Speech Background History: Naomi is a 1-­year, 10-month old female who was seen at New Beginnings Pediatrics on January 15, 2026 from 2:30pm to 3:30pm for an initial speech and language evaluation. Naomi was accompanied by her mother who served as her primary informant regarding Naomi's medical history. Parents report there is a history of speech and language delays within the family, stating her song, Naomi's brother currently receives both speech and occupation therapy through New Beginnings Pediatrics. Naomi was referred due to parental and physician concerns with her receptive and expressive language skills. Currently, Naomi produces approximately 15-words (what's that, apple, mommy). She is reported to have an increase in language and imitation within recent months. There are no concerns with fluency, voice, or feeding at this time. Naomi primary language is English, therefore, this evaluation was conducted in English.
Speech Medical History: There is no significant medical history or chronic conditions. Naomi was born at 40 weeks’ gestation via vaginal delivery, weighing 6lbs, 12oz, with no reported complications or perinatal concerns. Naomi's vision and hearing are within normal limits (WNL), with no current concerns or impairments noted. There is no history of allergies. No other surgical procedures or hospitalizations are reported in her history.

It is recommended Naomi receives services 1x/week for up to 60­minute sessions. Therapy should be conducted in patient’s dominant language. Parent is aware of the diagnosis and prognosis with and without treatment. Parents are also aware of New Beginnings Pediatric Therapy's attendance policy of not falling below 90% monthly attendance. If the patient does not demonstrate consistent, regular attendance in accordance with New Beginnings Pediatric Therapy's policy, they are subject to having speech therapy services reduced or placed on hold. Naomi is a strong candidate for speech therapy intervention. Parent understands the importance of carryover and the need for continuum of care within the home environment. A frequency of 1x/week for 6 months is recommended due to the Naomi's need for intervention in order to prevent regression and/or failure to retain newly learned skills. During non-preferred FM activity, pt required 9 VC and PA to initiate, demonstrating decreased intrinsic motivation, impaired exec functioning, and limited task initiation skills, remaining below goal of =5 cues. Once engaged, pt required continuous cueing to sustain participation and complete activity. During structured block-stacking activity targeting bilateral integration, crossing midline, and eye-hand coordination, pt demo improved sustained attn, engaging for up to 20 sec in 2/5 trials w/ MAX VC and PA, compared to prior baseline of 5–10 sec. Despite improvement, pt required frequent redirection due to distractibility and decreased task persistence. Motor performance characterized by decreased coordination, inconsistent force grading, and need for PA to align and stabilize objects during stacking. Pt exhibited multiple episodes of dysregulation throughout session, including SIB (head hitting), increased agitation, and difficulty w/ transitions, particularly during presentation of non-preferred tasks. These episodes required immediate MAX VC and PA to ensure safety. Therapist implemented co-regulation strategies including deep pressure input and proprioceptive activities, resulting in partial de-escalation and return to task in 1/3 opportunities. Pt demonstrated limited ability to independently utilize learned coping strategies, requiring full therapist support for regulation. During psychosocial intervention, pt was provided opportunity for parallel play in proximity to peer. Pt required constant MAX A to inhibit aggressive impulses and maintain safe behaviors. Pt unable to engage in reciprocal interaction or shared attention, indicating deficits in social participation, impulse control, and emotional regulation. Pt demonstrates minimal but measurable progress in sustained attn; however, continues to present w/ significant deficits in self-regulation, safety awareness, impulse control, exec functioning, and social participation. Persistent SIB and aggressive bx, particularly during non-preferred tasks and transitions, significantly impact functional performance and safety. Pt remains highly dependent on MAX A for task initiation, regulation, and participation.
