict Ava’s ability to effectively communicate basic wants and needs, follow daily routines, participate in learning activities, and engage in age-appropriate social interactions across environments. The severity of Ava’s communication impairment results in frequent communication breakdowns, which reportedly lead to significant frustration and unsafe behaviors when she is unable to express herself effectively. These functional and safety concerns underscore the necessity for skilled intervention. Without direct, ongoing speech-language therapy provided by a licensed speech-language pathologist, Ava is at high risk for continued communication failure, increased maladaptive behaviors, and limited progress toward functional independence. Based on the severity, persistence, and functional impact of Ava’s receptive and expressive language deficits, skilled speech-language therapy is medically necessary to address impairments that cannot be adequately managed through caregiver support.

Occupational Background History: Ava is a 5.6 year old girl who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. (Parent), was present in order to serve as the primary informant throughout the evaluation. Per (parent's) report child was born at 40 weeks gestation. Child did not experience any complications at birth, does not have any visual or hearing deficits. Child does not have history of aggression and seasonal allergies.
Occupational Medical History: Per parent report, medical history is not remarkable
Occupational Medications: None at this time
Occupational Behavioral Observations: Attending Skills: Distractible, Response Rate: Moderate delays, Level of Activity: Active, Cooperation: With numerous prompts, Behavior: Present, Awareness of Others: Poor, Reliability of Scores: Questionable, Awarencess of Environmental Events: Rarely, Social Interactions: Withdrawn, Prognosis for Improved Communicative Functioning: Good,
Occupational Communication Strengths:
Occupational Communication Weaknesses: Self-Esteem, Endurance, ADL's, Attention to Task, Cooperative, Problem Solving, Self-Control, Sensory Process/Mod, Social Interaction, Visual Motor Skills,
Occupational Background Summary: Child demonstrates significant difficulty with self regulation skills highly likely due to sensory processing difficulties. Child currently demonstrates attention span approximately 20 seconds before attempting to flee. Child struggled to initiate and/or complete simple one step directions with max assistance via visual demonstrations, and verbal prompting for motivation, task initiation and redirection to task. Child exhibits limited attention evident by fleeing/escaping non-preferred activities, and need for constant redirection.
Occupational Diagnosis/Impressions: Diagnosis/Impressions Ava is a 5-year, 7-month-old female who presents with significant global developmental delays impacting her functional participation across all areas of occupation, including activities of daily living, school-related tasks, social participation, and play. Formal, standardized assessment utilizing the Peabody Developmental Motor Scales, Second Edition (PDMS-2) reveals severe deficits in motor skills. Performance on the Grasping and Visual-Motor Integration subtests yielded standard scores of 3, which is more than -2.0 standard deviations below the mean and places the patient in the 1st percentile. These scores are well below the 50th percentile and correspond to age equivalencies of 20-26 months and 21 months, respectively, indicating a profound developmental delay of approximately 41 to 46 months. The reliability of these scores is considered questionable due to significant distractibility and difficulty with cooperation observed during the evaluation; however, the results are consistent with clinical observations of functional performance. These severe fine motor and visual-motor deficits directly and pervasively impede Ava's ability to participate independently in age-appropriate activities. Functionally, she is unable to perform essential self-care tasks, requiring assistance with handwashing, self-feeding with utensils, and managing clothing fasteners for dressing and toileting. These self-care skills reflect a delay of at least 19-31 months. Her inability to hold a crayon with a functional grasp, copy simple shapes, or cut with scissors prevents meaningful participation in typical kindergarten readiness activities, impacting her ability to engage in the educational curriculum. Her dependence on caregivers for these fundamental routines compromises the development of motor planning, coordination, and functional independence. Compounding these motor deficits are significant challenges in sensory processing and emotional regulation. Based on caregiver report and clinical observation, Ava presents with a sensory-seeking profile, characterized by a constant need for movement, crashing into objects, and seeking intense proprioceptive input. This is coupled with poor self-regulation, manifesting as frequent emotional outbursts, significant difficulty transitioning between activities, and an inability to calm independently following overstimulation. These underlying sensory and regulatory challenges severely impact her attending skills, which were observed to be highly distractible, and impair her ability to engage in cooperative play. She demonstrates a preference for solitary play, poor initiation of peer interactions, and difficulty with sharing, reflecting play and social skills that are significantly more than six months below her chronological age and which limit her ability to form friendships and participate in cooperative learning environments. The combination of severe, quantifiable fine and visual-motor delays, significant sensory dysregulation, and profound functional deficits in self-care, play, and social participation renders the patient unable to function at an age-appropriate level. The pervasiveness and severity of these deficits establish a clear medical necessity for skilled Occupational Therapy intervention to reduce these deficits, improve safety, and support functional independence. Therefore, ongoing Occupational Therapy is medically necessary and recommended at a frequency of two (2) sessions per week. This intensive frequency is justified by the profound severity of the patient's delays, with standardized scores falling more than -2.0 standard deviations below the mean and age-equivalent deficits exceeding 40 months. The patient's functional impairments cross all domains of occupation, creating safety concerns related to poor environmental awareness and sensory-seeking behaviors, and requiring significant adult assistance for basic self-care and participation. Without intensive, skilled intervention, the significant gap between her functional abilities and those of her peers is expected to widen, leading to further functional limitations and limiting her potential for future independence in home, school, and community environments.

I hope this message finds you well in the New Year. We are currently preparing to release some patients from our Speech Therapy waiting list, and before doing so, we would like to inform you of our available treatment times.

At this time, we do not have any after-school appointments available. However, we do have daytime availability between 9:00 a.m. and 1:00 p.m.

Please note that we are unable to guarantee a specific treatment time at this moment, as scheduling will be finalized once insurance approvals are received.

If these times work for you, please message us to confirm your interest, and we will send you a scheduling link to arrange an initial evaluation.

Thank you, and we look forward to hearing from you.

Kind regards,

These are the days and times that I have available: Monday 8am-4pm, Tuesday & Thursday 1:30pm-4pm, and Wednesday 8am-6pm. Please let me know which of these times works best for you. 02-23-2026 04:01 PMDaniel ValdiviaGood afternoon, this is Daniel Valdivia, a Certified Occupational Therapy Assistant at New Beginnings. I will be seeing Ava for her OT sessions and was hoping we can connect to schedule appointments for her. I will attach the days and times I currently have open, please let me know if any of these work so we can try to get her scheduled ASAP. Thank you!

Monday: 11, 12, 1
Tuesday: 9, 10, 11, 12, 1
Wednesday: 8, 12, 1
Thursday: 9, 12
02-23-2026 10:33 PMAva BillyAva (Prim) Replied: She has speech on Mondays and Wednesdays at 9:30am at the same location. If we can get her in before or after with not too long wait in between, that would be really helpful because she also gets speech and ot from the district on Thursdays. If that is not possible, I'll take Tues & Thurs 9am...

02-24-2026 12:22 PMDaniel ValdiviaHi, I noticed that Ava is scheduled for speech the next two Tuesdays at 9:30. Would you like to move those OT appointments to 10:00 AM, right after speech, so she can continue receiving both services that week and not fall behind? This will only affect the Tuesday appointments for the next two weeks. After that it will be Tuesdays at 9. Please let me know if that works and if there are any questions :)
