She was accompanied to the evaluation by her mother, who served as the primary historian. Per parent report, Braelyn was born full term via Cesarean section. She did not require a NICU stay and was discharged home shortly after birth without complications. Her mother reported that all developmental milestones, including walking and talking, were delayed. Braelyn recently began attending the 4-year-old program at Tierra del Sol, where she qualified for school-based speech and language services. To stay ahead of this change and help prevent any interruptions in your child’s therapy, we would like to know how you would like to proceed. The options to proceed would be to remain with Dr. Patel and switch to a different insurance plan, or remain with Superior Health and transition to a different pediatrician. If you choose to stay with Dr. Patel and switch insurance plans, we will need a new referral for all services under the new insurance, as well as a copy of your new insurance ID card so we can verify benefits. If you choose to switch pediatricians and remain with Superior Health, we will need a new referral for all services from the new primary care provider (PCP). Please contact the front office as soon as possible so we can help coordinate this change and ensure there are no gaps in your child’s therapy services. Thank you for allowing us to serve your child!
